Article 1. This Decision promulgates eight types of household registration books and fifty-seven types of civil status forms (hereinafter referred to as household registration books and civil status forms), which are listed and numbered in the attached List.
The household registration books and civil status forms prescribed herein shall be uniformly used throughout the country.
Article 2
1. The Director of the Legal Publishing House shall be responsible for coordinating with the Director of the Administrative Justice Department to organize the printing and distribution of sixteen types of civil status forms specified in Part I of the List, meeting the requirements for civil status registration and management at the local level, including the following civil status forms:
- Civil status forms on birth: BTP/HT-2006-KS.1, BTP/HT-2006-KS.1.a, BTP/HT-2006-KS.2, BTP/HT-2006-KS.2.a, BTP/HT-2006-KS.3, BTP/HT-2006-KS.4;
- Civil status forms on marriage: BTP/HT-2006-KH.1, BTP/HT-2006-KH.1.a, BTP/HT-2006-KH.2, BTP/HT-2006-KH.2.a, BTP/HT-2006-KH.3, BTP/HT-2006-KH.3.a;
- Civil status forms on death: BTP/HT-2006-KT.1, BTP/HT-2006-KT.1.a, BTP/HT-2006-KT.2, BTP/HT-2006-KT.2.a;
2. The Directors of the Justice Departments of the provinces and centrally governed cities shall organize the printing and distribution of eight types of household registration books and forty-one types of civil status forms as specified in Part II of the List in accordance with the format and content stipulated in the accompanying civil status forms.
Article 3. The issuance and sale of household registration books and civil status forms must comply with the prescribed price.
Strictly prohibit the printing, distribution, and use of household registration books and civil status forms contrary to the provisions of this Decision.
Article 4. This Decision takes effect fifteen days from the date of publication in the Official Gazette and replaces Decision No. 1203 QĐ/TP-HT dated December 26, 1998 of the Minister of Justice on the issuance of civil status forms and household registration books.
Article 5. The Director of the Administrative Justice Department, the Director of the Legal Publishing House, and the Directors of the Justice Departments of the provinces and centrally governed cities are responsible for implementing this Decision./.
