Pursuant to Decree No. 37/CP dated June 9, 1998 of the Government stipulating the functions and tasks of the Ministry of Public Security;

Pursuant to Decree No. 05/CP dated February 3, 1999 of the Government on Identity Cards;

Pursuant to Decree No. 51/CP dated May 10, 1997 of the Government on household registration and management;

Pursuant to Decree No. 58/CP dated August 24, 2001 of the Government on seal management and usage;

Pursuant to Decree No. 47/CP dated August 12, 1996 of the Government on weapons, explosives, and auxiliary tools management;

Pursuant to the Regulation on Community Police Officers issued pursuant to Decision No. 118/QĐ-BCA(C13) dated June 29, 1994;

Pursuant to Decree No. 36/CP dated July 10, 2001 of the Government on ensuring traffic order and safety on roads and urban traffic order and safety;

Pursuant to Decision No. 600/QĐ-BCA(C11) dated September 18, 1998 of the Minister of Public Security promulgating the working system, file retention, and information management for community police officers;

To unify the use of forms in administrative work on social order management.

At the proposal of the Director General of the Police General Department,

DECISION:

Article 1. Now hereby issue various types of forms and specialized registers collectively referred to as forms to be used in administrative work on social order management, comprising six groups:

1. Forms used in the issuance of Identity Cards and Travel Documents.

2. Forms used in household registration and management.

3. Forms used in seal registration and management.

4. Forms used in weapons and auxiliary tool registration and management.

5. Forms used in the work of Community Police Officers.

6. Forms used in the work of Traffic Police.

Article 2. The format, content, and specifications of the forms shall be uniformly implemented according to the model proposed by the Standardization Council of Forms of the National Police总局，按照给定的格式输出：the format specified in the accompanying list).

Article 3. The National Police Corps shall be responsible for providing guidance to ensure uniform implementation nationwide.

Article 4. This Decision takes effect from the date of signature. Existing forms currently in stock may continue to be used if they are suitable; when these are exhausted, all forms shall uniformly be those according to this Decision./.
