CIRCULARAMENDING AND SUPPLEMENTING CERTAIN PROVISIONS OF CIRCULAR NO. 16/2025/TT-BCT DATED FEBRUARY 1, 2025 OF THE MINISTER OF INDUSTRY AND TRADE ON THE OPERATION OF THE COMPETITIVE WHOLESALE ELECTRICITY MARKET

Amending and supplementing some articles of Circular No. 66/2015/TT-BCA

Pursuant to the Citizen Identity Card Law 2014;

Pursuant to Decree No. 01/2018/NĐ-CP dated August 6, 2018 on the functions, tasks, powers, and organizational structure of the Ministry of Public Security;The Department of Police for Administrative Management of Social Order;

At the proposal of the Director Amending and supplementing some articles of Circular No. 66/2015/TT-BCA dated December 15, 2015 of the Minister of Public Security on forms to be used in the issuance and management of Citizen Identification Cards, Citizen Identification Card archives, and the National Population Database which has been amended and supplemented by Circular No. 41/2019/TT-BCA dated October 1, 2019 of the Minister of Public Security.

The Minister of Public Security promulgates this Circular amending and supplementing some articles of Circular No. 66/2015/TT-BCA dated December 15, 2015 stipulating forms used in the issuance and management of Citizen Identity Cards, storage of Citizen Identity Cards and the National Population Database.

1. Supplementing points m and n after point l of Clause 1, Article 3 as follows:

“m) Record of receipt and dispatch of application files for issuing, replacing, and reissuing Citizen Identity Cards (Form CC12);

n) Application for confirmation of citizen identification number (Form CC13)”.

2. Amending and supplementing Clause 4 as follows:

Article 4. Management of forms

1. The Ministry of Public Security shall uniformly manage the forms issued together with this Circular; print and distribute Form CC11 to public security units at all levels.

2. The Administrative Police Management Department shall print, manage, and distribute Forms CC01 and CC13 for use at the National Citizen Identification Center.

3. Provincial and centrally-administered city public security organs shall organize printing, managing, and distributing Forms CC01, CC02, CC08, CC12, CC13, DC01, and DC02 to public security units under their jurisdiction.

4. Forms CC02, CC03, CC04, CC05, CC06, CC07, CC09, and CC10 shall be printed directly from computers when processing applications for issuing, replacing, and reissuing Citizen Identity Cards. The printed form will include information extracted and statistically compiled from the Citizen Identification Database according to each unit's storage scope.

5. Forms CC01, CC02, CC04, CC05, CC06, CC07, CC08, CC09, CC10, CC12, CC13, DC01, and DC02 shall be printed on A4 size paper (210 mm x 297 mm). Form CC03 shall be printed on A5 size paper (148 mm x 210 mm). Forms CC01 and CC08 shall be printed on both sides. Form CC11 shall have dimensions of 250 mm x 330 mm and be printed on Kraft paper with a weight of 250g/m².2.     

6. Forms prescribed in this Circular shall be printed in black ink on white paper. Form CC11 shall be printed in black ink on yellowish-brown paper. Printing shall not alter the content of the forms; public security units at all levels shall be responsible for managing the printing and distribution of forms and maintain records thereof.

3. Amending and supplementing Clause 2 and Clause 3 of Article 5 as follows:

“2. The Administrative Police Management Department shall take the lead and coordinate with the Planning and Finance Department and relevant units to prepare budgets for printing and distributing forms and paper for printing Forms CC01 and CC11, and paper for printing Form CC02 for use at the National Citizen Identification Center and Form CC11 for distribution to public security units at all levels.

In September each year, public security units at all levels shall prepare budget estimates for the use of Form CC11 in their units and regions for the following year and submit them to the Administrative Police Management Department for consolidation and reporting to the Ministry of Public Security.

3. Public security units at all levels shall prepare budgets for printing and distributing forms used in the issuance and management of Citizen Identity Cards, storage of Citizen Identity Cards, and the National Population Database as stipulated in Clauses 3 and 4 of Article 4 of this Circular.”

4. Amend and supplement Clause 1 of Article 7 as follows:

“1. Form CC01 shall be used for citizens to declare personal information when applying for issuing, replacing, or reissuing Citizen Identity Cards (including cases where citizens declare information through online public service portals).”

