Regarding the issuance of forms for managing cadres' and civil servants' files

THE MINISTER OF THE MINISTRY OF HOME AFFAIRS
Pursuant to the Law on Organization of the Government dated December 25, 2001;
Pursuant to the Cadres and Civil Servants Ordinance dated February 26, 1998; the Ordinance amending and supplementing certain articles of the Cadres and Civil Servants Ordinance dated April 28, 2000; and the Ordinance amending and supplementing certain articles of the Cadres and Civil Servants Ordinance dated April 29, 2003;
Pursuant to the Government Decree No. 114/2003/NĐ-CP dated October 10, 2003 on cadres and civil servants at commune, ward, and town levels; the Government Decree No. 115/2003/NĐ-CP dated October 10, 2003 on the reserve civil servant system; the Government Decree No. 116/2003/NĐ-CP dated October 10, 2003 on recruitment, utilization, and management of cadres and civil servants in state-owned units; and the Government Decree No. 117/2003/NĐ-CP dated October 10, 2003 on recruitment, utilization, and management of cadres and civil servants in state agencies;
Pursuant to the Government Decree No. 48/2008/NĐ-CP dated April 17, 2008 stipulating the functions, tasks, powers, and organizational structure of the Ministry of Home Affairs;
Pursuant to Article 7 of the Regulation on Managing Cadres' and Civil Servants' Files issued together with Decision No. 14/2006/QĐ-BNV dated November 6, 2006 of the Minister of Home Affairs;
Considering the proposal of the Director of the Cadre and Civil Servant Department,
DECISION:
Article 1. Issued together with this Decision are the forms to serve the management of cadres' and civil servants' files, including:
1. The Cadres and Civil Servants File Registration Book according to Model S01-BNV/2008, which has the following specifications:
a) The cover page is made of high-quality paper with size A4 (210 x 297 mm);
b) The inner pages of the book are made of white paper with size A4 (210 x 297 mm), containing nine columns of information criteria as follows: Serial number, file number, full name at birth, other names, date of birth, place of origin, position, working unit, remarks.
2. The Cadres and Civil Servants File Handover Book according to Model S02-BNV/2008, which has the following specifications:
a) The cover page is made of high-quality paper with size A4 (210 x 297 mm);
b) The inner pages of the book are made of white paper with size A4 (210 x 297 mm), containing ten columns of information criteria as follows: Serial number, handover date, handover person's full name, working unit, handover content, recipient's full name, recipient's working unit, recipient's signature, handover person's signature, remarks.
3. The Cadres and Civil Servants File Utilization Monitoring Book according to Model S03-BNV/2008, which has the following specifications:
a) The cover page is made of high-quality paper with size A4 (210 x 297 mm);
b) The inner pages of the book are made of white paper with size A4 (210 x 297 mm), containing eight columns of information criteria as follows: Serial number, research date, researcher's full name, working unit, research content, research method, return date, remarks.
4. Adding the criterion "salary progression process of cadres and civil servants" to the Cadres and Civil Servants Curriculum Vitae according to Model 2c-BNV/2008 and uniformly using this model to replace the previous Cadres and Civil Servants Curriculum Vitae Model 02a-BNV/2007 issued together with Decision No. 06/2007/QĐ-BNV dated June 18, 2007 of the Minister of Home Affairs on the components of cadres' and civil servants' files and forms for managing cadres' and civil servants' files.
Article 2. This Decision shall take effect fifteen days from the date of publication in the Official Gazette.
Article 3. The Minister, Heads of ministerial-level agencies, Heads of government-affiliated agencies, Chairpersons of provincial and centrally-administered city People's Committees, state-owned enterprises, and public service units are responsible for implementing this Decision./.
