Provisions on forms to be used in the issuance and management of identity cards
Identity Card, identity card archives, and the National Population Database
The Minister of National Defense hereby issues this Circular amending and supplementing certain articles of normative legal documents within the competence to issue of the Minister of National Defense related to the declaration of personal information when performing administrative procedures.
Pursuant to Decree No. 106/2014/NĐ-CP dated November 17, 2014, stipulating the functions, tasks, powers, and organizational structure of the Ministry of Public Security;
At the proposal of the Director of the General Police Department;
The Minister of Public Security shall issue a Circular stipulating the forms to be used in the issuance and management of Identity Cards, identity card archives, and the National Population Database.
PART I
GENERAL PROVISIONS
Article 1. Scope of Regulation
This Circular specifies the forms, methods of filling out the forms; printing costs and management of forms used in the issuance and management of Identity Cards, identity card archives, and the National Population Database.

1. Vietnamese citizens when applying for the issuance, replacement, reissuance of Identity Cards, updating changes in personal information in identity card archives, and providing information and materials to the National Population Database.
2. Public security units and localities.
3. Agencies, organizations, and individuals related to the issuance and management of Identity Cards, identity card archives, and the National Population Database.
Article 3. Forms to be used in the issuance and management of Identity Cards, identity card archives, and the National Population Database
1. Forms to be used in the issuance and management of Identity Cards, identity card archives (marked as CC), including:
a) Identity Card Application Form (Form CC01);
b) Information Collection Sheet for Identity Cards (Form CC02);
c) Appointment Notice for Receiving Identity Cards (Form CC03);
d) List of Recommendations for Approval of Applications for Issuance, Replacement, and Reissuance of Identity Cards (Form CC04);
đ) Receipt and Delivery Slip for Applications for Issuance, Replacement, and Reissuance of Identity Cards for Verification (Form CC05);
e) Recommendation for Approval of Applications for Issuance, Replacement, and Reissuance of Identity Cards (Form CC06);

Article 4. Management of forms
1. The Ministry of Public Security shall uniformly manage the forms issued together with this Circular. Printing and distributing Form CC11, issuing printing paper for Form CC02 to local police units.
2. The Department of Registration, Residence Management, and National Population Database shall print, manage, and distribute Form CC01 for use at the National Identity Card Center.
3. Provincial Police Departments shall organize the printing, management, and distribution of Forms CC01, CC08, DC01, and DC02 to local police units under their jurisdiction.
4. Forms CC02, CC03, CC04, CC05, CC06, CC07, CC09, and CC10 shall be directly printed from computers during the process of issuing, replacing, and reissuing Identity Cards. The printed form includes extracted and summarized information from the Identity Card Database within the storage scope of each unit.
5. Forms CC01, CC02, CC04, CC05, CC06, CC08, CC09, CC10, DC01, and DC02 shall be printed on A4 size paper (210 mm x 297 mm); Form CC02 is printed on 120 g/m² paper. Forms CC03 and CC07 are printed on A5 size paper (148 mm x 210 mm). Forms CC01 and CC08 are printed on both sides. Form CC11 has dimensions of 250 mm x 330 mm, printed on Kraft paper weighing 250 g/m².26. Forms prescribed in this Circular shall be printed in black ink on white paper. Form CC11 is printed in black ink on yellow-brown paper. When printing, the content of the forms must not be changed; local police units have the responsibility to manage the printing and distribution of forms and maintain records thereof.2.
6. The forms prescribed in this Circular shall be printed in black ink on white paper. Notably, Form CC11 shall be printed in black ink on light brown paper. Printing shall not alter the contents of the forms; local public security agencies shall be responsible for managing the printing, distribution of the forms, and maintaining records thereof.
Article 5. Funding for printing and distributing forms
1. The funding for printing and distributing forms used in the issuance and management of citizen identification cards, citizen identification records, and the National Population Database shall be sourced from the regular budget of the Ministry of Public Security allocated to local public security units.
2. The Criminal Police General Department shall take the lead and coordinate with the Financial Bureau and relevant units to prepare the budget for printing and distributing forms and form paper as stipulated in Clause 1 and Clause 2, Article 4 of this Circular.
