THE MINISTER OF JUSTICE

Pursuant to the Legal Aid Law;

Pursuant to Decree No. 07/2007/NĐ-CP dated January 12, 2007, issued by the Government, detailing and guiding the implementation of certain provisions of the Legal Aid Law;

Pursuant to Decree No. 62/2003/NĐ-CP dated June 6, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Justice;

At the proposal of the Director of the Legal Aid Agency.

DECISION:

Article 1. Issuing and uniformly applying nationwide the Legal Aid Practitioner Card model (Model TP-TGPL-1) and the Legal Aid Professional Training Certificate model (Model TP-TGPL-2).

Article 2. Issuing for uniform application nationwide the forms regarding the participation in legal aid by law firms, legal counseling organizations of political-social organizations, social-political organizations, occupational organizations, and social-occupational organizations:

1. Application for Participation in Legal Aid (Model TP-TGPL-3A);

2. Application for Modification of Participation in Legal Aid (Model TP-TGPL-3B);

3. Registration for Participation in Legal Aid (Model TP-TGPL-3C);

4. Notice of Termination of Participation in Legal Aid (Model TP-TGPL-3D);

5. Decision on Revocation of Registration for Participation in Legal Aid (Model TP-TGPL-3Đ).

Article 3. Issuing for uniform application nationwide the forms for assigning persons to participate in legal aid proceedings:

1. Decision on Assignment of Legal Aid Practitioner/Collaborating Lawyer to Participate in Proceedings (Model TP-TGPL-4A);

2. Decision on Replacement of Legal Aid Practitioner/Collaborating Lawyer to Participate in Proceedings (Model TP-TGPL-4B).

Article 4. Entrusting the Legal Aid Agency with unified management and free distribution of the Legal Aid Professional Training Certificate and blank Legal Aid Practitioner Cards according to the models prescribed in this Decision.

Article 5. This Decision shall take effect fifteen days from the date of publication in the Official Gazette.

Article 6. Provincial People's Committees, the Director of the Legal Aid Agency, Heads of Units under the Ministry, Directors of Departments of Justice, Directors of State Legal Aid Centers in provinces and centrally-administered cities, and those concerned are responsible for implementing this Decision./.
