Supplementing and amending forms used in criminal proceedings activities of the People's Public Security Forces issued together with Decision number 1351/2004/QĐ-BCA dated November 18, 2004 of the Minister of Public Security
Issued pursuant to the Ordinance on Criminal Investigation Organization;
Pursuant to Decree number 136/2003/NĐ-CP dated November 14, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Public Security;

Pursuant to the Criminal Procedure Code 2003;
At the proposal of the Director of the Police General Corps, the Director of the Security General Corps,
This Decision supplements 44 forms and amends 15 forms used in criminal proceedings activities of the People's Public Security Forces issued under Decision number 1351/2004/QĐ-BCA dated November 18, 2004 of the Minister of Public Security (with a list of supplemented and amended forms attached).
The printing and distribution of these forms must comply with the requirements set out in the List of Amended and Supplemented Forms for Use in Criminal Proceedings Activities issued together with this Decision.
DECISION:
Article 1. The Directors of the General Corps, Heads of units directly under the Minister, and Provincial Public Security Directors are responsible for implementing this Decision./.

Article 3. The Standard Measurement Quality Control Department shall be responsible for organizing and guiding the implementation of the Regulations adopted herein.
Article 4. The Directors of the Departments, Heads of units directly under the Minister, and Provincial Police Chiefs under the direct jurisdiction of the Central Government are responsible for implementing this Decision./.
