THE MINISTER OF HEALTH

Pursuant to Decree No. 75/2017/NĐ-CP dated June 20, 2017 of the Governmentand project management units managing one construction project in accordance with current laws on project management for construction investment (excluding project management units that are member organizations of enterprises);stipulating the functions, tasks, powers, and organizational structure of the Ministry of Health;Deputy ministers of ministerial-level agencies, The Preventive Medicine Department, Ministry of Health,

3. Emphasizing the responsibility of party committees at all levels and heads of public security units and localities.inhereby issues the “Guidelines for Event-Based Surveillance” along with this Decision.

DECISION:

Article 1. The “Guidelines for Event-Based Surveillance” are guiding documents applied in preventive healthcare facilities and public and private medical examination and treatment establishments nationwide.

Article 2. This Decision takes effect from the date of signature and issuance.

Article 3. The following officials are responsible for implementing this Decision: Director of the Ministry's Office; Director of the Ministry's Inspectorate; Heads of Departments, Directors of Bureaus, General Directors of General Bureaus under the Ministry of Health; Directors of Hygiene and Epidemiology Institutes/Pasteur Institutes; Director of the Central Health Communication and Education Center; Directors of medical examination and treatment establishments directly under the Ministry of Health; Directors of Provincial Health Services, Directors of Preventive Medical Centers/Disease Control Centers, Health Communication and Education Centers, International Health Quarantine Centers, medical examination and treatment establishments in provinces and centrally-administered cities; Heads of health services of ministries and sectors; Heads of related units.

Implementing the provisions of the International Health Regulations Charter (IHR 2005) regarding the detection, warning, reporting, and response to public health events, the World Health Organization (WHO) recommends that countries establish an early warning system - rapid response system (EWAR) and guide the implementation of indicator-based surveillance (IBS) and event-based surveillance (EBS) to operate the early warning system, rapid response system while providing data for monitoring, evaluation, and assessing disease outbreaks and public health events. The implementation of event-based surveillance along with indicator-based surveillance will enhance the role of early detection of diseases in communities and healthcare facilities, and strengthen coordination between preventive and treatment sectors, helping to detect early signs and risks of disease outbreaks and respond promptly and effectively.

CONCEPT AND OBJECTIVES OF EVENT-BASED SURVEILLANCE

1. Concept

In this guidance document, the terms are understood as follows:

Warning signs are initial information about infectious diseases, risks of infectious diseases, or risks affecting community health.

Event is a warning sign that has been verified to pose a risk of infectious disease or a risk affecting community health.

Surveillance based on events It is the process of identifying, recording, screening, verifying warning signs, evaluating, and recommending responses to events that pose a risk of causing infectious diseases or affecting community health.

Clause 2. Objectives

2.1 General Objective

Early detection of events that pose a risk of causing infectious diseases or affecting community health to enable timely and appropriate responses, thereby minimizing harm to the community.

2.2 Specific Objectives

2.2.1 Early detection of warning signs, events that pose a risk of causing infectious diseases or affecting community health.

2.2.2 Collection, tracking, evaluation, analysis of information about events, and proposing timely and appropriate solutions to address infectious disease outbreaks or events that may affect community health.

PART III

CONTENT AND PROCEDURES FOR SURVEILLANCE

1. Warning Signs to Monitor

1.11.1 Warning Signs from the CommunityJune 2024;1.1.1 One case of a child under 15 years old suddenly becoming weak or paralyzed.

1.1.2 One case of fever, rash, accompanied by cough or conjunctivitis.

1.1.3 One case of severe illness requiring hospitalization or death with one of the following symptoms:

a. Diarrhea three times or more within 24 hours, watery stool, signs of dehydration in individuals aged five years or older.

b. Individuals returning from abroad with respiratory tract infection and fever within 14 days.

c. Respiratory tract infection and fever within 14 days after contact with poultry.

d. Illness (disease) within seven days after vaccination.

e. Unusual or rare disease or symptoms not previously seen in the community.

f. Death of unknown cause.

1.1.4 Two or more cases hospitalized or died within seven days in the same residential area, school, workplace, with similar symptoms.

1.1.5 An unusual increase in the number of any of the following signs:

a. Students absent from school due to the same illness within seven days at the same school.

b. People purchasing fever-reducing medication, cough medicine, or diarrhea treatment medication within one week from pharmacies in the same residential area.

c. People falling ill simultaneously with similar symptoms in the same residential area.

d. Sick or dead poultry, livestock, or other animals.

1.1.6 Discovery:

a. Suspected rabid dog.

b. Dog falling ill and biting people.

c. A dog biting two or more people within ten days.

1.2 Warning Signs from Healthcare Facilities, Vaccination Sites

1.2.1 Healthcare workers falling severely ill and requiring hospitalization after contact with patients with similar symptoms.Deputy ministers of ministerial-level agencies,energy

1.2.2 Two or more cases of severe acute respiratory infection within seven days in the same residential area, household, school, or workplace.

1.2.3 Hospitalization due to suspected viral pneumonia.

1.2.4 A sudden increase in the number of cases with similar symptoms, based on the professional assessment of medical personnel.

1.2.5 Two or more cases of infectious disease with similar symptoms originating from the same location (household, community group, school, factory, etc.).

1.2.6 Cases of malaria in areas where malaria has been eliminated or was previously non-endemic.

1.2.7 Unusual clinical manifestations or unexplained treatment responses of a known infectious disease based on the professional assessment of a physician.

1.2.8 One or more cases of unusual or unexplained deaths from unknown diseases based on the professional assessment of a physician.

1.2.9 An unusual increase in the number of people vaccinated against rabies in the same residential area.

1.2.10 Any suspected Group A infectious disease case according to the Law on Prevention and Control of Infectious Diseases (2007).

