THE MINISTER OF HEALTH
Pursuant to Decree No. 188/2007/NĐ-CP dated December 27, 2007 of the Government on the functions, tasks, powers, and organizational structure of the Ministry of Health;
Pursuant to the Law on Donation, Collection, Transplantation of Tissues and Organs of the Human Body and Whole-Body Donation dated November 29, 2006;
At the proposal of the Director of the Department of Medical Examination and Treatment - Ministry of Health,
DECISION:
Article 1. The attached forms for voluntary donation, cancellation of registration for donation of tissues and organs from living persons, post-mortem, and whole-body donation; forms for registration cards for donation of tissues and organs from deceased persons and whole-body donation are hereby issued.
Article 2. The Standard Measurement Quality Control Department shall be responsible for organizing and guiding the implementation of the Regulations adopted herein.
Article 3. The Heads of the Ministry's Office, the Inspectorate of the Ministry, the Directors of Departments, the Heads of Bureaus under the Ministry of Health, the Directors of Provincial Health Services under the Central Government, hospitals directly under the Ministry of Health, and the Heads of Health Services of various sectors are responsible for implementing this Decision.
