Article 4. The number of a national vocational skill certificate includes: serial number, regional code, and year of issuance.

Article 5. Authority for Recognition and Issuance, Exchange, Reissuance of National Vocational Skill Certificates
The Ministry of Labour, Invalids and Social Affairs is the agency responsible for recognizing and issuing, exchanging, reissuing, and revoking and recalling national vocational skill certificates.
Article 6. Dossier for Application for Recognition and Issuance of National Vocational Skill Certificates
Article 4. Provisions on the serial number on the National Vocational Skill Certificate
a) A request for recognition and issuance of a national vocational skill certificate by the vocational skill assessment organization according to Model 01, Appendix 03 issued with this Circular;
Chapter III
ISSUANCE OF NATIONAL VOCATIONAL SKILL CERTIFICATES
b) The decision of the head of the vocational skill assessment organization recognizing the results of the assessment panel, accompanied by a summary of the test scores of the knowledge and practical tests of the workers participating in the assessment and certification process, signed by all members of the assessment panel confirming the assessment results;
c) A list of applicants for recognition and issuance of national vocational skill certificates for those who meet the requirements according to Model 02, Appendix 03 issued with this Circular, attached with each person's name a color photo on a white background measuring 3 x 4 cm in the format of a citizen identification card.
The dossier for exchanging a national vocational skill certificate stipulated in Article 3 of this Circular for individuals who have been issued a national vocational skill certificate before May 15, 2015, includes:
1. An application form for exchanging a national vocational skill certificate according to Model 02, Appendix 04 issued with this Circular and a color photo on a white background measuring 3 x 4 cm in the format of a citizen identification card;
2. The national vocational skill certificate already issued and a copy of the citizen identification card or passport still valid for comparison.
Article 7. Application Documents for National Vocational Skill Certificate Exchange
2. The national vocational skill certificate already issued (except in cases where the certificate has been lost) and a copy of the citizen identification card or passport still valid for comparison;
The national vocational skill certificate shall be reissued in cases where there is a request to amend certain contents recorded on the previously issued certificate due to changes or corrections in the person's civil status registration, or because the previously issued certificate has been damaged, deteriorated, or lost. The application dossier includes:
1. The procedures for recognition and issuance of national occupational skill certificates to workers who have successfully completed the assessment for such certificates shall be carried out as follows:
a) The organization conducting the occupational skill assessment shall submit one set of application files in accordance with Clause 1, Article 6 of this Circular or send them via postal service to the Ministry of Labor, Invalids, and Social Affairs;
b) Within ten working days from the date of receipt of the application files, the Ministry of Labor, Invalids, and Social Affairs shall be responsible for reviewing the application files, deciding on the recognition and issuance of the national occupational skill certificate; if not recognized, a written response must be provided along with the specific reasons.
2. The procedures for recognition and issuance of national occupational skill certificates to individuals who have won medals at World Skills Competitions, ASEAN Skills Competitions, or first, second, or third place awards at National Skills Competitions shall be carried out as follows:
a) The competition organizing body shall submit one set of application files in accordance with Clause 2, Article 6 of this Circular or send them via postal service to the Ministry of Labor, Invalids, and Social Affairs;
b) Within five working days from the date of receipt of the application files, the Ministry of Labor, Invalids, and Social Affairs shall be responsible for reviewing the application files and deciding on the recognition and issuance of the national occupational skill certificate.
3. The procedures for replacement and reissuance of national occupational skill certificates for cases stipulated in Articles 7 and 8 of this Circular shall be carried out as follows:
a) Individuals requesting to replace their national occupational skill certificate or those requesting reissuance of their national occupational skill certificate shall submit one set of application files in accordance with Article 7 or Article 8 of this Circular or send them via postal service to the Ministry of Labor, Invalids, and Social Affairs;
b) Within fifteen working days from the date of receipt of the application files, the Ministry of Labor, Invalids, and Social Affairs shall be responsible for reviewing the application files and replacing or reissuing the national occupational skill certificate; if not replaced or reissued, a written response must be provided along with the specific reasons.
Article 10. Revocation and Recovery of National Occupational Skill Certificates
1. The National Occupational Skill Certificate that has been issued shall be revoked or recovered in the following cases:
a) Misrepresentation of information in the application dossier for issuance, renewal, or reissuance of the National Occupational Skill Certificate;
b) Forgery of documents or texts included in the application dossier for issuance, renewal, or reissuance of the National Occupational Skill Certificate.
Applicants for recognition and issuance, replacement, and reissuance of national occupational skill certificates shall bear legal responsibility for the accuracy of the contents in the application files submitted to the competent authority for issuing national occupational skill certificates in accordance with this Circular.
Chapter IV
MANAGEMENT OF NATIONAL OCCUPATIONAL SKILL CERTIFICATE ISSUANCE
Article 12. Publicizing Information on Issuance, Renewal, Reissuance, and Revocation of National Occupational Skill Certificates
2. Information published on the website must ensure accuracy and be regularly updated; it must be easy to manage, access, and search.
Article 13. Reporting on Issuance, Renewal, Reissuance, and Revocation of National Occupational Skill Certificates
1. Organize the acceptance of dossiers for issuance, renewal, or reissuance of National Occupational Skill Certificates; check, compare, and accurately record all contents on the National Occupational Skill Certificate.
3. Accurately and completely record all contents in the business management books and manage, store the business management books related to issuing, renewing, reissuing national vocational skill certificates in accordance with the laws on archives.
5. Perform other tasks as prescribed in this Circular.
Article 15. Responsibilities of the organization assessing vocational skills
||| 3. Bear legal responsibility for the accuracy of the content in the dossier submitted to the competent authority for issuing national vocational skill certificates as stipulated in this Circular.
4. Accurately and completely record the contents in the transfer and delivery book of national vocational skill certificates and manage, store the transfer and delivery book of national vocational skill certificates in accordance with the laws on archives.
||| 5. Carry out the transfer and issuance of national vocational skill certificates to the certificate recipients and perform other tasks as stipulated in this Circular.
Chapter V
IMPLEMENTING PROVISIONS
Article 16. Effective Date
