between the Socialist Republic of Vietnam and the French Republic

Pursuant to Decree No. 38/CP dated June 4, 1993 of the Government on the functions, tasks, powers, and organization of the Ministry of Justice;
Pursuant to Decision No. 142/2000/QĐ-TTg dated December 11, 2000 of the Government Chairman promulgating the Regulation on the Implementation of the Agreement on Adoption Cooperation between the Socialist Republic of Vietnam and the French Republic;
Pursuant to Circular No. 04/2001/TT-BTP dated March 20, 2001 of the Ministry of Justice guiding the implementation of certain provisions of the Regulation on the Implementation of the Agreement on Adoption Cooperation between the Socialist Republic of Vietnam and the French Republic;
At the proposal of the Director of the Department for Notarization, Appraisal, Civil Status, Nationality, and Judicial Record Management and the Director of the Department for International Cooperation of the Ministry of Justice,
DECISION:
Article 1. Amend and supplement the Model Notification Document issued by the Department of Justice to the person seeking to adopt a child regarding the introduction of a child for adoption (for cases where the child is not named specifically - Appendix I.6) issued together with Circular No. 04/2001/TT-BTP dated March 20, 2001 of the Ministry of Justice guiding the implementation of certain provisions of the Regulation on the Implementation of the Agreement on Adoption Cooperation between the Socialist Republic of Vietnam and the French Republic as follows:
- The section introducing the child shall be supplemented with the following information: eligibility for adoption (meeting legal requirements and having complete documentation), personal circumstances, social and family environment, health status (accompanied by a copy of the health certificate or medical test results).
- The photograph of the child shall be stamped with the seal of the Department of Justice.
The new Model Notification Document shall be issued together with this Decision.
Article 2. This Decision shall take effect fifteen days from the date of signature.
Article 3. The Heads of the Office, Directors of the Departments for Notarization, Appraisal, Civil Status, Nationality, and Judicial Record Management, Directors of the Departments for International Cooperation, and Directors of the Departments of Justice of the provinces and centrally governed cities are responsible for implementing this Decision.
