GUIDELINES FOR DIAGNOSING AND TREATING ACUTE ENCEPHALITIS IN CHILDREN

1. Overview
Acute encephalitis is an acute infectious disease of the central nervous system. There are many causes of encephalitis, mainly viral.
Depending on the type of virus, the disease can be transmitted through mosquito bites (for Japanese encephalitis), respiratory routes, or digestive routes...
The disease usually occurs in young children of different ages. It often starts acutely, progresses severely, and may lead to rapid death or leave serious sequelae.
2. Epidemiology: The disease appears sporadically throughout the year in many different regions. The number of cases tends to increase during hot weather from March to August each year. Japanese encephalitis:
Circulates in almost all provinces and cities.
Appears sporadically throughout the year, often occurring in outbreaks in May, June, and July. Transmission is via mosquito bites.
The disease affects all age groups, most commonly in children aged 2 to 8 years.
Other viral encephalitides such as:
Enterovirus (Virus of the Intestine):
Occurs throughout the year, often in March, April, May, and June.
Transmission is via the digestive tract.
Commonly seen in young children.
Herpes Simplex Virus:
Usually Herpes Simplex Type 1 (HSV1).
Occurs sporadically throughout the year, transmission is via respiratory routes.
Commonly seen in children over 2 years old.

Based on:

Clinical symptoms and progression
Ancillary tests
Excluding diseases with neurological manifestations but not encephalitis.
3.1. Epidemiological factors:
Based on age, season, place of residence, and the number of cases in the same period. 3.2. Clinical 3.2.1. Initial stage:
Fever: Fever is a common symptom, occurring suddenly, continuous fever at 39-40°C, but sometimes it may not be high.
Headache, crying, irritability, decreased responsiveness.
Nausea, vomiting.
Other possible symptoms include:
Diarrhea, loose stools without mucus or blood.
Cough, runny nose.
Rash: red spots, blisters, or petechiae on hands and feet accompanied by mouth ulcers (seen in encephalitis caused by Enterovirus 71).
3.2.2 Full-stage: After the initial stage, neurological manifestations quickly appear:
Disordered consciousness ranging from mild to severe, such as drowsiness, lethargy, confusion, up to coma.
Often has seizures.
Other neurological signs may be present: meningeal syndrome, localized neurological signs (hemiplegia or quadriplegia), increased or decreased muscle tone...
Respiratory failure or shock may occur.
3.2.3 Clinical forms:
Critical form: Continuous high fever, deep coma, respiratory failure, circulatory collapse, leading to rapid death.
Acute form: Rapid progression with severe and typical clinical manifestations.
Mild form: Mild disordered consciousness and quick recovery.
3.3. Ancillary tests:
Cerebrospinal fluid:
Clear, normal or increased pressure.
Normal or increased cells, from several dozen to several hundred/ml, mainly mononuclear cells.
Protein normal or slightly increased under 1g/l.
Glucose normal.
Do not perform lumbar puncture in cases with increased intracranial pressure, shock, or severe respiratory failure. Blood:
Blood formula: slight increase or normal white blood cell count.
Malaria parasites negative.
Electrolytes and blood sugar usually within normal limits. Specific cause tests (applied in facilities with conditions):
ELISA test of cerebrospinal fluid or serum to detect IgM antibodies.
PCR amplification reaction in cerebrospinal fluid.
Viral isolation from cerebrospinal fluid, blood, skin vesicles, throat swabs, stool.
Viral isolation from brain tissue in deceased patients.
Other tests:
Electroencephalogram Computed tomography (CT) or magnetic resonance imaging (MRI) of the brain
(if conditions permit)
3.4. Differential diagnosis:
Seizures due to high fever
Purulent meningitis
Tuberculous meningitis
Acute poisoning Malaria cerebral form
Subarachnoid hemorrhage
Epilepsy
Hypoglycemia
Metabolic and electrolyte disorders
4. Treatment
Viral encephalitis is a serious disease with a high risk of death and sequelae, requiring early treatment.
Currently, there is no specific treatment except for herpes simplex encephalitis. Therefore, symptomatic and supportive treatment is primarily important.
Principles of treatment:
Lowering fever
Anti-seizure
Ensuring ventilation, preventing respiratory failure
Reducing cerebral edema
Adjusting water, electrolyte, and blood sugar disorders (if present)
Ensuring circulation
Ensuring care and nutrition
Treating the cause
Early rehabilitation
4.1. Lowering fever
Give the child enough water, loosen clothing and diapers, and cool down.
If fever exceeds 38.5°C, use Paracetamol 15mg/kg/dose orally or rectally (can repeat every 6 hours, up to 4 times a day if fever persists).
4.2. Anti-seizure:
Diazepam: Use one of the following methods:
Intramuscular injection: dose 0.2 - 0.3 mg/kg
Intravenous injection: dose 0.2 - 0.3 mg/kg, slowly injected intravenously. (Only performed in facilities with resuscitation conditions because it may cause respiratory arrest).
Rectal administration: dose 0.5 mg/kg
Method:
Draw the calculated amount of drug (based on weight) from the Diazepam syringe into a 1 ml syringe. Then remove the needle.
Insert the syringe into the rectum 4-5 cm and inject the drug.
Hold the child's buttocks for a few minutes. If seizures continue after 10 minutes, give a second dose of Diazepam.
If seizures continue, give a third dose of Diazepam, or Phenobarbital (Gardenal) 5 - 8 mg/kg/24 hours, divided into three doses, administered intramuscularly.
4.3. Ensuring ventilation, preventing respiratory failure

