Based on the Inspection Law dated June 15, 2004;

THE MINISTER OF PUBLIC SECURITY

Based on the Complaints and Reports Law dated December 2, 1998 and the Laws amending and supplementing certain articles of the Complaints and Reports Law dated June 15, 2004 and November 29, 2005;

Based on the People's Public Security Law dated November 29, 2005;

Based on Decree No. 41/2005/NĐ-CP dated March 25, 2005 detailing and guiding the implementation of certain articles of the Inspection Law;

Based on Decree No. 136/2006/NĐ-CP dated November 14, 2006 detailing and guiding the implementation of the Complaints and Reports Law and the Laws amending and supplementing certain articles of the Complaints and Reports Law;

Based on Decree No. 63/2006/NĐ-CP dated June 23, 2006 on the organization and operation of public security inspections;

Based on Decree No. 136/2003/NĐ-CP dated November 14, 2003 detailing the functions, tasks, powers, and organizational structure of the Ministry of Public Security;

Upon consideration of the proposal of the Inspector General of the Ministry of Public Security and the Director of the Legal Department of the Ministry of Public Security,

This Decision promulgates the following forms for use in inspection activities and handling complaints and reports by the People's Public Security force:

Public Security units and localities when printing the forms prescribed in Article 1 of this Decision must print them accurately according to the content and format, and uniformly on A4 size paper.

Article 2. This Decision takes effect from July 1, 2007 and replaces Decision No. 670/2000/QĐ-BCA(V24) dated August 14, 2000 of the Minister of Public Security promulgating the system of model documents and statistics used in public security inspection activities.

Article 3. The Inspector General of the Ministry shall be responsible for inspecting, guiding, and urging the implementation of this Decision.

Article 4. Comrades General Directors, Heads of units directly under the Minister, Provincial Public Security Directors, and Municipal Public Security Directors directly under the Central Government shall be responsible for implementing this Decision.
