Pursuant to the Vocational Training Law dated November 29, 2006;
Pursuant to Decree No. 29/2003/NĐ-CP dated March 31, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Labor, Invalids and Social Affairs;
At the proposal of the General Director of the Vocational Training General Department.
DECISION:
Article 1. This Decision promulgates templates for vocational certificates and diplomas to be issued to graduates of corresponding vocational training courses, including:
1. Template for a Higher Vocational Diploma;
2. Template for a Intermediate Vocational Diploma;
3. Template for a Basic Vocational Certificate.
Article 2. The template for a Higher Vocational Diploma is specified as follows:
1. A Higher Vocational Diploma consists of four pages: pages 1 and 4 are covered with dark red plastic; pages 2 and 3 have a white background, with light yellow bronze patterns, and the National Emblem is embossed in light red at the center of page 3.
2. Specific contents according to the template for a Higher Vocational Diploma at Appendix I.
3. Contents printed on page 1 are in yellow. The phrase "VOCATIONAL DIPLOMA OF HIGHER VOCATIONAL TRAINING" on page 3 is printed in bright red, while other contents on pages 2 and 3 are printed in black.
Article 3. The template for a Intermediate Vocational Diploma is specified as follows:
1. An Intermediate Vocational Diploma consists of four pages: pages 1 and 4 are covered with green plastic; pages 2 and 3 have a white background, with light yellow bronze patterns, and the National Emblem is embossed in light red at the center of page 3.
2. Specific contents according to the template for an Intermediate Vocational Diploma at Appendix II.

Article 4. The template for a Basic Vocational Certificate is specified as follows:
1. A Basic Vocational Certificate consists of four pages: pages 1 and 4 are covered with blue plastic; pages 2 and 3 have a white background, with light yellow bronze patterns, and the National Emblem is embossed in light red at the center of page 3.
2. Specific contents according to the template for a Basic Vocational Certificate at Appendix III.
3. Contents printed on page 1 are in yellow. The phrase "BASIC VOCATIONAL CERTIFICATE" on page 3 is printed in bright red, while other contents on pages 2 and 3 are printed in black.
Article 5. Templates for a Higher Vocational Diploma, an Intermediate Vocational Diploma, and a Basic Vocational Certificate as stipulated in Article 2, Article 3, and Article 4 of this Decision shall be uniformly applied throughout the country.
Vocational training institutions are permitted to print their own symbols on page 2 of the diplomas and certificates in the upper right corner after registering copyright with the competent state management agency for intellectual property rights.
Article 6. The head of vocational training institutions shall be responsible for:
1. Organizing the printing of vocational diplomas and certificates according to the templates prescribed in this Decision.
2. Organizing the issuance of vocational diplomas and certificates to vocational learners in accordance with their authority and as stipulated in Articles 16, 23, and 30 of the Vocational Training Law.
3. Maintaining records of blank diploma and certificate forms, records of issued Higher Vocational Diplomas, records of issued Intermediate Vocational Diplomas, and records of issued Basic Vocational Certificates.
4. Recording all contents in the diplomas and certificates accurately and completely.
5. Issuing decisions and implementing the revocation of diplomas and certificates in the following cases:
a) The person who received the diploma or certificate is found to have engaged in fraudulent behavior during study, examination, admission, or in falsifying documents to obtain the diploma or certificate;
b) Diplomas or certificates issued to persons who do not meet the requirements;
c) Diplomas or certificates issued by persons without authority;
d) Diplomas or certificates that have been erased or altered;
e) The person who received the diploma or certificate allows another person to use it.
6. Reporting periodically every quarter and annually on the issuance of vocational diplomas and certificates to the Department of Labor, Invalids and Social Affairs where the vocational training institution's main office is located for consolidation and reporting to the Vocational Training General Department.
Article 7. Vocational diplomas and certificates issued pursuant to Decision No. 1012/1998/QD-BLDTBXH dated October 9, 1998 of the Minister of Labor, Invalids and Social Affairs shall continue to be used to issue to graduates of long-term and short-term vocational training programs that were admitted and organized before the Vocational Training Law came into effect.
Article 8. Responsibilities of the Vocational Training General Department and Departments of Labor, Invalids and Social Affairs
1. The Vocational Training General Department shall be responsible for guiding, inspecting, and auditing the issuance of vocational diplomas and certificates, consolidating the issuance of vocational diplomas and certificates nationwide, and reporting to the Ministry of Labor, Invalids and Social Affairs.
2. Departments of Labor, Invalids and Social Affairs shall be responsible for guiding, inspecting, and auditing the issuance of vocational diplomas and certificates, consolidating the issuance of vocational diplomas and certificates of all vocational training institutions within their jurisdiction, and reporting to the Vocational Training General Department quarterly.
Article 9. The Standard Measurement Quality Control Department shall be responsible for organizing and guiding the implementation of the Regulations adopted herein.
Article 10. Ministers, Heads of ministerial-level agencies, agencies under the Government, Heads of central organizations of political and social affairs; Chairpersons of provincial People's Committees directly under the Central Government, Directors of Departments of Labor, Invalids and Social Affairs; Heads of the Office of the Ministry, General Directors of the Vocational Training General Department; Principals of vocational colleges, intermediate vocational colleges, Directors of vocational training centers; Heads of universities, colleges, intermediate vocational colleges, other educational institutions, enterprises, cooperatives, production, business, service establishments registered for vocational training activities; Heads of related agencies and units are responsible for implementing this Decision.
