REGARDING THE ISSUANCE OF MEDICAL RECORD FORMS AND TREATMENT CASES
THE MINISTER OF HEALTH
BASED ON THE POPULATION HEALTH PROTECTION LAW,
Pursuant to Decree No. 68/CP dated October 11, 1993 of the Government stipulating the functions, tasks, authorities, and organizational structure of the Ministry of Health,
AT THE REQUEST OF THE DIRECTOR OF THE TREATMENT DEPARTMENT OF THE MINISTRY OF HEALTH,

Article 1. NOW WE ISSUE THE MEDICAL RECORD FORMS AND TREATMENT CASES TO BE USED IN HOSPITALS (ATTACHED ARE THE FORMS).
Article 2. THE FORMS ISSUED WITH THIS DECISION SHALL BE APPLIED IN STATE, SEMI-PUBLIC, PRIVATE, AND FOREIGN-INVESTED MEDICAL ESTABLISHMENTS (COLLECTIVELY REFERRED TO AS HOSPITALS). SPECIALIZED LEADING HOSPITALS MAY SUPPLEMENT NECESSARY CONTENTS INTO THE FORMS AFTER OBTAINING THE AGREEMENT OF THE MINISTER OF HEALTH DUE TO SPECIFIC REQUIREMENTS.
Article 3. THE DIRECTOR OF THE TREATMENT DEPARTMENT OF THE MINISTRY OF HEALTH IS RESPONSIBLE FOR GUIDING AND MONITORING THE IMPLEMENTATION OF THE MEDICAL RECORD FORMS IN HOSPITALS ACROSS THE COUNTRY.
Article 4. THIS DECISION SHALL TAKE EFFECT 15 DAYS FROM THE DATE OF SIGNATURE. THE DECISION NUMBER 1333/1999/BYT-QD OF THE MINISTER OF HEALTH ON THE ISSUANCE OF MEDICAL RECORD FORMS AND TREATMENT CASES FOR USE IN HOSPITALS ON MAY 3, 1999 IS ABROGATED.
Article 5. MR./MS.: DIRECTOR OF THE OFFICE, DIRECTOR OF THE INSPECTION DEPARTMENT, DIRECTOR OF THE TREATMENT DEPARTMENT, DIRECTOR OF THE PLANNING DEPARTMENT, DIRECTORS OF OTHER DEPARTMENTS OF THE MINISTRY OF HEALTH, DIRECTOR OF THE VIETNAM PHARMACEUTICAL MANAGEMENT COUNCIL, DIRECTORS OF HEALTH SERVICES OF PROVINCES AND CENTRALLY-GOVERNED CITIES, DIRECTORS OF HOSPITALS AND INSTITUTES WITH BEDS UNDER THE MINISTRY OF HEALTH, HEADS OF HEALTH SECTORS, PRINCIPALS OF MEDICAL AND PHARMACY UNIVERSITIES AND COLLEGES ARE RESPONSIBLE FOR ENFORCING THIS DECISION./.
