Issuing Technical Procedures for Safety Inspection of K359-60 Demounting Support
K359-60 Used in the Ministry of National Defense (Safety Inspection Procedure 03:2017/MND)

Pursuant to Decree No. 39/2016/NĐ-CP dated May 15, 2016 of the Government detailing implementation of certain provisions of the Law on Labor Safety and Health;

At the proposal of the Director of the General Department of Technology;

The Minister of National Defense hereby issues the Technical Safety Inspection Procedures for machinery and equipment with strict requirements for labor safety within the Ministry of National Defense.

Demounting Support K359-60.

QTKĐ:0

1. SCOPE AND APPLICABLE SUBJECTS

1.1. Scope of Application

These technical safety inspection procedures apply to initial, periodic, and extraordinary technical safety inspections of demounting supports K359-60 listed in the Catalogue of Machinery, Equipment, and Materials with Strict Requirements for Military Occupational Safety issued by the Ministry of National Defense.

Based on these procedures, technical safety inspection units and inspectors within the Ministry of National Defense shall directly apply them or develop detailed procedures for each type of demounting support K359-60 without contravening the provisions of these procedures. Management and user organizations, enterprises, and individuals of demounting supports K359-60 must cooperate with inspection units and inspectors in accordance with the law.

1.2. Applicability

- Organizations, units, enterprises, institutions, and individuals managing and using demounting supports K359-60 within the Ministry of National Defense (collectively referred to as the facility);

- Relevant state management agencies.

+ TCVN 6174: 1997: Industrial explosives - Safety requirements for production, blasting tests, and acceptance;

2. FORMS OF INSPECTION

The technical safety inspection procedures for demounting supports K359-60 must be fully implemented in the following cases:

+ TCVN 4756 - 1989: Grounding and ungrounding regulations for electrical equipment;

+ TCVN 6734:2000: Electrical equipment used in mines and tunnels - Safety requirements for structure and use;

+ Electrical Equipment Regulations Part I. General Provisions - 11TCN-18-2006;

Including national technical standards and regulations on safety as follows:ANNEX

- QCVN 01:2011/BCT. National Standard on Safety in Underground Mining Operations;

- QCVN 7:2012/BLĐTBXH, National Technical Regulations on Safety for Lifting Equipment;

- QCVN 25:2015/BLĐTBXH, National Technical Regulations on Safety for Forklifts;

- QCVN 02:2008/BTC, National Technical Regulations on Safety in Storage, Transportation, Use, and Disposal of Industrial Explosives;

- TCVN 7550:2005. Steel Cables for Machine Bodies - Minimum Requirements;

In cases where the national technical standards and national technical regulations cited in this inspection procedure are supplemented, amended, or replaced, they shall be applied according to the latest document.

4. TERMS AND DEFINITIONS

Inspections of technical safety criteria for demounting supports K359-60 may follow other standards when requested by the user or manufacturer, provided that such standards have technical safety criteria equal to or higher than those specified in the national standards cited in these procedures.

4. TERMS AND DEFINITIONS

This procedure uses terms and definitions from the referenced documents and some terms and definitions in this procedure are understood as follows:

4.1. Demounting Support K359-60 It is a type of lifting device consisting of a lifting mechanism (in many cases equipped with a moving mechanism) driven by hand through chains and a speed reducer, wire rope hoists used for lifting and lowering missiles and torpedoes.

4.2. Initial technical safety inspection is an activity to assess the technical safety condition of demounting supports K359-60 according to technical safety standards and regulations at installation, before first use.

4.3. Periodic technical safety inspection is an activity to assess the technical safety condition of demounting supports K359-60 according to technical safety standards and regulations after the expiration of the previous inspection period.

4.4. Extraordinary technical safety inspection is an activity to assess the technical safety condition of demounting supports K359-60 according to technical safety standards and regulations when:

- After repair, upgrade, or modification affecting the technical safety condition of demounting supports K359-60;

- After changing the installation location;

- When requested by the facility or competent authority.

5. INSPECTION STEPS

When conducting technical safety inspections of demounting supports K359-60, the following steps must be carried out sequentially:

Step 1. Review documentation and history

Step 2. External technical inspection

Step 3. Technical inspection - No-load test

Step 4. Load testing modes - Testing methods

Step 5. Processing inspection results.

Note:

The subsequent inspection steps can only be conducted if the results of the previous step meet the requirements. All inspection results from each step must be fully recorded in the on-site record book according to the model specified in Appendix I and kept complete at the inspection organization. 6. 6. TESTING EQUIPMENT

6.1. Equipment and tools for examination:

- A magnifying glass with appropriate magnification;

- Mechanical measuring tools: Length measurement, diameter measurement, gap measurement.

