Regarding the issuance of the Drug Rehabilitation Operation Permit
THE MINISTER OF LABOR, WAR INVALIDS AND SOCIAL AFFAIRS
Pursuant to Decree No. 29/2003/NĐ-CP dated March 31, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Labor, Invalids and Social Affairs;
Pursuant to Decree No. 147/2003/NĐ-CP dated December 2, 2003 of the Government stipulating conditions, procedures for issuing permits, and management of voluntary drug rehabilitation facilities;
Pursuant to Circular Joint No. 15/2004/TTLT-BLĐTBXH-BYT dated November 12, 2004 of the joint Circular of the Ministry of Labor, Invalids and Social Affairs and the Ministry of Health guiding the implementation of certain provisions of Decree No. 147/2003/NĐ-CP dated December 2, 2003 of the Government stipulating conditions, procedures for issuing permits, and management of voluntary drug rehabilitation facilities;
Considering the proposal of the Director of the Department of Prevention and Control of Social Evils,
DECISION:
Article 1. The Department of Prevention and Control of Social Evils shall issue and manage the Drug Rehabilitation Operation Permit according to Model No. 5 issued pursuant to Circular Joint No. 15/2004/TTLT-BLĐTBXH-BYT dated November 12, 2004 of the Ministry of Labor, Invalids and Social Affairs and the Ministry of Health guiding the implementation of certain provisions of Decree No. 147/2003/NĐ-CP dated December 2, 2003 of the Government stipulating conditions, procedures for issuing permits, and management of voluntary drug rehabilitation facilities.
The printing costs of the permit shall be funded from the annual budget for drug control directed by the Department of Prevention and Control of Social Evils.
Article 2. The Department of Prevention and Control of Social Evils shall be responsible for reviewing the conditions and files of rehabilitation facilities that apply to the Minister of Labor, Invalids and Social Affairs for issuance, extension, modification, or revocation of the Drug Rehabilitation Operation Permit.
Article 3. The Head of the Ministry's Office, the Director of the Planning and Finance Department, the Director of the Cadre and Civil Servant Organization Department, the Director of the Department of Prevention and Control of Social Evils, and the Heads of related units shall be responsible for implementing this Decision.