5. Amending and supplementing point h of Clause 2, Article 7 as follows:

“h) Item "Place of Birth Registration": Record the administrative area name at the commune, district, and province level based on the birth certificate of the citizen. If the birth certificate does not fully record the administrative area name at the commune, district, and province level, then record the administrative area name based on the birth certificate. If there has been a change in the administrative area name, record it according to the new administrative area name as provided for by regulations;”

6. Amending and supplementing point i of Clause 2, Article 7 as follows:

“i) Item "Place of Origin": Record the administrative area name at the commune, district, and province level based on the birth certificate or household registration book. If these documents do not fully record the administrative area name at the commune, district, and province level, then record the administrative area name based on these documents. If there has been a change in the administrative area name, record it according to the new administrative area name as provided for by regulations;”

7. Amending and supplementing point o of Clause 2, Article 7 as follows:

“o) Item requested by the citizen:

- "Issuing, replacing, or reissuing Citizen Identity Cards": For initial issuance, record "new"; for damage, expiration, or changes/supplemental information on the Citizen Identity Card or if the citizen requests replacement, record "replacement"; for loss of the card or return to Vietnamese citizenship, record "reissue";

- "Delivery of Citizen Identity Cards to the citizen’s address": If the citizen requests delivery of the Citizen Identity Card during issuance, replacement, or reissuance, record "yes" and provide full recipient address and contact phone number; if no request is made, record "no";”

8. Supplementing point q after point p of Clause 2, Article 7 as follows:

“q) The two-dimensional barcode section encodes all personal information of the citizen and information for management purposes on the Citizen Identity Card Application Form.”

Section "Time of Appointment": Register the specific date when the citizen comes to the citizen identification management agency to handle the application for issuance, replacement, or reissuance of the Citizen Identification Card; Section "At": Record the name of the citizen identification management agency where the citizen registers to handle the application for issuance, replacement, or reissuance of the Citizen Identification Card.

9. Amend and supplement Clause 3 of Article 8 as follows:

 "3. Citizen Identification Card number: The staff handling the application for issuance, replacement, or reissuance of the Citizen Identification Card shall record the Citizen Identification Card number of the citizen on the Information Collection Receipt Form (CC02) upon receiving the approval result for issuance, replacement, or reissuance of the Citizen Identification Card (the Citizen Identification Card or the data of the approval result for issuance, replacement, or reissuance of the Citizen Identification Card returned through the system)."

10. Amend and supplement Clause 1 of Article 12 as follows:

"1. Model CC06 is established by the National Citizen Identification Center to propose the leadership of the Administrative Police Department under the Public Order Management Bureau to approve the application dossier for issuance, replacement, or reissuance of the Citizen Identification Card for citizens. This model is established based on the list of application dossiers for issuance, replacement, or reissuance of the Citizen Identification Card sent by the provincial police departments directly under the central government."

11. Amend and supplement Clause 1 of Article 14 as follows:

"1. Model CC08 is used by the provincial police department and county police department to compile statistics and report on the work of issuing and managing citizen identification cards. On a regular monthly, quarterly, semi-annual, and annual basis, the provincial police department must submit reports to the Administrative Police Department under the Public Order Management Bureau."

12. Add Article 17a after Article 17 as follows:

Article 17a. Record of Handover of Application Dossier for Issuance, Replacement, or Reissuance of the Citizen Identification Card (CC12)

1. Model CC12 is used by the unit handling the application for issuance, replacement, or reissuance of the Citizen Identification Card to hand over the application dossier for issuance, replacement, or reissuance of the Citizen Identification Card to the file management unit of the Administrative Police Department under the Public Order Management Bureau of the provincial police department or the municipal police department directly under the central government. The unit receiving the application dossier for issuance, replacement, or reissuance of the Citizen Identification Card is responsible for checking and accepting all the dossiers according to regulations.

2. Method of recording information:

a) Section "at...hours..., day, month, year": Record the full time, date, month, and year of handing over the dossier; "at": Record according to the location where the dossier is handed over;

b) Section "representative of the handing-over party" and "representative of the receiving party": Record the full name, rank, position, and working unit of the staff from the handing-over and receiving units;

c) Section "Total number of dossiers": Record the total number of dossiers handed over; including the total number of new issuance dossiers, the total number of replacement dossiers, and the total number of reissuance dossiers.

d) Section "Attached list from dossier number...to dossier number...": Record the serial number according to the list of handed-over dossiers.

