CIRCULAR
Provisions on Good Storage Practices for Medicines and Raw Materials for Medicine Production
Circular No. 36/2018/TT-BYT dated November 22, 2018 on Good Storage Practices for Medicines and Raw Materials for Medicine Production took effect from January 10, 2019 and was amended and supplemented by:
Circular No. 29/2020/TT-BYT dated December 31, 2020 issued by the Minister of Health to amend and supplement and abolish some legal normative documents issued by the Minister of Health, jointly issued, taking effect from February 15, 2021;

Pursuant to Law No. 105/2016/QH13 dated April 6, 2016 on pharmaceuticals;
Pursuant to the Decree No. 54/2017/NĐ-CP dated May 8, 2017 of the Government detailing some provisions and measures for implementing the Medicine Law;
Pursuant to the Decree No. 155/2018/NĐ-CP dated November 12, 2018 of the Government amending and supplementing certain regulations related to investment and business conditions within the scope of management of the Ministry of Health;
Pursuant to the Government's Decree No. 75/2017/NĐ-CP dated June 20, 2017 stipulating the functions, tasks, powers, and organizational structure of the Ministry of Health;
At the proposal of the Director of the Drug Administration Department,
The Minister of Health issues this Circular on Good Storage Practices for Medicines and Raw Materials for Medicine Production.[1]
Chapter I. GENERAL PROVISIONS
Article 1. Scope of Regulation
This Circular stipulates the announcement of application, issuance, and evaluation of compliance with Good Storage Practices for Medicines and Raw Materials for Medicine Production.
Article 2. Interpretation of Terms
In this Circular, the following terms are understood as follows:
1. Storage of medicines and raw materials for medicine production means safekeeping ensuring safety and quality of medicines and raw materials for medicine production, including the process of putting into use and maintaining a complete system of records and documents serving storage, export, and import of medicines and raw materials for medicine production at the storage location.
2. Good Storage Practices for Medicines and Raw Materials for Medicine Production is a set of principles and standards for storing medicines and raw materials for medicine production aimed at ensuring and maintaining the highest level of safety and quality of medicines and raw materials for medicine production through comprehensive control throughout the storage process.
3. Non-compliance is deviation from the principles and standards of Good Storage Practices for Medicines and Raw Materials for Medicine Production or from the provisions of pharmaceutical laws.
4. Storage facility is a facility engaged in storing medicines and raw materials for medicine production, including facilities importing medicines and raw materials for medicine production; facilities exporting medicines and raw materials for medicine production; facilities providing storage services for medicines and raw materials for medicine production; facilities engaging in pharmaceutical activities not requiring a Certificate of Compliance for Pharmaceutical Business Operations within the scope of storage (national health program drug storage headquarter facilities, people's armed forces drug storage facilities; national expanded immunization program drug storage facilities, medical examination and treatment facilities, and other facilities storing medicines and raw materials for medicine production for non-commercial purposes).
5. GSP stands for the English term "Good Storage Practices," which is translated into Vietnamese as Good Storage Practices for Medicines and Raw Materials for Medicine Production.
6. WHO stands for the English term "World Health Organization," which is translated into Vietnamese as World Health Organization.
Chapter II. ISSUANCE OF GOOD STORAGE PRACTICES FOR MEDICINES AND RAW MATERIALS FOR MEDICINE PRODUCTION
Article 3. Documentation on Principles and Standards of Good Storage Practices for Medicines and Raw Materials for Medicine Production
1. Issuing principles and standards of Good Storage Practices for Medicines and Raw Materials for Medicine Production as specified in Appendix I attached to this Circular based on guidance documents of the World Health Organization and updated documents as provided for in Clause 5 of this Article.
2. Issuing principles and standards of Good Storage Practices for Traditional Herbal Medicines as specified in Seal Registration Certificate promulgated together with this Circular.
3. Issuing principles and standards of Good Storage Practices for Medicines and Raw Materials for Medicine Production for facilities engaging in pharmaceutical activities not requiring a Certificate of Compliance for Pharmaceutical Business Operations within the scope of storage, except for facilities specified in Clause 4 of this Article, as specified in ANNEX III promulgated together with this Circular.
4. Issuing principles and standards of Good Storage Practices for Medicines and Raw Materials for Medicine Production for medical examination and treatment facilities as specified in REGULATIONS ON THE CERTIFICATE DESIGN FOR OUTSTANDING RURAL INDUSTRIAL PRODUCTS promulgated together with this Circular.