By September each year, local public security units shall prepare the forecasted demand for using CC02 and CC11 forms for the following year and submit it to the Citizen Registration and National Population Database Management Police Bureau for consolidation and reporting to the Criminal Police General Department.
3. Local public security units shall prepare the budget for printing and distributing forms used in the issuance and management of citizen identification cards, citizen identification records, and the National Population Database as stipulated in Clause 3 and Clause 4, Article 4 of this Circular.
Article 6. Requirements for filling out forms
1. All contents in each form must be fully, accurately, and clearly filled out, and all writings must be in the same ink type.
2. If a person coming to handle procedures cannot write or cannot fill out the application themselves, they may ask another person to fill it out on their behalf according to their declaration. The person filling out on behalf must write "Proxy Writer", truthfully fill out, sign, and clearly write their full name, and bear responsibility for the proxy filling.
3. Columns and items in the forms must be filled out according to the annotations and instructions in the forms (if any) and as prescribed in this Circular. In cases where information in columns and items of the forms is too long, it can be abbreviated but must ensure that basic information is clear.
4. The ink color for filling out forms, signatures of authorized persons, and other contents in the forms can only be blue, dark purple, or black ink.
Chapter II
METHODS FOR FILLING OUT FORMS
Article 7. Citizen Identification Card Application Form (CC01)
1. The CC01 form is used for citizens to declare personal information when applying for, changing, or reissuing a Citizen Identification Card.
2. Method of recording information:
a) Item "Surname, middle name, and given name", "Alternative name": Fill in the full surname, middle name, and given name as stated in the birth certificate; use uppercase letters with accents. Only fill in alternative names if they are listed in the birth certificate;
b) Item "Date of Birth": Fill in the date of birth of the citizen applying for, changing, or reissuing a Citizen Identification Card. Date of birth should be written with two digits; year of birth should be written with four digits. For months from March to September, write one digit, and for the remaining months, write two digits;
c) Item "Gender": Write "Male" if male, and "Female" if female;
d) Item "Ethnicity", "Religion": Fill in the ethnicity and religion of the citizen applying for, changing, or reissuing a Citizen Identification Card as stated in the birth certificate or ethnic and religious certification documents issued by competent authorities;
đ) Item "Nationality": Fill in the nationality of the citizen applying for, changing, or reissuing a Citizen Identification Card as stated in the birth certificate or nationality certification documents issued by competent authorities;
e) Item "Marital Status": Fill in the current marital status of the citizen applying for, changing, or reissuing a Citizen Identification Card, including: unmarried, married, or divorced;
g) Item "Blood Type" (if applicable): Fill in according to the conclusion of blood type testing for the citizen applying for, changing, or reissuing a Citizen Identification Card;
h) Item "Place of Birth Registration": Fill in the complete administrative address of the commune, district, and province where the birth certificate was issued. In case of changes in administrative addresses, record the new name of the administrative address according to the provisions of the law;
i) Item "Place of Origin": Fill in the complete administrative address of the commune, district, and province according to the birth certificate; if there is no birth certificate, fill in according to the household registration book. In case of changes in administrative addresses, record the new name of the administrative address according to the provisions of the law;
k) Item "Permanent Residence": Fill in accurately according to the household registration book. In cases where the citizen applies for, changes, or reissues a Citizen Identification Card while officially serving in the People's Army or People's Public Security force and residing in barracks or collective housing, fill in according to the introduction letter issued by the unit;
l) Item "Current Address": Fill in the current address of the citizen in detail and accurately in the order of house number, street; village, hamlet, village, ward, commune, township, town; district, county, city; province, centrally-administered municipality;
m) Item "Occupation": Clearly state the occupation currently held; leave blank if a military serviceman. Item "Education Level": Clearly state the highest education level (doctorate, master's degree, bachelor's degree, college, secondary vocational school, high school graduate, junior high school graduate...);
n) Items 17, 18, 19, 20, 21: Fill in the full surname, middle name, given name; nationality; citizen identification card number or identity card number in the corresponding fields in the form (if applicable);
o) Citizen's Request Item:
- "Delivered by postal service directly to the citizen": In cases where the citizen requests delivery of the Citizen Identification Card by postal service, indicate "yes" and provide the full address and phone number of the citizen; if not requested, indicate "no";
- "Issuance, change, reissue of Citizen Identification Card": For first-time issuance, indicate "new"; for damaged, expired, or information changed cards, indicate "change"; for lost cards, indicate "reissue";
- "Confirm Identity Card Number": In cases where the citizen requests confirmation of the nine-digit identity card number, indicate "yes"; if not requested, indicate "no";
p) Item "Date...Month...Year...": Clearly fill in the date, month, and year when the citizen declares the application for issuance, change, or reissuance of the Citizen Identification Card.