3. Section "Receiving Party", "Handing-over Party":

a) In cases where the dossier is handed over within the Administrative Police Department under the Public Order Management Bureau, the team leader is responsible for signing to confirm the handover record. Staff from both the handing-over and receiving parties are responsible for signing their full names on the handover record.

b) In cases where the dossier is handed over between the county police department and the Administrative Police Department under the Public Order Management Bureau, the leaders of the county police department and the Administrative Police Department under the Public Order Management Bureau must sign and stamp the handover record. For dossiers issued at the National Citizen Identification Center under the Administrative Police Department under the Public Order Management Bureau, the leaders of the National Citizen Identification Center and the Administrative Police Department under the Public Order Management Bureau receiving the handover must sign. Staff from both the handing-over and receiving parties are responsible for signing their full names on the handover record."

13. Add Article 17b after Article 17a as follows:

"Article 17b. Request for Confirmation of Identity Card Number (CC13)

1. Model CC13 is used in cases where a citizen has been issued a Citizen Identification Card but has not yet received a confirmation letter for the Identity Card number or has lost the confirmation letter for the Identity Card number.

2. Method of recording information:

a) Section "Surname, middle name, and given name": Record the full surname, middle name, and given name according to the birth certificate in uppercase with accents;

b) Section "Date of Birth": Record the date of birth of the citizen who has been issued, replaced, or reissued the Citizen Identification Card. Date of birth should be recorded with two digits; year of birth should be recorded with four digits. For months from March to September, record with one digit, and for other months, record with two digits;

c) Section "Gender": Record "Male" for male gender and "Female" for female gender;

d) Section "Ethnicity": Record the ethnicity of the citizen requesting the confirmation letter for the Identity Card number according to the birth certificate or ethnic certification document issued by the competent authority;

đ) Section "Nationality": Record the nationality of the citizen requesting the confirmation letter for the Identity Card number according to the birth certificate or nationality certification document issued by the competent authority;

e) Section "Place of Registration of Birth", Section "Native Place": Record according to point h, point i of Clause 2 of Article 7 of this Circular;

g) Section "Place of Permanent Residence": Record fully and accurately according to the household registration book."

14. Replace the forms promulgated together with Circular No. 66/2015/TT-BCA specifically as follows:

a) Citizen Identification Card Application Form (model number CC01) promulgated together with Circular No. 66/2015/TT-BCA is replaced by the Citizen Identification Card Application Form (model number CC01) promulgated together with this Circular.

b) Proposal for Approval of Application Dossier for Issuance, Replacement, or Reissuance of the Citizen Identification Card (model number CC06) promulgated together with Circular No. 66/2015/TT-BCA is replaced by the Proposal for Approval of Application Dossier for Issuance, Replacement, or Reissuance of the Citizen Identification Card (model number CC06) promulgated together with this Circular.

c) Confirmation Letter for Identity Card Number (model number CC07) promulgated together with Circular No. 66/2015/TT-BCA is replaced by the Confirmation Letter for Identity Card Number (model number CC07) promulgated together with this Circular.

d) Information Adjustment Form for Citizen Identification (model number CC09) promulgated together with Circular No. 66/2015/TT-BCA is replaced by the Information Adjustment Form for Citizen Identification (model number CC09) promulgated together with this Circular.

e) Community Information Collection Form (model number DC01) promulgated together with Circular No. 66/2015/TT-BCA is replaced by the Community Information Collection Form (model number DC01) promulgated together with this Circular.

15. Amend and supplement Article 21 as follows:

Article 21. Responsibility for Implementation

1. The Director of the Administrative Management Police Department under the Ministry of Public Security is responsible for guiding, inspecting, and urging the implementation of this Circular.

2. The heads of units under the Ministry of Public Security, the Directors of Public Security for provinces and centrally governed cities, and relevant agencies, organizations, and individuals are responsible for implementing this Circular.

During the implementation process, if there are difficulties or obstacles, local police units shall report to the Ministry of Public Security (through the Social Order Administrative Police Management Bureau) for timely guidance.

Article 2. Effective Date

This Circular takes effect from November 18, 2019.

Article 3. Responsibility for Implementation

1. The Director of the Administrative Management Police Department under the Ministry of Public Security is responsible for guiding, inspecting, and urging the implementation of this Circular.

2. The heads of units under the Ministry of Public Security, the Directors of Public Security for provinces and centrally governed cities, and relevant agencies, organizations, and individuals are responsible for implementing this Circular.

During the implementation process, if there are difficulties or obstacles, local public security units shall report to the Ministry of Public Security (through the Social Order Administrative Management Police Corps) for timely guidance./.