5. In case the World Health Organization amends and supplements the principles and standards of Good Storage Practices for Medicines and Raw Materials for Medicine Production as specified in Appendix I attached to this Circular, within six months from the date these updated documents are published on the WHO’s website, the Drug Administration will organize translation and publication of the amended and supplemented contents on the Ministry of Health’s website and the Drug Administration’s website, the Traditional Medicine Administration’s website for relevant parties to search, update, and apply.
Article 4. Apply the principles and standards of Good Storage Practices for drugs and drug ingredients.
1. Exporters, importers, and service providers for drug storage and drug ingredient storage (excluding exporters, importers, and service providers for traditional herbal medicine and traditional medicine ingredient storage) shall implement the GSP as prescribed in Appendix I issued together with this Circular and updated documents according to the provisions at Clause 5, Article 3 of this Circular..
2. Exporters, importers, and service providers for traditional herbal medicine and traditional medicine ingredient storage shall implement the GSP as prescribed in Seal Registration Certificate promulgated together with this Circular.
3. Central, regional, and provincial level national health program drug storage facilities, drug storage facilities for the armed forces, and vaccine storage facilities under the national expanded immunization program shall implement the GSP as prescribed in ANNEX III promulgated together with this Circular.
4. Drug storage warehouses of medical examination and treatment establishments (excluding warehouses for traditional herbal medicine and traditional medicine ingredient storage), vaccination centers, and agencies responsible for managing vaccinations at the commune level[2] shall implement the GSP as prescribed in REGULATIONS ON THE CERTIFICATE DESIGN FOR OUTSTANDING RURAL INDUSTRIAL PRODUCTS promulgated together with this Circular.
5. Traditional herbal medicine and traditional medicine ingredient storage warehouses of medical examination and treatment establishments shall implement the GSP as prescribed in Seal Registration Certificate issued together with this Circular (excluding the content prescribed in Point 3.2 of the Appendix to this Circular).
6. Entities that have the right to import but are not allowed to distribute drugs and drug ingredients in Vietnam shall implement and meet the GSP as prescribed in Appendix I issued together with this Circular (excluding the transportation for distribution of drugs and drug ingredients as prescribed in Clause 8) Appendix Iand updated documents according to the provisions at Clause 5, Article 3 of this Circular.
7. Drug and drug ingredient storage entities shall implement updated GSP documents as prescribed in Clause 1 of this Article within the following timeframes:
a) 12 months for cases requiring changes to the warehouse or equipment for drug and drug ingredient storage, from the date the updated documents are published on the Ministry of Health's website and the Drug Administration's website, and the Traditional Medicine and Herbal Medicine Administration's website;
b) 6 months for updates not covered by point a of this Clause, from the date the updated documents are published on the Ministry of Health's website and the Drug Administration's website, and the Traditional Medicine and Herbal Medicine Administration's website.
Chapter III. EVALUATION OF COMPLIANCE WITH GOOD STORAGE PRACTICES FOR DRUGS AND DRUG INGREDIENTS
Article 5. Documents serving as the basis for evaluating compliance with Good Storage Practices for drugs and drug ingredients.
1. The application documents for GSP evaluation for storage entities are the application documents for the issuance of the Certificate of Compliance for Drug Business Conditions (submitted when applying for the Certificate of Compliance for Drug Business Conditions, storage entities do not need to submit additional documents for GSP evaluation) as stipulated in Article 38 of the Drug Law and Article 32 of Decree No. 54/2017/ND-CP dated May 8, 2017 of the Government detailing certain provisions and measures to implement the Drug Law (hereinafter referred to as Decree No. 54/2017/ND-CP). For special control drug storage entities, they must comply with the provisions of Article 38 of the Drug Law and Clause 31, Article 5 of Decree No. 155/2018/ND-CP dated November 12, 2018 of the Government amending and supplementing certain provisions related to business conditions under the Ministry of Health's management (hereinafter referred to as Decree No. 155/2018/ND-CP).
Technical documents about storage facilities shall be presented according to the overall storage facility application document guidelines prescribed in Appendix VI issued together with this Circular or the updated overall application document in case of expansion of activities.
