Pursuant to Decree No. 49/2003/NĐ-CP dated May 15, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Health;
Pursuant to Decision No. 43/2000/QĐ-TTg dated April 7, 2000 of the Prime Minister on mobilizing and encouraging people to donate blood voluntarily;
Pursuant to Decision No. 198/2001/QĐ-TTg dated December 28, 2001 of the Prime Minister approving the Blood Safety Program;
Pursuant to Circular Joint No. 12/2004/TTLB-BYT-BTC of the Ministry of Health and the Ministry of Finance dated February 25, 2004 guiding the price for one unit of standard blood;
At the proposal of the Director of the Department of Treatment - Ministry of Health,
DECISION:
Article 1. Now hereby promulgated together with this Decision:
1. Regulations on the issuance, use, and management of voluntary blood donation certificates.
2. Model of voluntary blood donation certificate.
Article 2. The Standard Measurement Quality Control Department shall be responsible for organizing and guiding the implementation of the Regulations adopted herein.
Article 3. The Head of the Office, Directors of the Departments of Treatment, Legal Affairs, Planning and Finance, Director of the Central Institute of Hematology and Blood Transfusion, Heads of related units shall be responsible for implementing this Decision.

REGULATIONS
Issuance, management, and use of voluntary blood donation certificates
(issued together with Decision No. 1995/2004/QĐ-BYT dated June 4, 2004 of the Minister of Health)
I. GENERAL PROVISIONS
1. The voluntary blood donation certificate issued together with this Decision shall be uniformly used in public health facilities with the function of collecting blood, to be issued to voluntary blood donors in accordance with Circular Joint No. 12/2004/TTLB-BYT-BTC dated February 25, 2004 of the Ministry of Health and the Ministry of Finance regarding guidance on the price for one unit of standard blood.
2. The voluntary blood donation certificate aims to honor the noble act of voluntary blood donors.
3. The voluntary blood donation certificate has the value of free blood transfusion when the voluntary blood donor needs a blood transfusion at public health facilities nationwide.
II. SPECIFIC PROVISIONS
1. The Central Institute of Hematology and Blood Transfusion shall be responsible for: Organizing printing, managing, and distributing voluntary blood donation certificates to public health facilities with the task of collecting, screening, and storing blood nationwide.
2. Blood collection facilities shall be responsible for:
a) Annually, blood collection facilities shall coordinate with the Blood Donor Mobilization Committees of provinces and centrally-administered cities to develop plans on the number of voluntary blood donation certificates needed, send them to the Central Institute of Hematology and Blood Transfusion, and collect certificates from the institute.
b) Directly issue voluntary blood donation certificates to donors immediately after receiving blood at voluntary blood donation points. Ensure that all information on the certificate is fully recorded, handwriting must be clear and neat, without erasures.
c) The issuance of voluntary blood donation certificates to donors must be conducted openly and ensure the correct recipients. Each voluntary blood donation will result in the issuance of one voluntary blood donation certificate.
d) Health facilities shall be responsible for maintaining records of the issuance of voluntary blood donation certificates according to the serial number printed on the certificate, report to the provincial Blood Donor Mobilization Committee about the list of voluntary blood donors, the amount of collected blood, and the number of issued certificates.
3. Provincial Blood Donor Mobilization Committees shall be responsible for: Managing the quantity of voluntary blood donation certificates and the list of donors who have received certificates, and sending the list of donors who have donated blood, the corresponding amount of blood, and the serial number of the issued certificates to the Central Institute of Hematology and Blood Transfusion for unified national management.
4. Public medical facilities shall be responsible for:
a) Providing free blood transfusions to voluntary blood donors when they need it, with the maximum amount of free blood transfusion equal to the amount of blood donated as recorded on the voluntary blood donation certificate.
b) After the blood transfusion, the health facility shall be responsible for signing and stamping the back of the certificate (reserved for health facilities).
5. Voluntary blood donors shall be responsible for: preserving and protecting the certificate, ensuring it is not torn, erased, and presenting the certificate when they need a free blood transfusion at public health facilities.
6. The voluntary blood donation certificate loses its value for free blood transfusion when the donor has already received a free blood transfusion equivalent to the amount of blood donated (confirmed by the health facility on the certificate) or if the certificate is torn or erased.
7. Some important notes: Address: Clearly state the permanent address of the voluntary blood donor.
Blood receiving unit: Clearly record the name of the health facility collecting the blood.
Quantity: Mark (x) in the blank box corresponding to the amount of collected blood.
