Circular No. 26/2014/TT-BQP promulgates the List of Serious Diseases and the List of Diseases Requiring Long-Term Treatment for Implementation of Policies and Benefits in the Military.

CIRCULAR

Issuing the List of Serious Diseases and the List of Diseases Requiring Long-Term Treatment to implement policies within the Military
Pursuant to Decree No. 35/2013/NĐ-CP dated April 22, 2013 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of National Defense;

Pursuant to the Law on Legislative Documents of 2008;

Considering the proposal of the Director of the General Department of Health;

Issuing this Circular, the List of Serious Diseases and the List of Diseases Requiring Long-Term Treatment are hereby promulgated.

The Minister of National Defense hereby decides.

Clause 4 of Article 6The List of Serious Diseases and the List of Diseases Requiring Long-Term Treatment serve as the basis for management, treatment, care, and policy resolution for military personnel, civil servants, and employees under the management of the Military and individuals working in confidential organizations managed by the Military, as prescribed in Circular No. 157/2013/TT-BQP dated August 26, 2013 of the Minister of National Defense.

The Medical Appraisal Councils at various levels of the Ministry of National Defense shall appraise and conclude to determine whether a person suffers from a disease listed in the List of Serious Diseases and the disability rate of a person suffering from a disease listed in the List of Diseases Requiring Long-Term Treatment.

12/2025/TT-BNNMT dated June 19, 2025 issued by the Minister of Agriculture and EnvironmentMilitary health units at division level and equivalent or higher shall determine whether a person suffers from a disease listed in the List of Diseases Requiring Long-Term Treatment.

This Circular takes effect from July 7, 2014.

Article 3. The Chief of the General Staff, the Director of the Political Bureau, the Director of the General Department of Health, and the heads of relevant agencies and units are responsible for implementing this Circular.

Article 4. The General Department of Health is responsible for guiding the implementation of this Circular./.

I. LIST OF SERIOUS DISEASES
1. Cancer

Includes all types of cancer, treated or untreated, developed to the terminal stage (with tumor invasion, metastasis to multiple parts of the body, numerous complications, severe debilitation, bedridden). Poor prognosis, short remaining life expectancy.

2. Neurological diseases

- Central nervous system injuries due to various causes leaving non-reversible sequelae: Paralysis of all four limbs, paraplegia, unable to sit up or walk independently, severe debilitation, requiring continuous medical care.

- Severe dementia, complete loss of memory, requiring continuous medical care.

- Complete brain death following head trauma.

- Parkinson's disease in sequelae phase, requiring continuous medical care.

- Grand mal seizures (generalized), very frequent or continuous.

3. Liver diseases

Decompensated cirrhosis: Large ascites, frequently recurring; gastrointestinal bleeding due to esophageal varices, recurrent; hepatic encephalopathy; severe debilitation, unable to self-care.

4. Urinary system diseases

Decompensated chronic kidney failure requiring periodic dialysis for more than 12 hours per week, severe anemia, frequent subcutaneous and digestive hemorrhages; severe debilitation, requiring assistance.

5. Metabolic diseases

Type I and II diabetes in end-stage, with severe complications affecting vital organs (eyes, heart, kidneys, blood vessels), severely impacting health, requiring continuous assistance.

6. Respiratory system diseases

Chronic lung diseases (Chronic Obstructive Pulmonary Disease - COPD; multi-line cystic emphysema type A; pulmonary fibrosis, extensive pneumoconiosis; tuberculous fibrocavitary, drug-resistant) in decompensated stage, with numerous severe complications such as: Decompensated respiratory failure with frequent severe exacerbations; severe debilitation; inability to self-care.

7. Cardiovascular diseases

Decompensated heart failure degree 4 due to various causes, not recoverable despite treatment; persistent shortness of breath, unable to walk independently.

8. Musculoskeletal diseases

- Myasthenia gravis, ineffective treatment, requiring long-term mechanical ventilation.

- Joint diseases with deformities and stiffness in multiple joints, limiting whole-body movement, unable to walk, unable to self-care.

9. Immune deficiency syndromes

HIV infection progressed to clinical symptoms (AIDS), severe debilitation.

II. LIST OF DISEASES REQUIRING LONG-TERM TREATMENT

1. Cancer and benign tumors

a) Various types of cancer undergoing treatment.

b) Benign tumors causing compression, functional impairment of organs or tissues, or complications, recurrence after treatment.

2. Infectious diseases and sequelae

a) Tuberculosis of all types during treatment and sequelae.

b) Hansen's disease (leprosy) and sequelae.

c) Sequelae of bacterial, viral, parasitic, or fungal (candida, cryptococcus) meningitis or encephalitis.

3. Neurological diseases

a) Brain or spinal cord diseases causing paralysis of all four limbs; hemiplegia or paraplegia; complete paralysis of one limb.

b) Grand mal seizures (tonic-clonic).

c) Motor speech disorders (Broca's aphasia, Wernicke's aphasia, etc.).

d) Complete injury to cranial nerves, non-recoverable.

e) Extrapyramidal syndrome (Parkinson's disease; extrapyramidal disorder, cerebellar ataxia, chorea, tremor, etc.).

f) Unstable vertigo syndrome.

g) Complete injury to the cauda equina or spinal nerve roots, non-recoverable.

h) Amyotrophic lateral sclerosis.

i) Myasthenia gravis.