2. [3] In cases where entities apply for both the GSP Certificate and the Certificate of Compliance for Drug Business Conditions, the entity must clearly state this in the application for the Certificate of Compliance for Drug Business Conditions. If an entity has already been issued the Certificate of Compliance for Drug Business Conditions and needs to apply for the GSP Certificate, the entity must submit a written request for the GSP Certificate. The receiving authority will consider issuing the GSP Certificate based on the most recent GSP evaluation results.
3. [4] In cases where entities engage in drug activities without commercial purposes and need to apply for the GSP Certificate, the entity must submit a written request for the GSP Certificate along with technical documents according to the overall storage facility application document guidelines prescribed in Appendix VI issued together with this Circular. The receiving authority will consider the evaluation and issuance of the GSP Certificate.
Article 6. Procedure for evaluating compliance with Good Storage Practices for Medicines and Raw Materials for Medicines
1. Receiving documents:
The storage facility shall submit one (1) set of documents as prescribed in Article 5 of this Circular along with the assessment fee as stipulated by the Minister of Finance regarding the assessment fee for standards and conditions for storing medicines and raw materials for medicines to the receiving agency of the Ministry of Health as follows:
a) The Traditional Medicine Management Department for facilities exporting, importing, and providing storage services requesting a Certificate of Compliance for operating pharmaceuticals with a scope limited to trading traditional herbal medicines, traditional Chinese medicines, and traditional medicinal herbs at the time of submitting the application;
b) The Drug Management Department for facilities exporting, importing, and providing storage services requesting a Certificate of Compliance for operating pharmaceuticals with a scope including chemical drugs, vaccines, biological products, herbal medicines, and raw materials for medicines (excluding herbal medicines);
c) The Drug Management Department for facilities exporting, importing, and providing storage services requesting a Certificate of Compliance for operating pharmaceuticals with a scope including simultaneously one of the types of medicines or raw materials for medicines specified in point a of this clause and one of the types of medicines or raw materials for medicines specified in point b of this clause at the time of submitting the application.
2. Procedure for receiving and assessing the documents:
To be implemented according to the provisions of Clause 2 and Clause 5 of Article 33 of Decree No. 54/2017/NĐ-CP and Clause 12 of Article 5 of Decree No. 155/2018/NĐ-CP.
3. Within five days from the date of receipt of valid documents, the receiving agency shall establish an Evaluation Team and send the decision to establish the Evaluation Team to the storage facility, including the estimated time for on-site evaluation at the storage facility.
Within fifteen days from the date of the decision to establish the team, the Evaluation Team shall conduct on-site evaluations at the storage facility.
Article 7. Process for Evaluating Compliance and Classifying Compliance with Good Storage Practices for Medicines and Raw Materials for Medicines
1. Principles for Using GSP Documents in Evaluating Compliance with GSP:
The principles and standards of GSP are stipulated in Clauses 1 and 2 of Article 4 of this Circular corresponding to the activities of storing medicines and raw materials for medicines of the facility requesting a Certificate of Compliance for operating pharmaceuticals.
2. Evaluation Process:
a) Step 1. The Evaluation Team announces the Decision to Establish the Evaluation Team, the purpose, content, and plan of the evaluation at the storage facility;
b) Step 2. The storage facility presents a summary of its organizational structure, personnel, and implementation and application of GSP or specific content according to the content of the evaluation period;
c) Step 3. The Evaluation Team conducts an on-site evaluation of the implementation and application of GSP at the storage facility according to each specific content;
d) Step 4. The Evaluation Team meets with the storage facility to inform about any issues discovered during the evaluation (if any), assess the severity of each issue, and discuss with the storage facility if there is disagreement between the storage facility and the Evaluation Team's assessment of each issue or regarding the degree of compliance with GSP principles and standards of the storage facility;
e) Step 5. Drafting and signing the evaluation report:
Immediately after completing the on-site evaluation at the storage facility, the Evaluation Team drafts the evaluation report according to Form No. 03 prescribed in Appendix VII issued together with this Circular. The evaluation report must include the members of the Evaluation Team, the members of the storage facility, location, time, scope of the evaluation, and unresolved issues between the Evaluation Team and the storage facility (if any). The head of the storage facility and the head of the Evaluation Team sign to confirm the evaluation report. The evaluation report is made in three copies: one copy retained by the storage facility, two copies retained by the receiving agency.
f) Step 6. Completing the Evaluation Report:
The Evaluation Team is responsible for drafting the GSP evaluation report according to Form No. 04 prescribed in Appendix VII issued together with this Circular, listing and analyzing, classifying the severity of issues that the storage facility needs to address, repairing, and referring to relevant legal provisions and GSP principles and standards, evaluating the degree of compliance with GSP of the storage facility. The classification of the severity of issues and the evaluation of the degree of compliance with GSP of the storage facility are stipulated in Appendix V promulgated together with this Circular.
