CIRCULAR
ISSUING CERTAIN APPLICATION PROCEDURAL FORMS
IN THE FIELD OF SPORTS

Pursuant to the Law on Physical Education and Sports No. 77/2006/QH11 dated November 29, 2006;
“1. Position
BASED ON THE DECREE NUMBER 05/2022/ND-CP dated January 7, 2022 amending and supplementing certain provisions of the Decree Number 53/2019/ND-CP dated June 17, 2019 of the Government; pursuant to Decision No. 76/2013/NĐ-CP dated July 16, 2013 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Culture, Sports and Tourism;
At the request of the Director of the Legal Department,
The Minister of Culture, Sports and Tourism issues this Circular issuing certain application procedural forms in the field of sports.
Article 1. Scope of Regulation
This Circular issues certain application procedural forms in the field of sports.
Article 2. Applicability
Agencies, organizations, and individuals meeting the conditions to participate in activities related to administrative procedures as prescribed by laws on sports.
Article 3. Forms issued together with this Circular
1. Issuing seven forms with the following names and codes:
a) Application for hosting regional, continental, and world sports congresses; championships of each sport in the region, continent, and world held in Vietnam; national sports and physical culture congresses: Form No. 01;
b) Application for hosting national championships, national youth tournaments (held annually for each sport): Form No. 02;
c) Application for hosting championships of each sport (at provincial level): Form No. 03;
d) Application for obtaining a certificate of eligibility to operate sports activities for professional sports clubs, businesses operating sports activities: Form No. 04;
đ) Certificate of eligibility to operate sports activities for professional sports clubs, businesses operating sports activities: Form No. 05;
e) Summary of preparation for operating sports activities: Form No. 06;
g) Professional training certificate (issued to Physical Culture and Sports Collaborators): Form No. 07.
2. The forms prescribed in Points a, b, c, d, đ, and e of Clause 1 of this Article shall be designed for A4 paper size. For the professional training certificate prescribed in Point g of Clause 1 of this Article, the specified dimensions are: 130 mm x 190 mm.
Article 4. Effective date
This Circular takes effect from February 1, 2015.
Article 5. Responsibilities for Implementation
1. Heads of agencies and units under the Ministry of Culture, Sports and Tourism, Directors of Provincial Departments of Culture, Sports and Tourism, and relevant organizations and individuals are responsible for implementing this Circular.
2. In the course of implementation, if there are difficulties, agencies, organizations, and individuals shall promptly report them to the Ministry of Culture, Sports and Tourism for study and amendment./.
