This document corrects certain errors in Circular No. 59/2003/TT-BTC regarding Form Appendix No. 6, aiming to adjust the provisions to be applicable at the district and urban district level rather than only at the provincial and municipal level. This document is sent to People's Committees of provinces and cities for implementation.

- People's Committees of provinces and centrally governed cities must amend Form No. 34 in Appendix No. 6 of Circular No. 59/2003/TT-BTC from "(For use by the People's Committee of the province or centrally governed city to submit to the People's Council at the same level)" to "(For use by the People's Committee of the district, urban district, town, or city within the province to submit to the People's Council at the same level)".
- The Ministry of Finance requests People's Committees of provinces and cities to direct the implementation of the amendments as prescribed.
- Provincial and centrally governed city Departments of Finance also need to pay attention and organize the implementation of the amendments.
- The State Budget Department of the Ministry of Finance is responsible for storing this document.

Respectfully submitted to: People's Committees of provinces and centrally governed cities
The Ministry of Finance has issued Circular No. 59/2003/TT-BTC dated June 23, 2003 guiding the implementation of Decree No. 60/2003/NĐ-CP dated June 6, 2003 of the Government detailing and guiding the implementation of the Law on State Budget; due to errors in some forms in Appendix No. 6, the Ministry of Finance requests the following amendments:
- In Form No. 24 - Appendix No. 6, the provision: "Province (city): ..." is now amended to read: "District (urban district, town, city): ...".
- In Form No. 34 - Appendix 6, the provision: "("For submission by the People's Committee of the province, centrally governed city to the People's Council at the same level)" is now amended to read: "("For submission by the People's Committee of the district, urban district, town, city under the province to the People's Council at the same level)”.
The Ministry of Finance requests the People's Committees of provinces and centrally governed cities to direct relevant departments and lower-level local authorities to implement these amendments accordingly./.