3. Degree of Compliance with GSP:
The degree of compliance with GSP of the storage facility is stipulated in Appendix V issued together with this Circular, including the following degrees:
a) The storage facility complies with GSP at Level 1;
b) The storage facility complies with GSP at Level 2;
c) The storage facility complies with GSP at Level 3.
Article 8. Handling of Results of Good Storage Practice for Drugs and Drug Raw Materials Assessment
1. In cases where the GSP assessment report concludes that the storage facility complies with GSP at level 1 as prescribed in point a, Clause 3, Article 7 of this Circular:
Within ten days from the date of signing the assessment record, the Receiving Authority shall submit to the Minister of Health for issuance of the Certificate of Eligibility for Pharmaceutical Business Operations and issue the GSP Certificate according to Form No. 05 stipulated in Appendix VII issued together with this Circular if the storage facility has requested it in the Application for Issuance of the Certificate of Eligibility for Pharmaceutical Business Operations.
2. In cases where the GSP assessment report concludes that the storage facility complies with GSP at level 2 as prescribed in point b, Clause 3, Article 7 of this Circular:
a) Within five days from the date of signing the assessment record, the Receiving Authority shall send the GSP assessment report to the storage facility in accordance with point b, Clause 6, Article 33 of Decree No. 54/2017/NĐ-CP.
b) After completing the rectification and repair work, the storage facility must submit a rectification report including a plan and evidence (document files, images, videos, certificates, or other proof documents) demonstrating the rectification and repair of the deficiencies recorded in the GSP assessment report;
c) Within twenty days from the date of receiving the rectification report, the Receiving Authority shall evaluate the results of the rectification by the storage facility and conclude on the compliance status of the storage facility with GSP:
- If the rectification by the storage facility meets the requirements: The Receiving Authority shall submit to the Minister of Health for issuance of the Certificate of Eligibility for Pharmaceutical Business Operations and issue the GSP Certificate according to Form No. 05 stipulated in Appendix VII this Circular if the storage facility has requested it in the Application for Issuance of the Certificate of Eligibility for Pharmaceutical Business Operations;
- If the rectification by the storage facility does not meet the requirements, the Receiving Authority shall issue a notification letter to the storage facility specifying the reasons.
d) Within six months from the date the Receiving Authority sends the assessment report containing the requirement for modification and supplementation, the storage facility must submit the modified and supplemented dossier. If the storage facility does not modify and supplement within the above period or if the modified and supplemented dossier submitted twelve months after the initial application submission does not meet the requirements, the submitted application will lose its validity.
3. In cases where the GSP assessment report concludes that the storage facility complies with GSP at level 3 as prescribed in point c, Clause 3, Article 7 of this Circular:
Within five days from the date of signing the assessment record, the Receiving Authority shall issue a notification letter regarding non-compliance with GSP along with the GSP assessment report to the storage facility and shall not issue the Certificate of Eligibility for Pharmaceutical Business Operations.
4. In cases where the storage facility disagrees with the deficiencies identified in the assessment report by the Assessment Team, within thirty days from the date the Assessment Team issues the assessment report or the corrective action assessment report, the storage facility
shall submit a recommendation letter to the Receiving Authority accompanied by evidence (document files, images, videos, certificates) related to the deficiency.
Within ten days from the date of receipt of the recommendation letter from the storage facility, the Receiving Authority shall review the GSP assessment report, the content of the storage facility's recommendation, seek expert advice if necessary, and issue a response letter to the storage facility. The response letter must clearly state whether the recommendation is accepted or rejected and provide reasons for rejection. This time does not count towards the assessment period.
5. Within five days from the date of issuance of the Certificate of Eligibility for Pharmaceutical Business Operations, the Receiving Authority shall publish on the Ministry of Health's electronic portal and the Receiving Authority's website the following information:
a) Name and address of the drug and drug raw materials storage facility;
b) Name of the person responsible for pharmaceutical expertise and their Pharmaceutical Practitioner License number;
c) Number of the Certificate of Eligibility for Pharmaceutical Business Operations and the number of the GSP Certificate (if applicable);
d) Expiry date of the GSP compliance evaluation;
đ) Scope of operation of the storage facility.
