DIRECTIVE
On strengthening work to prevent and control avian influenza A (H5N1)

From the beginning of 2003 to now, avian influenza has occurred in 12 countries worldwide, mainly in Southeast Asian countries. According to the World Health Organization's warning, there is a risk that a pandemic of avian influenza A (H5N1) could cause up to 2-7 million deaths and infect billions of people.
In Vietnam, according to reports from localities, from December 16, 2004 to April 4, 2005, the disease occurred in 16 provinces/cities nationwide, with 33 cases recorded, including 16 fatalities. Avian influenza occurred sporadically due to incomplete handling, making it easy for the avian influenza A (H5N1) virus to spread into the environment and infect humans. To urgently implement preventive measures against the disease and prevent its spread, the Minister of Health issues the following directive:
1. Directors of Health Departments of provinces/cities directly under the Central Government
a) Advise People's Committees of provinces/cities to strengthen activities of the Steering Committee for Preventing and Controlling Avian Influenza in Humans, effectively coordinate inter-departmental activities in preventing and controlling the disease at all levels in localities. Implement environmental sanitation campaigns and livestock farm sanitation throughout the province/city to prevent the spread of the avian influenza A (H5N1) virus into the environment and infect humans in April 2005.
b) Closely cooperate with Departments of Agriculture and Rural Development in inspection and supervision of avian influenza outbreaks. Strictly supervise down to individual households, minimize the risk of undetected avian influenza outbreaks. Widespread use of Chloramine B for disinfection, decontamination, and thorough treatment of outbreaks.
c) For localities with international border gates, strictly inspect incoming passengers from areas with outbreaks, previously affected areas, to prevent the disease from entering. Quarantine agencies closely coordinate with specialized agencies in inspecting and supervising imported poultry, food products, and cross-border transportation.
d) Direct Preventive Health Centers to closely monitor, early detect patients, promptly isolate them. Implement thorough outbreak control measures in humans. Guide protective measures for healthcare workers and those directly involved in outbreak control at the local level and facilities.
đ) Develop specific plans to direct hospitals and facilities at all levels to timely admit patients, treat them according to the "Guidelines for Diagnosis, Management, and Prevention of Viral Pneumonia" issued by Decision No. 3422/2004/QĐ-BYT dated September 30, 2004 of the Minister of Health.
e) Continue extensive publicity on causes of illness, prevention methods, help the public not to panic and properly report when avian influenza occurs in poultry and humans. Particularly promote "4 Measures to Prevent Avian Influenza Transmission to Humans" as stipulated by the Ministry of Health.
g) Ensure adequate funding, sufficient medicines, chemicals, and specialized materials to serve prevention and control efforts, as well as personal protective equipment for staff directly involved in outbreak investigation and control, patient treatment.
h) Regularly report to Provincial/City People's Committees and the Ministry of Health on the epidemic situation and prevention and control activities implemented locally, strictly adhere to the "Regulations on Information and Reporting of Infectious Diseases Causing Epidemics" issued together with Decision No. 4880/2002/QĐ-BYT dated December 6, 2002 of the Minister of Health.
2. Department of Preventive Medicine and HIV/AIDS Control
a) Closely monitor the development of the epidemic to advise the Minister of Health on directing prevention and control efforts against acute respiratory infections caused by viruses nationwide.
b) Directly direct, urge, and inspect localities and units under the preventive health system to implement preventive measures.
3. Treatment Department
a) Advise the Minister of Health on organizing, directing, and inspecting treatment activities for avian influenza A (H5N1) nationwide.
b) Develop specific plans for treatment tiering, strengthen treatment facilities if the epidemic spreads widely and becomes complex. Ensure good reception, emergency care, treatment, and isolation of patients.
- The Clinical Tropical Medicine Institute, Central Children's Hospital is responsible for receiving patients from northern provinces from Ha Tinh northward. North Thang Long Hospital, Duc Giang Hospital, and Dong Da Hospital in Hanoi will be reinforced if the epidemic intensifies.
- Hue Central Hospital is responsible for receiving patients from central and western highland regions (from Quang Binh to Khanh Hoa). Provincial general hospitals: Da Nang, Binh Dinh, Khanh Hoa, Dak Lak will be mobilized to receive and treat patients in their regions if the epidemic intensifies.
- Ho Chi Minh City Tropical Disease Hospital, Cho Ray Hospital, Ho Chi Minh City Children's Hospital 1, Ho Chi Minh City Children's Hospital 2 are responsible for receiving patients from southern provinces from Ninh Thuan southward.
c) Direct affiliated hospitals and provincial/city hospitals to prepare sufficient medicines, resuscitation equipment, and necessary tools for treatment.
d) Continue researching, revising, and supplementing guidelines for diagnosis, management, and prevention of viral pneumonia transmission and train treatment system staff accordingly.
4. Legal Affairs Department, Health Education and Communication Center
Advise the Minister of Health on directing publicity activities on preventing and controlling viral pneumonia. Coordinate with the Department of Preventive Medicine and HIV/AIDS Control and other propaganda agencies to effectively implement publicity, information dissemination, and epidemic situation announcements.
5. Planning and Finance Department
Aggregate, mobilize, coordinate, and respond to resources for epidemic prevention and control from state budget, domestic and foreign organizations.
6. Institutes: National Hygiene and Epidemiology Institute, Ho Chi Minh City Pasteur Institute, Nha Trang Pasteur Institute, Western Highlands Hygiene and Epidemiology Institute.
a) Direct localities to strengthen surveillance activities for early detection, diagnosis, determination of causative factors, sources of infection, and proactively implement measures to control risk factors for outbreaks. Organize training courses on disease prevention and control for localities under their jurisdiction.
- The Central Institute of Hygiene and Epidemiology shall direct 29 northern provinces.
- The Nha Trang Pasteur Institute shall direct 11 central provinces.
- The Ho Chi Minh City Pasteur Institute shall direct 20 southern provinces.
- The West Highland Institute of Hygiene and Epidemiology shall direct 4 western highland provinces.
b) Prepare sufficient disinfectants, equipment, and specialized materials, and promptly carry out disease control activities.
c) Establish disease control teams ready to support localities experiencing complex outbreak situations.
7. National Hospitals under the Ministry of Health
a) Prepare adequate stocks of medicines, medical equipment, and other necessary means to promptly support localities and effectively manage patient reception, emergency care, and treatment according to the Ministry of Health's directives.
b) Dispatch specialists and treatment teams to be ready to support localities with outbreaks upon request.
8. Other Units under the Ministry of Health
Cooperate with related units to organize disease prevention activities when required within their assigned functions and responsibilities.
This Directive replaces Directive No. 08/2004/CT-BYT dated October 21, 2004, issued by the Minister of Health regarding the strengthening of work on the prevention and control of acute respiratory infections caused by viruses.
Upon receipt of this Directive, heads of units under the Ministry, Directors of Health Departments of provinces and centrally governed cities shall immediately organize its implementation and report the results to the Ministry of Health./.
