CIRCULAR

REGULATIONS ON THE SYSTEM OF HEALTH STATISTICAL REPORTING

Pursuant to the Law on Government Organization 2025;

Pursuant to the Law on Local Administration Organization 2025;

Pursuant to the Law on Statistics 2015 amended and supplemented in 2021;

Pursuant to Resolution No. 190/2025/QH15 dated February 19, 2025 of the National Assembly on handling certain issues related to the restructuring of state administrative bodies;

Pursuant to the Decree No. 94/2016/NĐ-CP dated July 1, 2016 of the Government detailing and guiding the implementation of certain provisions of the Law on Statistics;

Pursuant to Decree No. 42/2025/NĐ-CP dated February 27, 2025 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Health; At the proposal of the Director of the Department of Planning and Finance, The Minister of Health issues this Circular regulating the system of health statistical reporting.

Article 1. Scope of Regulation

This Circular regulates the system of health statistical reporting.

Article 2. Basic Statistical Indicators System for the Health Sector

The basic statistical indicators system for the health sector includes:

1. The list of basic statistical indicators for the health sector as specified in Appendix I attached hereto.

2. The contents of basic statistical indicators for the health sector as specified in Appendix II attached hereto.

Article 3. Primary Record Books and Statistical Reporting Forms

Primary record books and statistical reporting forms include:

1. Primary record books and guidance on recording as prescribed in Appendix III attached hereto.

2. Health statistical reporting form at provincial level (hereinafter referred to as provincial level) and guidance on recording as prescribed in Appendix IV attached hereto.

3. Central health statistical reporting form and guidance on recording as prescribed in Appendix V attached hereto.

Article 4. Periods of Statistical Reporting

The periods of statistical reporting are calculated according to the Gregorian calendar:

1. Monthly reporting period: From 00:00 on the first day of the month until 24:00 on the last day of the month.

2. Annual reporting period: From 00:00 on January 1st until 24:00 on December 31st of the reporting year.

3. Ad hoc statistical reporting: In cases where ad hoc statistical reporting is required to fulfill management requirements of the Ministry of Health, the requesting agency must submit a written request specifying the time, deadline, and criteria for statistical reporting.

Article 5. Health Statistical Reporting System

1. At commune, town, special administrative region (hereinafter referred to as commune level) and units within the province, centrally governed city (hereinafter referred to as provincial level):

a) Content of report: As specified in Appendix IV attached hereto;

b) Units submitting reports: Commune-level units assigned by the People's Committee of the commune; all units under the provincial and central levels, and private healthcare facilities located within the province;

c) Units receiving reports: The main unit assigned by the People's Committee of the province;

d) Deadline for submission: Five working days from the end date of the reporting period as stipulated in Article 4 of this Circular.

2. At provincial level:

a) Content of report: As specified in Appendix IV attached hereto;

b) Units submitting reports: The main unit assigned by the People's Committee of the province to aggregate;

c) Units receiving reports: The Department of Planning and Finance and relevant Departments and Bureaus of the Ministry of Health;

d) Deadline for submission: Twenty days from the end date of the reporting period as stipulated in Article 4 of this Circular.

4. At Central Level:

a) Content of report: As specified in Appendix V attached hereto;

b) Units submitting reports: Units directly under the Ministry of Health;

c) Units receiving reports: The Department of Planning and Finance and relevant Departments and Bureaus of the Ministry of Health;

d) Deadline for submission: Ten days from the end date of the reporting period as stipulated in Article 4 of this Circular.

Article 6. Responsibility for Implementation

1. Reporting Units:

a) Shall comply with full, accurate, and timely reporting as prescribed in this Circular and bear legal responsibility for the reported content;

b) Shall review, resubmit reports and related information upon request of the receiving unit.

2. Receiving Units:

a) Shall aggregate statistical information according to the prescribed reporting forms;

b) Shall request reporting units to review, resubmit reports and related information when necessary to verify the accuracy of the reported data;

c) Shall ensure the publication, use, and confidentiality of statistical information in accordance with the law.

3. People's Committees at all levels shall assign the main unit at the provincial level to collect, aggregate, and report statistical information within their jurisdiction as prescribed in Article 5 of this Circular.

4. The Department of Planning and Finance, Ministry of Health:

a) Shall be responsible for receiving, aggregating, and analyzing health sector statistical data;

b) Shall take the lead and coordinate with relevant units to develop a comprehensive health sector statistical reporting system to serve the collection of basic statistical indicators uniformly across the country;

c) Shall provide guidance and oversight on the implementation of laws on statistics, the statistical reporting system, and other matters related to the field of statistics.

5. The National Health Information Center shall be responsible for leading and coordinating with the Department of Planning and Finance and relevant units to implement information technology applications in the statistical work of the health sector, including activities:

a) Completing and implementing the Electronic Health Statistical Reporting Software;

b) Integrating and utilizing statistical data from specialized software applications and other administrative databases under its responsibility to serve the statistical activities of the Ministry of Health and national statistical activities as prescribed by law.

Article 7. Implementation Provisions

Article 32. Responsibilities of Agencies

2. This Circular shall cease to be effective from March 1, 2027.

3. During the period this Circular is effective, if the provisions regarding management authority and responsibilities in this Circular differ from those in Circulars issued by the Minister of Health before July 1, 2025, the provisions in this Circular shall apply.

4. Circular No. 32/2014/TT-BYT dated September 30, 2014 of the Minister of Health promulgating the list of basic health statistical indicators applicable at provincial, district, and commune levels shall cease to be effective from the date this Circular takes effect.

5. Abolish Point 3 of Part 1 and Point 3 of Part 2 of the Appendix attached to Circular No. 19/2025/TT-BYT dated June 15, 2025 of the Minister of Health on the division and decentralization of management authority at two levels in the disease prevention sector.

Article 8. Responsibility for Implementation

The following entities are responsible for implementing this Decision: Directors of the Office of the Ministry of Health, Heads of Departments, Directors of Bureaus, Heads of units under the Ministry of Health, Chairpersons of People's Committees of provinces and centrally governed cities, and related organizations and individuals.

Any difficulties encountered during implementation should be promptly reported to the Ministry of Health (Department of Planning and Finance) for consideration and resolution.