3. Item "Verification Result": In cases where verification through citizen identification records is required for applications for issuance, change, or reissuance of Citizen Identification Cards, the receiving unit shall compare and verify with the original file and respond with the result to the requesting unit.
Article 8. Citizen Identity Information Receipt Form (CC02)
1. The CC02 form shall be established by the police station receiving applications for issuance, replacement, or reissuance of citizen identity cards based on cross-referencing the citizen's information in the National Population Database with their citizen identity declaration forms. After verifying the citizen's information, the CC02 form will be directly printed from the printer for the citizen to sign on the Citizen Identity Information Receipt Form.
2. One-dimensional barcode number: each application for issuance, replacement, or reissuance of citizen identity cards shall be assigned a unique number and barcode for management purposes and printed on the Citizen Identity Information Receipt Form.
3. Citizen identity card number: record the citizen identity card number for cases where a citizen identity card is issued, specifically as follows:
The National Citizen Identity Center, when returning citizen identity cards, shall send along a list of citizens who have been issued citizen identity cards to the Provincial Public Order Administrative Police Department. Based on this list, the provincial police officer responsible for managing citizen identity records shall fully record the 12-digit citizen identity card number on the Citizen Identity Information Receipt Form. In cases where citizens are issued citizen identity cards at the National Citizen Identity Center before transferring the citizen identity records to the provincial police department where the citizen has permanent residence, the transfer officer shall fully record the citizen identity card number on the Citizen Identity Information Receipt Form.
4. Information recording method:
a) From Section 1 to Section 13: record the citizen's information according to the guidelines set out in Clause 2, Article 7 of this Circular;
b) From Section 14 to Section 16: record the citizen's information according to the instructions provided in the CC01 form notes;
c) Section 17: a citizen's portrait photograph measuring 4 cm x 6 cm with a white background;
d) Section 18: record the citizen's distinguishing features as prescribed;
đ) Section 19: record only one of the cases of issuance, replacement, or reissuance;
e) Section 20: record the number of times a citizen identification card/citizen identity card has been issued to the citizen (including the current issuance);
f) Section 21: record the name of the police agency issuing the Citizen Identity Information Receipt Form;
g) Section 22: the officer checking the Citizen Identity Information Receipt Form signs and clearly writes their full name after ensuring that all information is complete and correct according to regulations;
h) Section 23: the citizen records the date, month, and year of the procedure for issuance, replacement, or reissuance of the citizen identity card, signs and clearly writes their full name.
Article 9. Card for Appointment to Receive Citizen Identity Cards (CC03)
1. The CC03 form shall be established by the police agency receiving applications for issuance, replacement, or reissuance of citizen identity cards from citizens, and issued to citizens to schedule a date to receive their citizen identity cards.
2. The appointment card for receiving citizen identity cards shall be directly printed from the printer after collecting the personal information of citizens applying for issuance, replacement, or reissuance of citizen identity cards as prescribed.
Article 10. List of Applications for Approval of Issuance, Replacement, or Reissuance of Citizen Identity Cards (CC04)
1. The CC04 form shall be established by the police agency receiving applications for issuance, replacement, or reissuance of citizen identity cards, and proposed to the unit commander for approval of applications for issuance, replacement, or reissuance of citizen identity cards.