- Load testing weighing device (force gauge).

6.3. Measuring equipment

- Cable and steel quality testing equipment;

- Distance measuring equipment;

- Longitudinal and rotational speed measuring equipment;

- Chain and cable quality testing equipment;

- Weld quality testing equipment;

7. INSPECTION CONDITIONS

7. INSPECTION CONDITIONS

When conducting inspections of demounting supports K359-60, the following conditions must be met:

7.1. The demounting support K359-60 must be in a state ready for inspection.

7.2. The technical documentation of the demounting support K359-60 must be complete as required.

7.4. Inspection equipment and tools must be complete and suitable for the object being inspected.

7.5. Safety and labor hygiene conditions must be met to operate cables and cranes.

7.5. Conditions for labor safety and hygiene must be met to operate the demounting support K359-60.

8. PREPARATION FOR INSPECTION

8.1. Unify the inspection plan, preparatory work, and coordination between the inspection unit and the facility as follows:

8.1.1. Prepare the technical documentation of the demounting support K359-60;

8.1.2. Clean the demounting support K359-60;

8.1.3. Prepare human and material resources for inspection; assign personnel to participate and witness the inspection;

8.2. Checking Documentation and History

Depending on the form of inspection, check and review the following documents:

8.2.1. For initial inspection:

a) Check the records and history

- History and technical documentation of the demounting support K359-60 (evaluated according to the provisions at 1.3.2, 3.5.1.5 QCVN 07:2012/BLĐTBXH and 1.4 TCVN 4244:2005) including:

+ Brake mechanism drawing - technical requirements;

+ Operating procedures.

- Check the documentation:

+ Strength calculation of load-bearing parts (if applicable);

+ Manufacturing drawings with all key dimensions recorded;

+ Operating, maintenance, and repair guidelines;

- Factory release documentation of the equipment (if available);

+ Certificates regarding the manufacturing metal, welding metal (as specified in Clause 3.1.2 TCVN 4244:2005);

+ Weld quality inspection results (as specified in Clause 3.3.4 TCVN 4244:2005);

+ Factory release test report;

- Installation documentation (if available);

- Conformity certificate issued by the designated organization according to regulations (in case applicable);

- History, inspection report, and previous inspection result sheet;

- Usage management, operation, maintenance documentation; inspection and supervision records (if available).

- Documentation on management, usage, operation, maintenance; inspection and testing reports (if available);

- In case of repair or renovation: Design documentation for repair and renovation, acceptance records after repair and renovation of cables and cranes;

- In case of repair, modification, or upgrade: Repair design documentation, modification design documentation, upgrade design documentation, post-repair acceptance report for dismantled support K359-60;

- In case of installation position change: Consider supplementing installation documentation;

- Inspection and testing results, and implementation of recommendations from previous inspections and tests (if available);

Evaluation:

The result of file review and evaluation of the dismantled support K359-60's record meets requirements when complete and comply with the provisions at Clause 8.2 of this procedure. If not met, the facility must take supplementary corrective measures;

8.4. Develop and unify the implementation of safety measures with the facility before inspection. Equip sufficient personal protective equipment to ensure safety during the inspection.

9. CONDUCTING INSPECTION

9. PERFORMING INSPECTION

When performing inspection of the dismantled support K359-60, it must be carried out in the following sequence:

9.1. External Technical Inspection

9.1.1. Inspect the equipment installation location, user instruction sign, layout plan, distance, safety measures, and obstacles that need attention throughout the inspection process;

9.1.2. Check the compatibility and synchronization of the dismantled support K359-60 components with the documentation and record;

9.1.3. Examine sequentially and comprehensively all structures and parts of the dismantled support K359-60, particularly focusing on the following parts:

a) Metal structure of the dismantled support K359-60: Important load-bearing welds, riveted joints (if present) (conducted according to the provisions in Appendix 6 TCVN 4244:2005);

b) Hooks and hook detail parts (inspected and evaluated according to the provisions in Appendices 13A, 13B, 13C TCVN 4244:2005);

c) Chain drive, chain puller, and fixed chain end parts (meeting the manufacturer's requirements or referring to the provisions in Appendix 7 TCVN 4244:2005);

d) Inspect pulleys, shafts, and fixed shaft parts (inspected and evaluated according to Appendices 19A, 20A, 20B TCVN 4244:2005): Inspect the part preventing the chain from jumping off the pulley; inspect the gap between the side face of the pulley and the protective cover;

e) Brakes and holding devices must be inspected according to Clause 1.5.3.3 TCVN 4244:2005.

