REGARDING THE ISSUANCE OF FORMS AND RECORD BOOKS FOR MANAGEMENT OF TEACHING AND LEARNING IN VOCATIONAL TRAINING
THE MINISTER OF LABOR, WAR INVALIDS AND SOCIAL AFFAIRS
Pursuant to the Vocational Training Law dated November 29, 2006;
Pursuant to Decree No. 186/2007/ND-CP dated December 25, 2007 of the Government on the functions, tasks, powers, and organizational structure of the Ministry of Labor, War Invalids and Social Affairs;
At the proposal of the Director General of the Vocational Training Administration,
DECISION:
Article 1. This Decision promulgates forms and record books for management of teaching and learning in vocational training at the primary vocational level, intermediate vocational level, and higher vocational college level to be uniformly applied to vocational training centers, intermediate vocational schools, higher vocational colleges, intermediate specialized schools, higher colleges, universities, enterprises, and other entities engaged in vocational training (hereinafter referred to collectively as vocational training institutions) throughout the country, including:

Article 2. This Decision takes effect 15 days after its publication in the Official Gazette and replaces Decision No. 830/1999/QD-BLDTBXH dated July 20, 1999 of the Minister of Labor, War Invalids and Social Affairs on the issuance of forms and record books for management of teaching and learning in vocational training institutions.
Article 3. Ministers, Heads of ministerial-level agencies, Heads of government-affiliated agencies, Heads of central bodies of political and social organizations; Chairpersons of People's Committees of provinces and centrally governed cities; Directors of Departments of Labor, War Invalids and Social Affairs, Heads of the Office of the Ministry, General Directors of the Vocational Training General Department; Heads of vocational training institutions and Heads of related agencies have the responsibility to implement this Decision.
