- BASED ON Articles 182 and 183 of the Labor Code dated JUNE 23, 1994;
- BASED ON Article 43 of Decree No. 12/CP dated JANUARY 26, 1995 on the issuance of the Social Insurance Regulations;

THE MINISTER OF LABOR, WAR INVALIDS AND SOCIAL AFFAIRS
- Pursuant to the Government Decree No. 96/CP dated December 7, 1993 regarding the functions, tasks, powers, and organizational structure of the Ministry of Labor - Social Affairs and War Invalids;
NOW ISSUES HEREBY ATTACHED TO THIS DECISION the model Social Insurance Booklet to be uniformly applied throughout the country for recording the work history and social insurance contributions of social insurance participants, serving as the basis for resolving social insurance benefits according to the provisions of the law.
The Social Insurance Booklet measures 18 cm in length and 12 cm in width, with a dark blue cover bearing the golden inscription "Socialist Republic of Vietnam" and "Social Insurance Booklet". Inside, there are 44 pages for filling out personal resumes of social insurance participants, recording their work history and social insurance contributions, and listing the social insurance benefits they have received. The white pages are adorned with blue patterns.

Article 1. The Vietnam Social Security is responsible for printing and distributing the Social Insurance Booklets to each social insurance participant. The distribution of Social Insurance Booklets will be subject to a fee (including the cost of printing the booklet and transportation fees) from each social insurance participant.
Article 2. The Director of the Ministry's Office, the Head of the Social Insurance Department, and the Director of the Vietnam Social Security are responsible for implementing this decision.
Article 3. The Vietnam Social Security is responsible for printing and organizing the distribution of the Social Insurance Booklet to each individual participating in social insurance. The issuance of the Social Insurance Booklet shall be subject to a fee (including the cost of printing the booklet and transportation fees) for each individual participating in social insurance.
Article 4. The Director of the Ministry's Office, the Director of the Social Insurance Department, and the Director of the Vietnam Social Security are responsible for implementing this decision.
