ON THE ISSUANCE OF MODEL FORMS AND REGISTRATION BOOKS FOR HOUSEHOLD REGISTRATION

THE MINISTER OF JUSTICE

Pursuant to the Government Decree No. 38/CP dated June 4, 1993 on the functions, tasks, powers, and organization of the Ministry of Justice;
Pursuant to Decree No. 83/1998/ND-CP dated October 10, 1998 of the Government on Household Registration;
After reaching consensus with the Ministry of Foreign Affairs;
Based on the proposal of the Director of the Office and the Director of the Department of Notaries, Authentication, Household Registration, Citizenship, and Personal Records of the Ministry of Justice,

DECISION:

Article 1. This Decision promulgates 48 model forms and registration books for household management and registration (hereinafter referred to as Model Forms and Registration Books for Household Registration) listed and numbered in the attached Catalogue.

Article 2. The Model Forms and Registration Books for Household Registration prescribed in Article 1 of this Decision shall be uniformly used throughout the country and at diplomatic agencies and consular offices of the Socialist Republic of Vietnam abroad (hereinafter referred to as Vietnamese Diplomatic and Consular Agencies) from January 1, 1999, replacing all previous model forms and registration books for household management and registration issued before that date.

Article 3.

1. The Ministry of Justice shall organize the printing and direct distribution of the following 19 Model Forms and 5 Registration Books for Household Registration:

- Birth registration forms: TP/HT-1999-A.1, TP/HT-1999-A.2, TP/HT-1999-A.2.A, TP/HT-1999-A.3, TP/HT-1999-A.4;

- Marriage registration forms: TP/HT-1999-B.1, TP/HT-1999-B.2, TP/HT-1999-C.2.a, TP/HT-1999-C.3, TP/HT-1999-C.4, TP/HT-1999-C.5;

- Death registration forms: TP/HT-1999-C.1, TP/HT-1999-C.2, TP/HT-1999-C.2.a, TP/HT-1999-C.3, TP/HT-1999-C.4, TP/HT-1999-C.5;

- Forms for changing and correcting household registration: TP/HT-1999-G.1, TP/HT-G.2, TP/HT-1999-B, TP/HT-1999-C.2a;

- Types of books: TP/HT-1999-A, TP/HT-1999-B, TP/HT-1999-C, TP/HT-1999-D, TP/HT-1999-G;

- Statistical report form for household registration data: TP/HT-1999-L.

2. Provincial Departments of Justice under the central government and Vietnamese Diplomatic and Consular Agencies shall organize the printing and distribution of the remaining Model Forms and Registration Books for Household Registration according to the specifications and contents stipulated in the Model Forms and Registration Books for Household Registration issued with this Decision.

Article 4.

1. The Director of the Office shall be responsible for coordinating with the Director of the Department of Notaries, Authentication, Household Registration, Citizenship, and Personal Records to implement the printing and distribution of the Model Forms and Registration Books for Household Registration prescribed in Clause 1 of Article 3 of this Decision, fully meeting the requirements for household registration and management in provinces and centrally-administered cities.

2. The Director of the Department of Notaries, Authentication, Household Registration, Citizenship, and Personal Records shall be responsible for coordinating with the Director General of the Consular Department of the Ministry of Foreign Affairs to adjust the Model Forms and Registration Books for Household Registration (the section recording the name of the registration authority) to suit their use in Vietnamese Diplomatic and Consular Agencies and to implement the printing and distribution of the Model Forms and Registration Books for Household Registration prescribed in Clause 2 of Article 3 of this Decision, fully meeting the requirements for household registration and management abroad.

Article 5. The issuance and sale of Model Forms and Registration Books for Household Registration must comply with the prescribed price.

Article 6. Strictly prohibited is the printing, issuance, and use of Model Forms and Registration Books for Household Registration contrary to the provisions of this Decision.

Article 7. This Decision shall take effect from the date of signing.

Article 8. The Directors of the Office, the Department of Notaries, Authentication, Household Registration, Citizenship, and Personal Records, the Heads of Provincial Departments of Justice under the central government, and Vietnamese Diplomatic and Consular Agencies are responsible for implementing this Decision.
