THE MINISTER OF FINANCE
Pursuant to the Customs Law No. 29/2001/QH10 dated June 29, 2001 and the Law Amending and Supplementing Certain Articles of the Customs Law No. 42/2005/QH11 dated June 14, 2005;
Pursuant to Decree No. 77/2003/ND-CP dated July 1, 2003 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Finance;
Pursuant to Decree No. 66/2002/ND-CP dated July 1, 2002 of the Government stipulating the allowance for luggage of persons entering and exiting the country and gifts imported duty-free;
Pursuant to Decree No. 154/2005/NĐ-CP dated December 15, 2005 of the Government detailing certain provisions of the Customs Law on customs procedures, inspection, and supervision;
Pursuant to the directive of the Prime Minister in Document No. 175/VPCP-KTTH dated January 9, 2003 regarding the adjustment and amendment of the content of the Vietnamese entry/exit declaration form;
After reaching consensus with the Ministry of Public Security, the Ministry of National Defense, and the Ministry of Health;
Considering the proposal of the Director General of the General Department of Customs.

Article 2. This Decision shall take effect 15 days from the date of publication in the Official Gazette and shall replace Decision No. 483/2000/QD-TCHQ dated October 23, 2000 of the Director General of the General Department of Customs.
The Vietnamese Entry-Exit Declaration Form (CHY 2000) will continue to be used concurrently with the two models of entry/exit declaration forms specified in Article 1 until the existing printed and issued stock is exhausted.
Article 3. The Director General of the General Department of Customs, heads of units under the Ministry of Finance, and related organizations and individuals are responsible for implementing this Decision.
The Director General of the General Department of Customs is responsible for organizing coordination with relevant agencies to guide, direct, and inspect the implementation of the provisions in this Decision./.