지시사항: 경련, 호흡 부전, 산소 포화도 SaO2가 92% 미만(측정 가능할 경우)
방법: 1-3 리터/분의 양으로 산소를 통나무 마스크나 튜브를 통해 흡입한다.
인공호흡관 삽입 및 인공호흡기 사용:
지시사항: 호흡 중단 또는 호흡 정지가 발생하거나 산소 흡입이 실패한 경우
방법:
인공호흡기 사용 시 초기 설정값:
모드: 볼륨 제어형 인공호흡
FiO2 초기 값: 100%
흘러가는 공기량(Tidal Volume, TV): 10-15 ml/kg
호흡 주파수: 1세 미만: 25회/분; 1-5세: 20회/분; 5세 이상: 15회/분.
흡기/호기 비율(I/E): 1/2
PEEP 설정: 4cmH2O.
임상 반응, 산소 포화도(SaO2), 혈중 가스 분석 결과에 따라 설정 값을 조정한다.
인공호흡관 삽입이 불가능한 경우에는 마스크를 통해 볼륨 팅기로 호흡을 돕는다. 호흡기 없이 볼륨 팅기를 사용하는 경우 내기관 관을 통해 볼륨 팅기를 실시하며, 팅기 주기는 20-30회/분으로 한다.
환자가 자발적으로 호흡하고 경련이 멎으며 혈역학적 상태가 안정되고 폐 X선 검사와 혈중 가스 분석 결과가 정상이며 FiO2가 40% 미만이고 PEEP가 4cmH2O 이하일 때 인공호흡기를 중단하고 인공호흡관을 제거한다.
4.4. 뇌부종 치료
지시사항: 뇌부종 징후가 있을 때 두통과 함께 자극 증상, 혼란, 무의식 상태, 시신경 부종, 동공 크기 불균형, 국소 마비, 근육 경직, 호흡 불균형, 천박한 심박동, 고혈압 등이 나타날 때 빈번하게 기도를 청소한다.
방법:
자세: 머리 고도 30도
산소 흡입. 인공호흡기 사용 시 통기량을 늘리고 PaO2를 90-100mmHg, PaCO2를 25-35mmHg로 유지한다.
만니톨 20% 용액: 0.5g/kg(2.5ml/kg)를 정맥주사하여 15-30분간 주사한다. 뇌부종 징후가 계속될 경우 8시간 후 다시 주사할 수 있으나 하루 3회 초과하지 않으며 3일 이상은 주사하지 않는다. 만니톨 사용 금지 쇼크, 폐부종 상태에서는
주사 중 임상적 변화와 전해질 변화를 관찰하여 과부하와 전해질 불균형 징후를 확인한다.
4.5. 전해질 및 혈당 조절(있을 경우)
충분한 수분과 전해질 보충이 필요하다. 폐부종 징후가 있을 경우 주의해야 한다.
동등한 등장성 염분 용액 사용. 주사량은 체중에 따른다.
전해질 및 산염기 균형 조절은 전해질 검사와 혈중 가스 분석 결과를 바탕으로 한다.
4.6. 쇼크 치료:
쇼크 상태가 있을 경우 다포민을 정맥주사하여 시작량 3-5 μg/kg/min으로 주사하고 점진적으로 증가시키되 최대 10 μg/kg/min을 넘지 않아야 한다.
4.7. 4.7. 영양 보충 및 재활 치료
4.7.1. 4.7.1. 영양 보충
소화하기 쉬운 음식, 고 에너지, 충분한 미네랄 및 비타민을 제공한다. 에너지 공급은 일일 50-60 kcal/kg을 보장한다.
모유 수유를 보장한다. 수유가 어려운 경우 어머니의 유즙을 짜내어 작은 숟가락으로 주거나 위관을 통해 주입한다(여러 번 나누어 주거나 연속적으로 주입). 주의해야 할 점은 질식과 역류 증상을 유발할 수 있다.
스스로 먹을 수 없는 경우 위관을 통해 음식을 주거나 정맥영양을 실시한다. 비타민 C와 비타민 B군을 추가로 제공할 수 있다.

중등도 이상의 사례와 경증 사례 중 뇌척수액 검사를 받을 수 없는 사례를 상위 단계로 이송한다.
5.3. 시 단위:
모든 급성 뇌염 사례를 진료 및 치료한다.
전문적인 진단 및 치료 능력이 부족한 중등도 이상의 사례만 상위 단계로 이송한다.
5.4. 중앙 단위:
모든 급성 뇌염 사례와 하위 단계에서 올라온 사례를 진료 및 치료한다.
하위 단계에서 진단 및 치료를 지도하고 지원한다.
원인 진단을 위한 검사를 수행한다.
6. 질병 예방
6.1. 예방 보건
개인 위생, 모기로부터 보호하기 위해 모기帳 사용.
식사 위생을 통해 소화기 경로를 통해 질병 전파를 피한다.
외부 환경 위생, 가축을 주택이나 주거 지역 근처에 기르지 않는다. 벌레, 매개체 파괴, 벌레 유충, 모기 파괴.
6.2. 예방 접종:
6.2.1. 일본뇌염 예방 접종:
피하 주사;
용량: 5세 미만 어린이 0.5ml, 5세 이상 어린이 1ml