Chapter IV. ASSESSMENT OF THE MAINTENANCE OF GOOD STORAGE PRACTICE FOR DRUGS AND DRUG RAW MATERIALS COMPLIANCE
Article 9. Periodic Assessment of Compliance with Good Storage Practices for Medicines and Raw Materials
1.[5] The periodic assessment cycle for maintaining compliance with GSP at storage facilities is three years, starting from the date of signing the evaluation report of the previous assessment (excluding any unscheduled assessments, inspections, or checks conducted by the Ministry of Health, provincial People's Committees (specialized agencies responsible for pharmaceutical affairs at the provincial level, hereinafter referred to as specialized health agencies of the province)).
2. Each year in November, the Receiving Agency publishes on its electronic information website the plan for periodic assessments of compliance with GSP for storage facilities in the following year and sends this plan to all storage facilities listed in the plan. For storage facilities specified in point c Clause 1 Article 6 of this Circular,the Drug Administration of Vietnam will publish and implement the plan for periodic assessments, except in cases where the facility requests a separate assessment.
At least 30 days before the scheduled periodic assessment date announced by the Receiving Agency, the storage facility must submit to the Receiving Agency a report on business activities related to exporting, importing, or providing drug storage services, and the maintenance of GSP standards (hereinafter referred to as the activity report - GSP compliance report) of the facility according to Form No. 2 stipulated in Appendix VII issued together with this Circular, along with updated technical documents regarding the facility's physical conditions, technical capabilities, and personnel (if there have been changes).
Example: If the planned periodic assessment date at Storage Facility A is August 18, 2018, then Storage Facility A must submit the activity report - GSP compliance report to the Receiving Agency before July 18, 2018.
4. In case a storage facility fails to submit the activity report - GSP compliance report within the time limit set out in Clause 3 of this Article, within 15 days from the deadline for submission, the Receiving Agency shall issue a written request for the facility to submit the report as required.
Within 45 days from the date the Receiving Agency issues the request, the storage facility must submit the activity report - GSP compliance report along with an explanation for the delay in submitting the report. If the facility does not submit the report after this period, the Receiving Agency will conduct an unscheduled inspection, audit, or check on the facility's compliance with GSP as provided in Article 12. This Circular.
6. [6] After submitting the activity report - GSP compliance report within the prescribed timeframe, the storage facility may continue operations within the scope of business stated in the Pharmaceutical Business License already issued, using this license until the results of the periodic assessment of GSP compliance are available, and must ensure continued compliance with GSP throughout the operation period.
7. The procedures for assessment, the assessment process, and classification of assessment results for GSP compliance are carried out in accordance with the provisions of Article 6 and Article 7 of this Circular.
Article 10. Handling Results of Periodic Assessments of Compliance with Good Storage Practices for Medicines and Raw Materials
1.[7] In the event that the GSP assessment report concludes that the storage facility complies with GSP at Level 1 as stipulated in point a, Clause 3, Article 7 of this Circular:
Within 10 days from the completion of the actual assessment at the facility, the Receiving Agency shall issue a notification on the facility's compliance with GSP or issue a GSP Compliance Certificate according to Form No. 05 stipulated in Appendix VII issued together with this Circular for facilities requesting a GSP Compliance Certificate; the Receiving Agency updates the status of GSP compliance of the facility on its electronic information website.
2. In cases where the GSP assessment report concludes that the storage facility complies with GSP at level 2 as prescribed in point b, Clause 3, Article 7 of this Circular:
a) Within five working days from the date of signing the assessment report, the Receiving Agency sends the GSP assessment report to the storage facility to address deficiencies and submit a corrective action report to the Receiving Agency;
b) Within 45 days from the date the Receiving Agency sends the GSP assessment report, the storage facility must submit a corrective action report including a plan and evidence (documentary files, images, videos, certificates, or other proof) demonstrating the correction of deficiencies noted in the GSP assessment report;
c) [8] Within 20 days from receiving the corrective action report from the storage facility, the Receiving Agency evaluates the corrective actions taken by the facility and concludes on the GSP compliance status of the facility as follows:
- If the corrective actions taken by the facility meet the requirements: The Receiving Agency shall issue a notification on the facility's compliance with GSP or issue a GSP Compliance Certificate according to Form No. 05 stipulated in Appendix VII issued together with this Circular for facilities requesting a GSP Compliance Certificate; the Receiving Agency updates the status of GSP compliance of the facility on its electronic information website.
