Decision No. 01/2008/QD-BTP On the issuance of certain forms of documents for notarization activities

DECISION OF THE MINISTER OF JUSTICE

ON ISSUING CERTAIN FORMS OF DOCUMENTS USED IN NOTARIAL ACTIVITIES

THE MINISTER OF JUSTICE

BASED ON THE Notary Law number 82/2006/QH11 dated November 29, 2006;

Pursuant to Decree No. 02/2008/ND-CP dated January 4, 2008 of the Government detailing and guiding the implementation of certain provisions of the Notarization Law;

Pursuant to Decree No. 62/2003/ND-CP dated June 6, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Justice;

At the proposal of the Director of the Administrative Justice Department,

DECISION:

Article 1. ATTACHING TO THIS DECISION ARE THE FOLLOWING 09 FORMS OF DOCUMENTS USED IN NOTARIAL ACTIVITIES:

1. Application for Appointment of a Notary Public (Form TP-CC-01);

2. Application for Establishment of a Notary Office (for a Notary Office established by one Notary Public) (Form TP-CC-02);

3. Application for Establishment of a Notary Office (for a Notary Office established by two or more Notary Publics) (Form TP-CC-03);

4. Registration Form for Operation of a Notary Office (for a Notary Office established by one Notary Public) (Form TP-CC-04);

5. Registration Form for Operation of a Notary Office (for a Notary Office established by two or more Notary Publics) (Form TP-CC-05);

6. Decision on Permission to Establish a Notary Office (for a Notary Office established by one Notary Public) (Form TP-CC-06);

7. Decision on Permission to Establish a Notary Office (for a Notary Office established by two or more Notary Publics) (Form TP-CC-07);

Certificate of Operation of a Notary Office (for a Notary Office established by one Notary Public) (Form TP-CC-08);

Certificate of Operation of a Notary Office (for a Notary Office established by two or more Notary Publics) (Form TP-CC-09).

Article 2. This Decision shall take effect 15 days from the date of publication in the Official Gazette.

Article 3. Provincial People's Committees, the Director of the Administrative Justice Department, Heads of Units under the Ministry, Directors of Justice Departments of provinces and centrally governed cities, and those concerned are responsible for implementing this Decision./.
