Issuing forms for Application for Registration, Request for Permit Form, Registration Certificate, and License as prescribed in the Regulation on the Management of Pay Television Services
and the model of the License prescribed in the Regulation on the Management of Pay Television Activities

MINISTER OF INFORMATION AND COMMUNICATIONS
Pursuant to the Press Law dated December 28, 1989; the Law Amending and Supplementing Certain Provisions of the Press Law dated June 12, 1999;
Pursuant to Decree No. 51/2002/NĐ-CP dated April 26, 2002 of the Government detailing the implementation of the Press Law and the Law Amending and Supplementing Certain Provisions of the Press Law;
BASED ON THE GOVERNMENT DECREE NO. 187/2007/NĐ-CP OF DECEMBER 25, 2007 ON THE FUNCTIONS, TASKS, POWERS, AND ORGANIZATIONAL STRUCTURE OF THE MINISTRY OF INFORMATION AND COMMUNICATIONS;
Pursuant to Decree No. 50/2011/NĐ-CP dated June 24, 2011 of the Government amending Decree No. 187/2007/NĐ-CP dated December 25, 2007 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Information and Communications;
Pursuant to Decision No. 20/2011/QĐ-TTg dated March 24, 2011 of the Prime Minister promulgating the Regulation on the Management of Pay Television Services;
At the proposal of the Director of the Radio, Television and Electronic Information Administration Department,
c) Enterprises may be granted permission for no more than one block out of the total three dual-frequency blocks (FDD) B
Article 1. This Circular issues the forms for Application for Registration, Request for Permit Form, Registration Certificate, and License as prescribed in the Regulation on the Management of Pay Television Services, including:
1. Form No. 1: Application for Registration of Receiving Foreign Television Signals Directly from Satellite (TVRO).
2. Form No. 2: Certificate of Registration for Receiving Foreign Television Signals Directly from Satellite (TVRO).
3. Form No. 3: Request for Permit to Produce Pay Television Channels.
4. Form No. 4: License for Producing Pay Television Channels.
5. Form No. 5: Application for Registration as an Agent Providing Foreign Channels on Pay Television.
6. Form No. 6: Certificate of Registration as an Agent Providing Foreign Channels on Pay Television.
7. Form No. 7: Request for Permit to Edit Foreign Channels on Pay Television.
8. Form No. 8: License for Editing Foreign Channels on Pay Television.
9. Form No. 9: Request for Permit to Provide Pay Television Services.
10. Form No. 10: License for Providing Pay Television Services.
11. Form No. 11: Application for Registration of Channel Program Catalogue on Pay Television Services.
12. Form No. 12: Certificate of Registration of Channel Program Catalogue on Pay Television Services.
13. Form No. 13: Application for Registration of Supplier Name with Attached Catalogue of Film Programs on Demand Pay Television Services.
14. Form No. 14: Certificate of Registration of Supplier Name with Attached Catalogue of Film Programs on Demand Pay Television Services.
Article 2. This Circular takes effect from December 15, 2011.