- If the corrective actions taken by the facility do not meet the requirements: The Receiving Agency shall issue a notification on the necessary further corrective actions and submit additional reports. The extended period for further corrective actions and reporting is 45 days from the date of the request.
d) Within 90 days from the date of signing the assessment report, if the storage facility does not submit a corrective action report or if the corrective actions taken in accordance with point c Clause 3 of this Article still fail to meet the requirements, the Receiving Agency shall issue a notification on non-compliance with GSP and, depending on the nature and severity of the violation, the Receiving Agency shall implement one or more measures as provided in points a and b Clause 3 of this Article.
3. In cases where the GSP assessment report concludes that the storage facility complies with GSP at level 3 as prescribed in point c Clause 3 Article 7 of this Circular:
Within five working days from the date of signing the assessment report, based on the risk assessment of deficiencies discovered concerning the quality of medicines and raw materials and the safety of users, the Receiving Agency shall issue a notification on non-compliance with GSP along with the GSP assessment report. Depending on the nature and severity of the violation, the Receiving Agency shall implement one or more of the following measures:
a) Administrative penalties according to laws on administrative violations.
b) Submit to the Minister of Health for a decision to revoke the Drug Business Condition Certificate that has been issued in accordance with Clause 2 of Article 40 of the Drug Law, and carry out the revocation of the GSP Certificate that has been issued (if any).
c) In cases where the storage facility does not meet one or more business scopes specified in the Drug Business Condition Certificate that has been issued, the Receiving Authority:
- Submit to the Minister of Health for a decision to revoke the Drug Business Condition Certificate that has been issued to remove the non-compliant business scope in accordance with Article 40 of the Drug Law, and simultaneously issue a new Drug Business Condition Certificate appropriate to the compliant business scope of the storage facility.
- Issue a GSP Certificate appropriate to the business scope maintained by the storage facility if requested by the facility.
4. Within five working days from the date of conclusion that the storage facility maintains compliance with GSP or from the date of issuance of the Decision to revoke the Drug Business Condition Certificate due to non-compliance with GSP, the Receiving Authority shall update the GSP compliance status on the Receiving Authority's electronic information website in accordance with the provisions set forth at Clause 5, Article 8 of this Circular for storage facilities that comply with GSP or information regarding the revocation of the Drug Business Condition Certificate and GSP Certificate (if any) issued for storage facilities that fail to maintain GSP compliance.
5. In cases where drug samples or raw materials for drugs collected during the assessment are found to violate quality standards, the Receiving Authority shall handle the non-compliant drugs and raw materials in accordance with current regulations.
Article 11. Change Control
1. After making changes, the storage facility must submit an application for a Drug Business Condition Certificate or report the changes according to Form No. 06 stipulated in Appendix VII accompanying this Circular if it falls under any of the following circumstances:
a) Changes as provided in Point b of Article 36 of the Drug Law;
b) Changing the location of the warehouse within the same business premises;
c) Adding a new warehouse at a different location within the same business premises;
d) Expanding the existing warehouse based on its current structure;
đ) Repairing or making significant changes to the warehouse structure and layout;
e) Modifying auxiliary systems or changing the design principle and operation of utility systems that affect storage requirements and conditions.
2. In cases where the storage facility makes changes as prescribed in point a of Clause 1 of this Article, the facility must submit an application for a Drug Business Condition Certificate in accordance with Clauses 2 and 4 of Article 38 of the Drug Law.
The procedures for assessing GSP compliance, classifying assessment results, and handling assessment outcomes are carried out in accordance with the provisions of Articles 6, 7, and 8 of this Circular.
3. In cases where the storage facility makes changes as prescribed in points b or c of Clause 1 of this Article, the facility must submit a change report along with corresponding technical documentation to the Receiving Authority.
a) The Receiving Authority conducts an on-site evaluation at the storage facility. If the facility meets the requirements, the Receiving Authority issues a document agreeing to the facility's changes;
b) The procedures for evaluating, classifying assessment results, and handling assessment outcomes for storage facilities that make changes as prescribed in point b of Clause 1 of this Article are carried out in accordance with the provisions of Articles Article 6, 7 and 10 of this Circular;
c) The procedure for evaluating, classifying results, and handling evaluation results for storage facilities that change according to the provisions of point c, Clause 1 of this Article shall be implemented in accordance with the provisions of the following 6, 7, and 8 of this Circular.
