Respected: DEPARTMENTS OF EDUCATION AND TRAINING

 

Following Circular No. 6025/THPT dated July 17, 2003 on management and issuance of high school and junior high school graduation certificates, the Ministry supplements certain regulations regarding the issuance of copies of graduation certificates as follows:

1.1. Issuance of copies of graduation certificates:

 - As of September 2003, the Ministry of Education and Training will no longer issue certificates of completion instead of graduation certificates. In cases where graduation certificates are lost, damaged due to unexpected risks from all academic years, or requests for additional copies beyond those stipulated in Circular No. 6025/THPT dated July 17, 2003, copies of graduation certificates will be issued.

The applicant requesting issuance of a copy of a graduation certificate (including cases based on the original issuance record book or final grade registration sheet) must have the following complete dossier as prescribed below:

+ An application for issuance of a copy with confirmation of the loss of the original certificate, damage to the original certificate, or loss of a copy, issued by the local authority or police station.

+ Confirmation of having been issued a graduation certificate or having graduated from the school where the student completed their final year.

+ The original student record book of the secondary education level.

The Department of High School Education or the Examination Department is responsible for verifying the accuracy of the dossier before issuing a copy.

- The serial number of the copy shall be determined by the Department of Education and Training for the locality under its jurisdiction.

- The fee for issuing a copy shall be approved by the People's Committee at the provincial level upon submission by the Department of Education and Training.

1.2. Record of issuance of graduation certificates:

Starting from the 2002-2003 academic year, all high schools and junior high schools must strictly follow the regulations when recording the issuance of graduation certificates to students according to the model issued by the Ministry of Education and Training. Upon receipt at the Ministry of Education and Training Office, each volume will be reduced by 4,000 VND for circulation fees.

Upon receipt of this circular, the Departments of Education and Training shall immediately notify all secondary schools and all students. Any issues encountered during implementation should be reported promptly to the Ministry (High School Education Department).