4. Cardiovascular diseases

a) Pericarditis, myocarditis with complications.

b) Primary cardiomyopathy.

c) Chronic ischemic heart disease: Frequent angina affecting daily activities (from Class II, III onwards) or with complications (arrhythmias, heart failure, etc.), ineffective medical treatment requiring interventional procedures.

d) Acute myocardial infarction stabilized after treatment.

e) Unstable angina.

f) Valve disease or endocarditis unresponsive to medical treatment requiring interventional or surgical procedures.

g) Arrhythmias: Atrial fibrillation, atrial flutter, ventricular tachycardia, third-degree AV block, complex ventricular premature beats unresponsive to medical treatment or causing complications (embolism, etc.), requiring permanent pacemaker implantation, radiofrequency ablation, etc.

e) Valvular heart disease pathology, endocarditis that does not respond to medical treatment or responds inadequately, requiring interventional or surgical treatment.

g) Cardiac arrhythmias: Atrial fibrillation, atrial flutter, ventricular tachycardia, third-degree atrioventricular block, complex ventricular premature contractions that do not respond to medical treatment or respond inadequately; or causing complications (embolism...), requiring permanent pacemaker implantation, radiofrequency ablation...

h) Heart failure stages III, IV due to various causes.

i) Hypertension with complications.

k) Other arterial diseases (arteritis, aneurysm...) with complications or requiring vascular intervention, surgery.

l) Venous thrombosis with complications (damage to organs, limbs).

Group 5. Respiratory System Diseases

a) Chronic obstructive pulmonary disease with irreversible lung ventilation disorder.

b) Bronchiectasis with complications.

c) Interstitial lung disease with irreversible lung ventilation disorder.

d) Pleural disease causing thick adhesions and irreversible lung ventilation disorder.

đ) Chronic respiratory insufficiency.

e) Cardiopulmonary insufficiency stages III, IV.

6. Digestive System Diseases

a) Progressive chronic hepatitis.

b) Cirrhosis.

c) Biliary tract disease treated surgically but still unstable.

d) Severe liver dysfunction.

đ) Pancreatic disease treated surgically but still unstable.

e) Non-cancerous gastrointestinal disease causing narrowing, affecting eating, requiring gastric opening surgery.

g) Gastric and duodenal ulcers with complications, ineffective medical treatment, requiring partial gastrectomy but still unstable.

h) Permanent artificial opening of the digestive tract.

i) Small intestine, colon, rectum disease causing complications (partial bowel obstruction; bleeding; malabsorption) recurring multiple times affecting health or requiring partial bowel resection.

7. Urinary System - Reproductive System - Obstetrics Diseases

a) Chronic kidney failure due to various causes.

b) Nephrotic syndrome.

c) Chronic glomerulonephritis.

d) Permanent artificial opening of the urinary tract.

đ) Ectopic pregnancy with poor treatment outcome or complications.

e) Female genital fistula surgery unsuccessful.

8. Endocrine and Metabolic Diseases

a) Complete pituitary dysfunction.

b) Basedow's disease with poor treatment outcome or complications.

c) Adrenal insufficiency.

d) Diabetes with complications.

đ) Gout with complications.

9. Musculoskeletal System Diseases

a) Inflammatory, degenerative joint disease of major joints with muscle atrophy, joint stiffness, deformity affecting joint function and daily activities.

b) Lumbar spine disease causing spinal inflammation or fusion surgery.

c) Pathological bone fracture.

10. Skin and Subcutaneous Tissue Diseases

a) Systemic lupus erythematosus.

b) Scleroderma.

c) Dermatomyositis and dermatitis.

d) Pemphigus.

đ) Psoriasis with lesions covering 20% or more of body surface area or with complications (kidney, joints, etc.).

e) Perivasculitis.

g) Generalized exfoliative dermatitis, erythroderma.

11. Blood and Hematopoietic Organ Diseases

a) Primary polycythemia.

b) Essential thrombocythemia.

c) Bone marrow failure with complications (anemia, bleeding, thrombosis, etc.).

d) Deficiency of coagulation factors causing organ, tissue complications.

đ) Hereditary or autoimmune hemolytic anemia.

e) Multiple myeloma.

12. Visual Organ Diseases

a) Blindness (light-dark vision loss) in one or both eyes due to ocular diseases.

b) Narrowed visual field in both eyes.

c) Hemianopia (due to optic chiasm damage) on both sides.

13. Ear, Nose, Throat Diseases

a) Severe hearing loss in one ear, both ears (hearing loss over 76%).

b) Parotid tumor (types B, C, D).

c) Pharynx, larynx disease affecting speech, swallowing, breathing (irreversible): Difficulty swallowing requiring feeding through a tube or gastric opening, voice loss requiring communication by writing or sign language; persistent shortness of breath, even at rest.

14. Immune System Diseases

Acquired immune deficiency syndrome (AIDS).

15. Organ Transplantation and Post-transplant Treatment

Status requiring immunosuppressive drug treatment after organ transplantation./.
