Based on the Law on Residence dated November 29, 2006;
Based on Decree No. 136/2003/NĐ-CP dated November 14, 2003 stipulating the functions, tasks, powers, and organizational structure of the Ministry of Public Security;
At the proposal of the Director General of the Police General Department,
DECISION:
Article 1. Issuing fifteen forms for use in registration and management of residence, including:
1. Household Registration Form (code HK01).
2. Change of Household Registration and Population Report Form (code HK02).
3. Verification of Household Registration and Population Form (code HK03).
4. Information on Changes to Household Registration and Population Form (code HK04).
5. Temporary Absence Declaration Form (code HK05).
6. Household File Tracking Form (code HK06).
7. Household Registration Transfer Certificate (code HK07).
8. Household Booklet (code HK08).
9. Temporary Residence Booklet (code HK09).
10. Household Resolution Tracking Booklet (code HK10).
11. Permanent Residence Registration Booklet (code HK11).
12. Temporary Residence Registration Booklet (code HK12).
13. Reception and Stay Record Booklet (code HK13).
14. Household File Folder (code HK14).
15. Household and Population Statistics (code HK15).
Article 2. The content, format, and specifications of the forms shall be uniformly implemented according to the model proposed for approval by the Standardization Council of the National Police总局。附录另行提供。(The appendix will be provided separately.)
Article 3. This Decision takes effect from July 20, 2007, replacing Clause 2 of Article 1 regarding the forms for registration and management of household registration in Decision No. 998/2001/QĐ-BCA(C11) dated October 10, 2001 of the Minister of Public Security on the issuance of various forms for use in administrative work related to social order management. Existing forms currently in stock may continue to be used until exhausted, after which these forms under this Decision shall be uniformly adopted.
Article 4. The Director of the National Police Administration is responsible for guiding the use, planning printing, inspecting, urging the implementation, and ensuring uniform implementation nationwide.
Article 5. Directors of the General Corps, Heads of units directly under the Minister, and Provincial Police Chiefs and Chief of Municipalities directly under the Central Government are responsible for implementing this Decision./.
