Pursuant to Circular No. 02/2008/TT-BNG dated February 4, 2008 of the Ministry of Foreign Affairs guiding the issuance, extension, amendment, and supplementation of diplomatic passports and official passports within the country and abroad under Decree No. 136/2007/NĐ-CP dated August 17, 2007 of the Government on exit and entry of Vietnamese citizens;
Pursuant to Decision No. 1622/2008/QD-BNG dated June 23, 2008 of the Minister of Foreign Affairs stipulating the functions, tasks, powers, and organizational structure of the Consular Department
Considering the proposal of the Head of the Exit and Entry Department of the Consular Department;

Article 1. This Decision hereby promulgates the Form for applying for issuance, extension, amendment, and supplementation of diplomatic passports and official passports within the country and abroad attached hereto.
Article 2. This Decision shall take effect from the date of signing.
Article 3. The Exit and Entry Department of the Consular Department, the Department of Foreign Affairs of Ho Chi Minh City, and overseas Vietnamese representative offices shall be responsible for implementing this Decision.