2. Method of recording information:
a) Item (1): record the name of the direct superior agency of the agency proposing the approval of applications for issuance, replacement, or reissuance of citizen identity cards;
b) Item (2): record the name of the agency proposing the approval of applications for issuance, replacement, or reissuance of citizen identity cards;
c) Item (3): record the title of the person authorized to approve applications for issuance, replacement, or reissuance of citizen identity cards;
d) Item "Number: ... /...": record the number of the list proposing the approval of applications for issuance, replacement, or reissuance of citizen identity cards;
đ) Item "List Code": is the common code for all applications in the list proposing the approval of applications for issuance, replacement, or reissuance of citizen identity cards;
Column "STT": record the serial number of each application in the list proposing the approval of applications for issuance, replacement, or reissuance of citizen identity cards;
Column "Citizen Identity Information Receipt Form Number": record the number on the Citizen Identity Information Receipt Form of the citizen;
Column "Surname, Middle Name, Given Name at Birth", "Gender", "Date of Birth", "Type of Issue", "Issued Citizen Identity Card Number": record according to the corresponding sections of information on the Citizen Identity Information Receipt Form;
Item "Opinion of the Person Authorized to Approve the Application": the person authorized to approve applications for issuance, replacement, or reissuance of citizen identity cards shall clearly write their opinion as "agree" or "disagree", if disagreeing with certain cases, specify those cases.
Article 11. Application for Issuance, Replacement, Reissuance of Citizen Identity Card File Tracking Slip (CC05)
Clause 1. The CC05 form shall be established by the unit receiving applications for issuance, replacement, reissuance of citizen identity cards, proposing the unit's leadership to decide on transferring files requiring tracking through the citizen identity card archive.
2. Method of recording information:
Point a) Section (1): Record the name of the direct superior agency of the agency submitting the application for issuance, replacement, reissuance of citizen identity cards requiring tracking;
Point b) Section (2): Record the name of the agency submitting the application for issuance, replacement, reissuance of citizen identity cards requiring tracking;
Point c) Column "STT": Record the serial number of each application for issuance, replacement, reissuance of citizen identity cards in the list sent;
Point d) Columns "Surname, Middle Name, Given Name", "Gender", "Date of Birth": Record the information of citizens according to the application for issuance, replacement, reissuance of citizen identity cards;
Point đ) Column "Citizen Identity Card Information Receipt Number": Record the number on the Citizen Identity Card Information Receipt of the citizen;
Point e) Section "Total Number of Files": Record the total number of applications for issuance, replacement, reissuance of citizen identity cards handed over;
Point g) Section "Form Preparer": The preparer of the form signs and clearly records their full name for reporting to the leadership for approval;
Point h) Section "Unit Head": The unit head approves, signs, and clearly records their full name;
Point i) Section "Handover of Files": Clearly record the date, month, year of file handover. Both the giver and receiver sign and clearly record their full names;
Point k) Section "Receipt of Search Results": Clearly record the date, month, year of receipt of search results. Both the giver and receiver sign and clearly record their full names.
Article 12. Proposal for Approval of Applications for Issuance, Replacement, Reissuance of Citizen Identity Cards (CC06)
Clause 1. The CC06 form is established by the National Citizen Identity Card Center to propose the leadership of the Registration and National Population Data Management Police Department to approve applications for issuance, replacement, reissuance of citizen identity cards submitted by citizens. This form is established based on the list of applications for issuance, replacement, reissuance of citizen identity cards sent by provincial and centrally-administered city police departments.
2. Method of recording information:
Point a) Section "No.: ……./………": Record the number of the proposal document for approval of applications for issuance, replacement, reissuance of citizen identity cards;
Point b) Column "STT": Record the serial number of each list code in the proposal for approval of applications for issuance, replacement, reissuance of citizen identity cards;
Point c) Column "Unit": Record the name of the unit receiving applications for issuance, replacement, reissuance of citizen identity cards from citizens;
Point d) Column "List Codes": Record the code of each list proposed for approval of applications for issuance, replacement, reissuance of citizen identity cards;
Point đ) Column "Total Number of Lists": Record the total number of lists in the "list codes" column;
Point e) Column "Files": Record the total number of files in the list with the corresponding list code in the "Total" column, where the number of valid files meeting the conditions for issuance, replacement, reissuance of citizen identity cards is recorded in the "Valid" column, and the number of invalid files not meeting the conditions for issuance, replacement, reissuance of citizen identity cards is recorded in the "Invalid" column.
Article 13. Certificate of Citizen Identification Card Number (CC07)
Clause 1. The CC07 form is established by the public security agency receiving applications for issuance of citizen identity cards to confirm the citizen identification card number (9 digits) most recently issued when the citizen requests confirmation of the old citizen identification card number.
