THE MINISTER OF HEALTH
Pursuant to Decree No. 49/2003/NĐ-CP dated May 15, 2003 of the Government, stipulating the functions, tasks, powers, and organizational structure of the Ministry of Health;
Pursuant to Decision No. 379/2002/QĐ-BYT dated February 8, 2002 of the Minister of Health promulgating the Regulations on Health Statistics;
At the proposal of the following officials: Director of the Preventive Medicine Department of Vietnam, Head of the Planning and Finance Department - Ministry of Health,
DECISION:
Article 1. Issuing additional forms on accidents and injuries into the system of forms of the Ministry of Health issued together with Decision No. 2554/2002/QĐ-BYT dated July 4, 2002 of the Minister of Health regarding the issuance of medical record books and health statistics reporting forms for application at all healthcare facilities nationwide, including:
1. Accident and Injury Report Form - (Annex 1)
2. A1/YTCS Medical Examination Register - (Annex 2)
3. A6/YTCS Cause of Death Follow-up Book - (Annex 3)
4. Accident and Injury Statistical Report at commune, ward, town (collectively referred to as commune), district, urban district, provincial city (collectively referred to as district), and province, centrally governed municipality (collectively referred to as province) - (Annex 4).
Article 2. This Decision shall take effect fifteen days from the date of publication in the Official Gazette. All previous provisions contrary to this Decision are hereby repealed.
Article 3. The following officials are responsible for implementing this Decision: Director of the Office, Inspector General, Heads of Departments, Directors of Bureaus under the Ministry of Health, Heads of subordinate units under the Ministry, Provincial Health Service Directors, centrally governed municipalities, and Health Service Heads of sectors.