Evaluation:

The external technical inspection results of the dismantled support K359-60 meet requirements when installed according to the design drawings, comply with the standards and regulations mentioned above, show no damage, and meet the requirements at Clause 9.1 of this standard.

9.2. Technical Inspection - No-load Test

No-load testing (according to Clause 4.3.2 TCVN 4244:2005) includes

- Testing the operation of all mechanisms and equipment: All mechanisms, safety devices, holding devices, and other equipment;

- These tests must be conducted no less than three times.

Evaluation:

The no-load testing results of the dismantled support K359-60 meet requirements when the system's mechanisms and safety devices operate within the designed specifications and ensure reliability.

9.3. Load testing methods

9.3.1. Static load testing:

- Test load: 150% SWL (safe working load) but not exceeding the design load and must be consistent with the actual quality of the dismantled support K359-60;

- Test duration is 10 minutes;

- During loading, check the operation of the overload limiting device (if present) at these positions. The overload control device must prevent mechanisms from continuing to operate beyond the safe limit of the equipment and only allow them to operate in reverse to bring the load to a safer state.

Evaluation:

The static load testing results of the dismantled support K359-60 meet requirements when during the testing period, there is no movement, the system shows no cracks, permanent deformation, or other damages, and complies with the provisions at Clause 4.3.2 TCVN 4244:2005.

9.3.2. Dynamic load testing:

- Test load: 125% SWL (safe working load) but not exceeding the design load and must be consistent with the actual quality of the dismantled support K359-60;

- Conduct all lifting and lowering mechanism tests no less than three times and inspect the operational status of the mechanisms and working structures.

Evaluation:

The dynamic load testing results of the dismantled support K359-60 meet requirements when during the testing period, there is no movement, after unloading, the mechanisms and parts of the dismantled support K359-60 show no cracks, permanent deformation, or other damages, and comply with the provisions at Clauses 4.3.2 and 4.3.3 TCVN 4244:2005.

10. HANDLING INSPECTION RESULTS

10.1. Prepare an inspection report with full content according to the model prescribed in Appendix II attached to this procedure. The report must clearly record all contents and standards applied during the inspection, including any higher safety criteria requested by the equipment owner compared to those specified in the TCVN at Article 3 of this procedure (when the equipment is manufactured according to the standards and corresponding safety criteria).

10.2. Approve the inspection report

The minimum participants required to approve the inspection report must include the following members:

- A representative of the facility or a person authorized by the facility;

- A person assigned to participate and witness the inspection;

- Inspector conducting the inspection.

When the report is approved, the inspector, witnesses to the inspection, representatives of the facility, or persons authorized by the facility must sign and stamp (if applicable) the report. The inspection report is prepared in two (02) copies, each party responsible for retaining one copy.

10.3. Summarize the inspection results in the record of the dismantled support K359-60 (clearly stating the inspector's name, date of inspection, and applied standards).

10.4. Affix the inspection label: When the dismantled support K359-60 passes the technical safety inspection, the inspector affixes the inspection label to the equipment (Inspection label model according to the Ministry of National Defense regulations). The inspection label is affixed in a visible location.

10.5. Issue the Certificate of Inspection Results

10.5.1. When the dismantled support K359-60 passes the technical safety inspection, the inspection unit issues a certificate of inspection results to the equipment within five working days from the date of approval of the inspection report at the facility.

10.5.2. When the K359-60 dismantling support frame fails to meet the requirements upon inspection, only the steps specified in Articles 10.1 and 10.2 shall be carried out; only issue the inspection report to the facility, which must clearly state the reasons for the equipment not meeting the inspection requirements, recommend corrective actions by the facility, and specify the deadlines for implementing such recommendations; at the same time, send the inspection report and notification to the labor safety management agency under the Ministry of National Defense that oversees the installation and use of the equipment.

11. INSPECTION PERIOD

11.1. The periodic inspection period for fixed indoor K359-60 dismantling support frames is two years, while for outdoor and mobile ones it is one year.

For K359-60 dismantling support frames that have been in use for over twenty years, the inspection period is one year.

11.2. In cases where the manufacturer or the facility requests a shorter inspection period, it shall be carried out according to the manufacturer's and the facility's regulations.

11.3. When shortening the technical inspection period, the inspector must clearly state the reasons in the inspection report.

11.4. When the inspection period is defined in national technical standards, it shall be carried out according to the provisions of those standards.

11.5. In cases requiring irregular inspections, follow the provisions in Clause 4.4 of this procedure./.