Clause 2. The head of the unit receiving applications for issuance, replacement, reissuance of citizen identity cards signs the certificate of the citizen identification card number of the citizen.
Article 14. Report on the Issuance and Management of Citizen Identity Cards (CC08)
1. The CC08 form shall be used by provincial public security organs and district-level public security organs to compile statistics and report on the results of the issuance and management of citizen identity cards. Quarterly, every six months, and annually, the provincial public security organ must submit reports to the National Police Corps (through the Registration and Population Management Bureau and the National Population Database).
2. The data and contents in the report must be accurate, clear, and complete.
Article 15. Information Adjustment Form for Citizen Identity Cards (CC09)
1. The CC09 form shall be established by the public security agency receiving applications for the issuance, replacement, or reissuance of citizen identity cards when citizens have changes in their place of permanent residence, household registration, or other types of changes to adjust the information of citizens in the citizen identity card archive. The public security agency receiving the application has the responsibility to transfer the Information Adjustment Form for Citizen Identity Cards to the citizen identity card archive management agency under the provincial public security organ for management and exploitation.
2. Method of recording information:
a) From Item 1 to Item 3: record the citizen's information according to the guidance provided in Clause 2, Article 7 of this Circular;
b) Item "Content of Information Adjustment": clearly record the date, month, and year when the citizen has changes in their registered place of permanent residence or other changes (note: for the new place of permanent residence, it should be fully and accurately recorded according to the Household Registration Change Report; for other changes, they should be recorded according to the decision of the competent authority allowing changes to the citizen's information);
c) Item "Date...Month...Year...": clearly record the date, month, and year when the citizen fills out the Information Adjustment Form for Citizen Identity Cards;
d) Item "Approval of Unit Head": The head of the citizen identity card management agency responsible for adjusting the information must approve the information collected in the citizen's Information Adjustment Form for Citizen Identity Cards;
đ) Item "Person whose information is adjusted": The person whose information is being adjusted must check the information collected in the Information Adjustment Form for Citizen Identity Cards, then sign and clearly write their full name.
Article 16. Transfer Form for Citizen Identity Card Files (CC10)
1. The CC10 form is used by the agency receiving applications for the issuance, replacement, or reissuance of citizen identity cards to transfer citizen identity card files in cases where citizens move their registered place of permanent residence to other provinces or cities, and in cases where citizens are issued citizen identity cards outside their registered place of permanent residence, these files will be transferred to the citizen identity card archive management agency under the provincial public security organ where the citizen is registered for permanent residence.
2. Method of recording information:
a) Item "Respectfully sent to": clearly record the unit receiving the citizen identity card file;
b) From Item 1 to Item 4: record the citizen's information according to the guidance provided in Clause 2, Article 7 of this Circular;
c) Item "Proposal": briefly and specifically record the exchange of opinions between the sending unit and the receiving unit of the citizen identity card file;
d) Item "Please return this form to": fully and specifically record the name of the sending unit of the citizen identity card file;
đ) Item "Date...Month...Year...": clearly record the date, month, and year of sending the Transfer Form for Citizen Identity Card Files and the date, month, and year of receiving the Transfer Form for Citizen Identity Card Files.
Article 17. File Bag for Citizen Identity Card Files (CC11)
1. The CC11 form is used by the citizen identity card file management agency to store documents, including: Citizen Identity Card Application Form, Information Collection Form for Citizen Identity Cards, Transfer Form for Citizen Identity Card Files, Information Adjustment Form, and other related documents concerning the issuance and management of citizen identity cards.
2. Method of recording information:
a) Item "Citizen Identity Card Number": fully and specifically record the 12-digit Citizen Identity Card number;
b) Item "Surname, Middle Name, Given Name"; "Gender"; "Date of Birth"; "Place of Permanent Residence"; "Native Place": record the citizen's information according to the guidance provided in Clause 2, Article 7 of this Circular;
c) Item "Date of File Creation": clearly record the specific date, month, and year of creating the citizen identity card file;
d) Item "File Creator"; "Creating Agency": fully and specifically record the name of the file creator and the creating agency of the citizen identity card file;
đ) Item "File Includes": fully and specifically record the documents stored in the citizen identity card file.