4. In cases where the storage facility changes under one of the circumstances stipulated in points d, đ, or e, Clause 1 of this Article, the storage facility must submit a change report along with corresponding technical documentation regarding the change to the Receiving Authority. The Receiving Authority will conduct an assessment of the storage facility's change report.
a) Within ten days from the date of receipt of the notification document, the Receiving Authority issues a notification document agreeing with the content of the change if the change meets the requirements, or notifies the content that needs to be rectified or repaired if it does not meet the requirements;
b) Within forty-five days from the date the Receiving Authority issues the notification document, the storage facility must complete the rectification or repair and issue a notification document accompanied by evidence (document files, images, videos, certificates, and other documents) proving the completion of the rectification or repair of the deficiencies mentioned in the notification document;
c) Within ten days from the date of receipt of the storage facility's rectification report accompanied by evidence (document files, images, videos, certificates, and other documents), the Receiving Authority evaluates the rectification result of the storage facility and concludes on the compliance status with GSP of the storage facility:
- If the rectification has met the requirements: The Receiving Authority issues a notification document agreeing with the content of the change;
- If the rectification has not met the requirements: The Receiving Authority conducts an urgent assessment and handles the assessment results according to the provisions at Article 12. This Circular.
Article 12. Urgent Assessment, Inspection, and Supervision of the Maintenance of Compliance with Good Storage Practices for Medicines and Raw Materials for Medicines
1. The work of inspecting and supervising the maintenance of compliance with GSP of storage facilities is carried out in accordance with the provisions of the law.
2. The Receiving Authority conducts an urgent assessment of the maintenance of compliance with GSP at storage facilities when the storage facility falls under one of the following circumstances:
a) The storage facility's rectification has not met the requirements stipulated in Subpoint 2, Point c, Clause 4, Article 11 of this Circular;
b) The storage facility complies with GSP at Level 2 or 3 as stipulated in Point b and c, Clause 3, Article 7 of this Circular must be subject to an urgent assessment at least once within three years from the end date of the previous assessment period;
c) The storage facility has inspection and supervision results from competent authorities concluding with serious violations of GSP principles and standards;
d) In cases where there is information reflecting or reporting serious violations of GSP principles and standards by the storage facility;
e) The storage facility fails to submit activity reports and compliance maintenance reports as required by Clause 5, Article 9 of this Circular.
3. The composition of the Assessment Team is decided by the Head of the Receiving Authority in accordance with the provisions at Article 15 of this Circular.
4. The procedure for conducting urgent assessments at storage facilities is carried out in accordance with the provisions at Article 7 of this Circular.
5. The handling of inspection and assessment results at storage facilities is carried out in accordance with the provisions at Article 10 of this Circular.
Chapter V. COMPLIANCE WITH GOOD STORAGE PRACTICES FOR MEDICINES AND RAW MATERIALS FOR MEDICINES BY PHARMACEUTICAL ACTIVITIES THAT ARE NOT LICENSED FOR PHARMACEUTICAL BUSINESS OPERATIONS
Article 13. Compliance with Good Storage Practices for Medicines and Raw Materials for Medicines at medical examination and treatment facilities, vaccination centers, district-level agencies responsible for managing vaccinations, and vaccine storage facilities under the national expanded immunization program at the provincial level.
1. The facility must comply with GSP as stipulated in Clauses 3, 4, and 5 of Article 4 of this Circular and submit a notification document regarding compliance with GSP according to Model No. 01 prescribed in Appendix VII issued together with this Circular to the specialized health agency of the province where the facility operates.[10] 2. Within five days from the date of receipt of the notification document regarding compliance with GSP from the facility, the specialized health agency of the province
shall publish on its official website the following information:[11] a) Name and address of the facility that has notified compliance with GSP;[12] b) Date of notification of compliance with GSP;
c) Scope of storage activities of the facility that has notified compliance with GSP.