1.3 Warning Signs from Laboratory

1.3. Domestic air passenger transport service on regular basic economy class1.3.1 Pathogens not detected for a long time (based on laboratory staff's assessment). Community1.3.2 A sudden increase in the number of specimens with the same testing request or positive for the same pathogen (including antibiotic-resistant pathogens) within seven days.

1.3.3 Any specimen positive for Group A infectious disease pathogens according to the Law on Prevention and Control of Infectious Diseases (2007).

1.3.4. Detecting a new strain of influenza virus or a strain that cannot be typed from a patient with severe acute respiratory infection or severe pneumonia suspected to be caused by a virus.

1.3.5. A new agent or one that has not been previously identified.

2. Sources of information on warning signs

Warning signs can be detected or recorded from various sources of information, which may be within or outside the healthcare sector, related to humans or not, formal or informal.

3.1. Steps for event-based surveillance

- Community members, members of social organizations (women's unions, farmers' unions, veterans' associations, youth unions...)

- Local leaders (authorities, neighborhood people's committees)

- Traditional healers

- Health workers at village and commune levels

- Health and social workers

- Private clinics

- Pharmacies

- Educational and training institutions

- Animal health agencies

- Companies, factories, production and business establishments, industrial zones

- Food safety agencies

All healthcare units at all levels are responsible for detecting, recording, and reporting information according to the form provided in Appendix lục 1 regarding warning signs from the community, healthcare and treatment facilities, vaccination centers, laboratories, and the network of communication and information from the internet and social media. The information to be reported includes:

- Content of the warning sign

- Time of occurrence and time of detection

- Location of occurrence

- Number of cases/deaths/hospitalizations or potential spread

- Other relevant information (if applicable)shall)

3.1Step 2 - Screening warning signsn lealth Community°

Screening warning signs aims to identify appropriate information to minimize unnecessary verification and investigation. Screening warning signs is conducted by answering the following questions:

- Is the warning sign included in the list of warning signs according to the guidelines?

- Has the warning sign actually occurred?

- Is there any duplication of the warning sign?

If the screened warning sign is included in the list according to the guidelines, has actually occurred, and is not duplicated (not yet recorded), it will move to Step 3 - Verifying warning signsn lealth bon to determine the risk of causing infectious diseases or affecting public health.

If the screened warning sign is not included in the list according to the guidelines, has not actually occurred, or is duplicated (already recorded), monitoring activities for that warning sign will be discontinued.

3.1Step 3 - Verifying warning signsn lealth Community°

Verifying warning signs aims to determine whether the warning sign poses a risk of causing infectious diseases or affecting public health. After verification, warning signs that pose a risk of causing infectious diseases are considered events. Verification is carried out through the following activities:

- Verifying through local health authorities, treating doctors, and/or interviewing patients or their families

- Contacting original data and information sources

- Cross-checking with other information sources

- Collecting additional necessary information related to the warning sign.

Step 4 Evaluating the event - to issue risk warnings and propose appropriate response activities.price In case the warning sign is verified as not posing a risk of causing infectious diseases or affecting public health, monitoring activities for that warning sign will be discontinued. Step 4 - Evaluating the event

Evaluating the event is a continuous process of synthesis and analysis aimed at determining the risk and impact of an event on public health. The results of evaluating the event serve as the basis for issuing risk warnings and proposing appropriate response activities. Depending on the level of impact of the event, evaluation may be jointly implemented at all levels and by related units. Below are some questions to help with the evaluation of the event:

- Is the event related to priority diseases?

- Does the suspected disease have a high potential for transmission?

- Is there a higher than expected mortality or morbidity rate?

- Is this an unusual or unexpected event in the community?

- Are there clusters of similar symptomatic cases?

- Does the disease pose a risk to trade or tourism?

- Does the event pose a risk to public health?

- Can it be easily controlled (based on local response capacity)?

- Is there a possibility of recurrence or prolongation?

Step 5 - Proposing responses to the event (organize credit institutions, foreign bank branches are responsible for organizing the implementation of this Circular.Proposals for responding to events should be made immediately after the event is evaluated as posing a risk of causing infectious diseases. Response activities will be carried out in accordance with current regulations, technical guidance from the Ministry of Health, and not mentioned in this document.

3.2. Regulations on implementing event-based surveillance

Depending on the impact level of the warning signs or events, the commune-level Health Station may request higher-level healthcare facilities to assist in verifying warning signs, conducting assessments, and implementing responses to events.

3.2.4. 3.2.5. District Health Center, Preventive District Health CenterpoliciesEmergency Operations Center

Upon detecting or recording warning signs, the district-level Health Center shall be responsible for filling out the warning sign recording form Annex lục 1); conducting screening, verifying warning signs; implementing assessment and response to events; and performing monthly event-based surveillance activity reports (Appendix 2) to the Provincial Preventive Health Center (PPHC)/Provincial Disease Control Center (PDCC) before the 10th day of the following month. Depending on the impact level of the warning signs or events, the district-level Health Center may request lower-level healthcare facilities to assist in verifying warning signs, conducting assessments, and implementing responses to events.

Depending on the impact level of the warning signs or events, the Hygiene and Epidemiology Institutes/Pasteur Institutes may request lower-level healthcare facilities to conduct screening, verify warning signs, conduct assessments, and implement responses to events. PART 4

IMPLEMENTATION

1. Preventive Medicine Department

- Advise the Minister of Health on the development and implementation of plans for event-based surveillance nationwide.

- Direct, urge, inspect, and monitor localities and units to implement event-based surveillance activities according to assigned plans.

- Maintain information screening activities from communication networks, the Internet, and social media at the Center for Public Health Emergency Response, Ministry of Health.
