ON THE ISSUE OF ISSUING SOCIAL INSURANCE FILE FORMS
THE MINISTER OF LABOR, WAR INVALIDS AND SOCIAL AFFAIRS
Pursuant to the Government Organization Law dated September 30, 1992;
Pursuant to Decree No. 96/CP dated December 7, 1993 of the Government on the functions, tasks, powers, and organizational structure of the Ministry of Labor, War Invalids and Social Affairs;
Pursuant to Decree No. 43/CP dated June 22, 1993; Decree No. 66/CP dated September 30, 1993 of the Government providing temporary regulations on social insurance benefits;
3. Workers without original records showing working time in the state sector before January 1, 1995, as stipulated in point b Clause 3 Article 2 of Decree No. 89/2020/NĐ-CP dated August 4, 2020 of the Government stipulating the functions, tasks, powers, and organizational structure of the Vietnam Social Security.

Article 1: The Ministry of Labor, War Invalids and Social Affairs hereby issues new forms attached hereto for the examination and enjoyment of social insurance benefits.
Article 2: The Department of Social Insurance shall be responsible for issuing and managing the forms prescribed in this Decision.
Article 3: The forms prescribed in this Decision shall take effect from January 1, 1994, all previous forms inconsistent with the forms prescribed in this Decision shall be abolished.
Article 4: The Director of the Office, the Head of the Department of Social Insurance, and the Directors of the Departments of Labor, War Invalids and Social Affairs shall be responsible for implementing this Decision.
