Regarding the printing, issuance, and management of permits for the use of various types of machines, equipment, materials, and substances with strict requirements for labor safety

Pursuant to Decree No. 15/CP dated March 2, 1993 of the Government on the tasks, powers, and responsibilities of State management by Ministries and ministerial-level agencies,

THE MINISTER OF LABOR, WAR INVALIDS AND SOCIAL AFFAIRS

Pursuant to the Government Organization Law dated September 30, 1992;

Pursuant to Articles 96, 185, and 186 of the Labor Code dated June 23, 1994;

Considering the proposal of the Chairman of the National Inspectorate for Labor Safety,

Pursuant to Decree No. 96/CP dated December 7, 1993 of the Government on the functions, tasks, powers, and organizational structure of the Ministry of Labor - Invalids and Social Affairs;

Now hereby promulgating along with this Decision the model Permit for the use of machines, equipment, and materials (referred to as objects) with strict requirements for labor safety throughout the country including:

Article 1. - Permit for the use of pressure-bearing equipment;

- Permit for the use of lifting equipment;

- Permit for the use of elevators;

- Permit for the use of escalators;

The permit models have dimensions of 15 x 21 cm, consisting of two pages, with a white background, blue wave patterns, and an embossed national emblem in the center along with a code number; the phrase "Permit for the use of…" is printed in bold red font.

Article 2. Entrusting the National Inspectorate for Labor Safety under the Ministry of Labor, Invalids, and Social Affairs to issue and uniformly manage the permit models, including printing, defining serial numbers, and distributing the permit models to the National Inspectorates for Labor Safety under the Departments of Labor, Invalids, and Social Affairs.

Article 3. The distribution of permit models shall be charged (including the cost of printing the permit models and transportation fees).

The National Inspectorates for Labor Safety under the Departments of Labor, Invalids, and Social Affairs shall implement the issuance of permits using the permit models distributed by the National Inspectorate for Labor Safety under the Ministry of Labor, Invalids, and Social Affairs.

Article 4. Ministries, sectors, People's Committees of provinces and centrally governed cities shall direct and urge entities under their jurisdiction to implement this Decision.
