CIRCULAR
REGULATIONS ON STATISTICAL REPORTING SYSTEM IN THE HEALTH SECTOR
Pursuant to the Law on Statistics dated June 19, 2015;
Pursuant to Decree No. 94/2016/NĐ-CP dated July 1, 2016 of the Government detailing and guiding the implementation of certain provisions of the Law on Statistics;
Pursuant to Decree No. 97/2016/NĐ-CP dated July 1, 2016 of the Government stipulating the contents of statistical indicators within the national statistical indicator system;
Pursuant to Decree No. 60/2018/NĐ-CP dated April 20, 2018 of the Government detailing the contents of the national statistical reporting system;
Pursuant to Decree No. 75/2017/NĐ-CP dated June 20, 2017 of the Government stipulating the functions, tasks, powers, and organizational structure of the Ministry of Health;
Considering the proposal of the Director of the Department of Planning and Finance;
The Minister of Health issues this Circular regulating the statistical reporting system in the health sector.
Article 1. Original Record Books and Statistical Reporting Forms
1. National Technical Regulation on Railway Traffic Signals, number QCVN 21:2025/BXD.
1. The model and instructions for recording original record books are set out in Appendix I.
2. The model and instructions for recording statistical reports at commune, ward, town level (hereinafter referred to as commune) are set out in Appendix II.
3. The model and instructions for recording statistical reports at district, urban district, provincial city level (hereinafter referred to as district) are set out in Appendix III.
4. The model and instructions for recording statistical reports at provincial level (hereinafter referred to as province) are set out in Appendix IV.
5. The model and instructions for recording central-level statistical reports are set out in Appendix V.
Article 2. Reporting Periods
The reporting periods are calculated according to the Gregorian calendar:
1. Monthly reporting period: From 00:00 on the first day of the month until 24:00 on the last day of the month.
2. Quarterly reporting period:
a) From 00:00 on January 1st to 24:00 on the last day of March each year;
b) From 00:00 on April 1st to 24:00 on the last day of June each year;
c) From 00:00 on July 1st to 24:00 on the last day of September each year;
d) From 00:00 on October 1st to 24:00 on the last day of December each year;
3. Semi-annual reporting period:
a) From 00:00 on January 1st to 24:00 on the last day of June each year;
b) From 00:00 on July 1st to 24:00 on the last day of December each year.
4. Nine-month reporting period: From 00:00 on January 1st to 24:00 on the last day of September each year.
5. Annual reporting period: From 00:00 on January 1st to 24:00 on December 31st of the reporting period.
6. Ad hoc statistical reporting: In cases where ad hoc statistical reporting is required to fulfill management requirements of the Ministry of Health, the requesting agency must submit a written request specifying the time, deadline, and specific criteria for the statistical report.
Article 3. Statistical Reporting System in the Health Sector
1. Commune Level:
a) Reporting Unit: Commune Health Station;
b) Content of Report: Statistical report according to the model prescribed in Appendix II;
c) Receiving Unit: The central unit at the district level assigned by the Provincial Health Department;
d) Deadline for Reporting: Five working days from the end of the statistical period specified in Article 2 of this Circular.
2. District Level:
a) Reporting Unit: The central unit at the district level assigned by the Provincial Health Department;
b) Content of Report: Statistical report according to the model prescribed in Appendix III;
c) Receiving Unit: Provincial Health Department;
d) Deadline for Reporting: Fifteen days from the end of the reporting period specified in Article 2 of this Circular.
3. Provincial Level:
a) Reporting Unit: Provincial Health Department;
b) Content of Report: Statistical report according to the model prescribed in Appendix IV;
c) Receiving Unit: Ministry of Health (Department of Planning and Finance).
d) Deadline for Reporting: Twenty-five days from the end of the reporting period specified in Article 2 of this Circular.
4. Central Level:
a) Reporting Unit: Central-level units;
b) Content of Report: Statistical report according to the model prescribed in Appendix V;
c) Receiving Unit: Ministry of Health (Department of Planning and Finance).
d) Deadline for Reporting: Before March 31 each year.
Article 4. Responsibilities for Implementation
1. Reporting Units:
a) Shall comply with full, accurate, and timely reporting as prescribed in this Circular and bear legal responsibility for the reported content;
b) Shall review, resubmit reports and related information upon request of the receiving unit.
2. Receiving Units:
a) Shall aggregate statistical information according to the prescribed reporting forms;
b) Shall request reporting units to review, resubmit reports and related information when necessary to verify the accuracy of the reported data;
c) Shall ensure the publication, use, and confidentiality of statistical information in accordance with the law.
3. Provincial Health Departments have the responsibility to assign a unit at the district level to be the central unit to aggregate data from districts and report according to the provisions of Article 3 of this Circular.
4. Department of Planning and Finance:
a) Shall be responsible for receiving, aggregating, and analyzing health sector statistical data;
b) Guide, inspect, and audit the implementation of laws on statistics, the statistical reporting system, and other related matters;
c) Coordinate with audits of the implementation of the statistical reporting system and other related matters in accordance with the specialized inspection regulations on statistics.
5. The Information Technology Agency has the responsibility to lead and coordinate with the Department of Planning and Finance and other relevant units under the Ministry of Health to implement information technology applications in statistical work, including activities:
a) Completing and implementing the Electronic Statistical Reporting Software for the Health Sector;
b) Integrating and utilizing statistical data from specialized application software and other administrative databases within its jurisdiction to serve the statistical activities of the Ministry of Health and state statistical activities as prescribed by law.
Article 5. Effective Date
1. This Circular takes effect from April 1, 2020.
2. Circular No. 27/2014/TT-BYT dated August 14, 2014 of the Ministry of Health stipulating the Statistical Form System applicable to provincial, district, and commune-level healthcare facilities, Decision No. 3266/QĐ-BYT dated September 3, 2008 on issuing central-level reporting forms, and Decision No. 2360/1997/QĐ-BYT dated November 14, 1997 on issuing hospital statistical forms according to the health sector's statistical indicators become invalid from the date this Circular takes effect.
Article 6. Transitional Provisions
Start applying the statistical indicator system prescribed in Circular No. 20/2019/TT-BYT dated July 31, 2019 from the date this Circular takes effect.
During the implementation process, if difficulties arise, agencies, organizations, and individuals are requested to reflect them to the Ministry of Health (Department of Planning and Finance) for consideration and resolution./.
