THE MINISTER OF JUSTICE
Pursuant to Decree No. 62/2003/NĐ-CP dated June 6, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Justice;
Pursuant to Decree No. 06/2012/NĐ-CP dated February 2, 2012 of the Government amending and supplementing certain articles of decrees on civil status, marriage and family, and notarization;
After reaching consensus with the Minister of Foreign Affairs;
At the proposal of the Director of the Administrative Justice Department and the Director of the Legal Publishing House.
DECISION:
Article 1.
This Decision promulgates eight types of household registration books and thirty-eight types of household registration forms (hereinafter referred to as household registration books and forms) listed and numbered in the Household Registration Forms and Household Registration Books Catalogue (hereinafter referred to as the Catalogue) attached hereto.
The household registration books and forms prescribed herein shall be uniformly used at Diplomatic Representative Agencies and Consular Offices of Vietnam abroad (hereinafter referred to as Diplomatic Representative Agencies).
Article 2
1. The Director of the Legal Publishing House shall be responsible for coordinating with the Director of the Administrative Justice Department to organize printing and distribution of nine types of household registration forms specified in Part I of the Catalogue upon request of the Consular Affairs Bureau Director of the Ministry of Foreign Affairs, including the following household registration forms:
- Birth registration forms: BTP-NG/HT-2007-KS.1, BTP-NG/HT-2007-KS.1.a, BTP-NG/HT-2007-KS.2.
- Marriage registration forms: BTP-NG/HT-2007-KH.1, BTP-NG/HT-2007-KH.1.a, BTP-NG/HT-2007-KH.2, BTP-NG/HT-2007-KH.2.a.
- Death registration forms: BTP-NG/HT-2007-KT.1, BTP-NG/HT-2007-KT.1.a.
2. Diplomatic Representative Agencies shall be responsible for issuing eight types of household registration books and twenty-nine types of household registration forms specified in Part II of the Catalogue.
Individuals wishing to register household matters may access and print the household registration application forms on the Administrative Procedures webpage of the Ministry of Justice's website (http://www.moj.gov.vn) for use.
3. Household registration books and forms must be printed strictly according to the specifications and contents prescribed in the household registration books and forms issued pursuant to this Decision.
Article 3.
Strictly prohibited is the printing, distribution, and use of household registration books and forms contrary to the provisions of this Decision.
Article 4.
This Decision takes effect fifteen days from the date of publication in the Official Gazette.
Article 5.
The Director of the Administrative Justice Department, the Director of the Legal Publishing House, and the Heads of Diplomatic Representative Agencies are responsible for implementing this Decision./.