Article 18. Civilian Information Collection Form (DC01)
1. The DC01 model is used for citizens to declare their personal information to collect and update into the National Population Database.
2. Method of recording information:
a) The field "Province/City; District/County/Municipality; Commune/Ward/Town; Village/Hamlet/Ban/Phum/Soc/Group; Village/Street; Neighborhood/House Number": record the full administrative place name according to the birth certificate; if there is no birth certificate, record according to the household registration book;
b) The fields "Surname, middle name and given name at birth/Surname, middle name and given name of father/Surname, middle name and given name of mother/Surname, middle name and given name of spouse"/ Surname, middle name and given name of the legal representative/Surname, middle name and given name of the head of household"; "ID number/CCCD number"; "Place of birth registration"; "Native place"; "Ethnic group"; "Religion"; "Nationality"; "Permanent residence/Current address": record as directed in the instructions section of the form (if available);
c) The field "Date of Birth"; "Gender": record according to the guidance provided in Points a and b, Clause 2, Article 7 of this Circular;
d) The field "Blood type": in cases where citizens request updates and have a conclusion from a blood type test. For which blood type, mark "X" in the corresponding box;
đ) The field "Marital status": in cases where citizens are unmarried, married, or divorced, mark "X" in the corresponding box;
e) The field "Date of declaration": clearly record the date, month, and year when the citizen fills out the Civilian Information Collection Form; in cases where the person does not know how to write or cannot fill it out themselves, follow the provisions set forth in Clause 2, Article 6 of this Circular.
3. The field "Head of Police Station": The Head of the Police Station has the responsibility to confirm the Civilian Information Collection Form of citizens residing within their jurisdiction.
4. The field "Area Police Officer/Police Officer": The Area Police Officer or Police Officer has the responsibility to confirm the Civilian Information Collection Form of citizens under their management.
Article 19. Civilian Information Update and Amendment Form (DC02)
1. The DC02 model is used for citizens to declare changes in personal information as stipulated in Clause 1, Article 9 of the Law on Identity Cards for Citizens.
2. Method of recording information:
a) The fields "Surname, middle name and given name"; "CCCD number/ID number"; "Permanent residence": record as directed in the instructions section of the form;
b) The field "Date of Birth": record the citizen's information according to the guidance provided in Point b, Clause 2, Article 7 of this Circular;
c) The field "Gender": if male, mark "X" in the "Male" box, if female, mark "X" in the "Female" box;
d) The field "Content of updated and amended information": record according to the decision of the competent authority regarding the change in the citizen's information;
đ) The field "Attached files and documents": record all attached files and documents clearly and completely with the Civilian Information Update and Amendment Form;
e) The field "Proposing officer": the police officer of the Administrative Order Maintenance Police Team of the County Police Department, upon receiving the file for inspection and verification of the citizen's information, must propose to the County Police Department leadership;
g) The field "Approval of the agency managing the National Population Database": the representative of the agency managing the National Population Database signs, writes their full name, and stamps.
Chapter III
IMPLEMENTATION
Article 20. Effective Date and Transitional Provisions
1. This Circular takes effect from February 2, 2016, and replaces Circular No. 05/2014/TT-BCA dated November 22, 2014, issued by the Minister of Public Security on forms used in the issuance and management of Identity Cards.
2. Places that have not yet had the conditions to implement the issuance of Citizen Identity Cards according to the Law on Identity Cards for Citizens shall continue to use the forms for the issuance and management of Identity Cards as stipulated in Decision No. 998/2001/QĐ-BCA(C11) dated October 10, 2001, issued by the Minister of Public Security on the promulgation of forms used in administrative work related to social order, and must unify implementation according to this Circular no later than January 1, 2020.
Article 21. Responsibility for Implementation
1. The Director of the Criminal Police General Corps shall be responsible for guiding and inspecting the implementation of this Circular.
2. Directors of General Departments, heads of units directly under the Ministry of Public Security, Provincial Police Chiefs, Fire Prevention and Control Chiefs of centrally governed cities, and relevant agencies, organizations, and individuals are responsible for implementing this Circular.
During the implementation process, if there are difficulties or obstacles, local police units shall report to the Ministry of Public Security (through the General Police Department) for timely guidance./.