3. The specialized health agency of the province
shall inspect, audit, and evaluate the facility's compliance with GSP as stipulated in
Article 14. Compliance with Good Storage Practices for Medicines and Raw Materials for Medicines at central-level drug storage facilities of the national health program, people's armed forces units, and vaccine storage facilities under the national expanded immunization program at the central and regional levels.[13] 1. The facility must comply with GSP as stipulated in Article 15 of this Circular.
Clause 3 and Clause 4 of Article 4 of this Circular
and submit the notification document to the receiving agency as specified in . The facility is permitted to operate storage activities from the date of submission of the notification document. Model No. 01 prescribed in Appendix VII issued together with this Circular to the specialized health agency of the province 2. Within five days from the date of receipt of the notification document regarding compliance with GSP from the facility, the receiving agency shall publish on its official website the following information: Clause 1, Article 6 of this Circular3. In cases where the facility conducts storage activities for medicines and raw materials for medicines for commercial purposes, the facility must submit a request for evaluation of compliance with GSP and technical documents as stipulated in

Articles 14 and 15 of this Circular

c) If the inspection and assessment conclude that the facility complies with GSP at Level 3 as stipulated in point b, Clause 3, Article 7 of this Circular the inspection and assessment agency shall decide to temporarily suspend storage activities within the non-compliant scope until the facility rectifies and meets the requirements;
d) If samples taken during the inspection and assessment are found to violate quality standards due to non-compliance with GSP by the facility, the receiving agency shall handle the medicines and raw materials for medicines according to current regulations on medicine quality management. The head of the storage facility shall bear legal responsibility for such violations. point c Clause 3 Article 7 of this Circular The authority conducting the inspection and evaluation shall suspend the operation of the storage activity that does not meet the requirements until the storage facility implements corrective actions to address the deficiencies satisfactorily.
d) In cases where the samples of drugs or drug ingredients taken during the inspection and evaluation by the Inspection and Evaluation Team are found to violate quality standards due to non-compliance with GSP requirements by the facility, the receiving authority shall handle the drugs and drug ingredients according to current regulations on drug quality management. The head of the storage facility shall bear legal responsibility for this violation.
Article 16. Implementation timeline for Good Storage Practices for Medicines and Active Pharmaceutical Ingredients for entities engaged in pharmaceutical activities not subject to issuance of a Business Registration Certificate for Pharmaceutical Operations
1. By no later than July 1, 2019, entities involved in vaccine storage, inventory, and supply (vaccine storage facilities under the national expanded immunization program at central, regional, provincial levels and local agencies responsible for immunization management at commune level, vaccination service providers, medical establishments conducting vaccinations) must fully implement and comply with GSP for vaccine storage as stipulated in this Circular.[14](such as vaccination service facilities, medical examination and treatment facilities with vaccination activities) must implement and comply fully with GSP for vaccine storage activities as prescribed in this Circular.
2. By no later than January 1, 2021, primary storage facilities for medicines and active pharmaceutical ingredients of the national health program, of the people's armed forces, medical establishments (excluding medical establishments conducting vaccinations as specified in Clause 1 of this Article), traditional medicine medical establishments must fully comply with GSP for medicine storage as stipulated in this Circular.
Chapter VI. EVALUATION TEAM FOR COMPLIANCE WITH GOOD STORAGE PRACTICES FOR MEDICINES AND ACTIVE PHARMACEUTICAL INGREDIENTS
Article 17. Composition and criteria for members of the Evaluation Team
1. The composition of the Evaluation Team includes:
a) The Team Leader and one member from the receiving agency. For storage facilities as specified in point c Clause 1 Article 6 of this Circular, , an additional member from the Traditional Medicine and Pharmaceutical Management Department shall be added;
b) One member representing the specialized health authority of the province where the storage facility's warehouse is located;[15] at the location where the storage facility sets up the storage warehouse;
c) A relevant agency member when necessary.
2. Members participating in the Evaluation Team must meet the following criteria:
a) Hold a bachelor’s degree or higher in pharmacy, medicine, chemistry, biology;
b) Have been trained and instructed on GSP, inspection, evaluation of GSP, and have a thorough understanding of GSP principles and standards;
c) Be honest, impartial, and strictly adhere to regulations and laws during the evaluation process, without conflicts of interest with the evaluated storage facility as stipulated in Clause 3 of this Article;
d) The Evaluation Team Leader must hold a bachelor’s degree in pharmacy or higher and have at least two years of experience in pharmaceutical management work.
3. Conflict of Interest Evaluation Principle: An Evaluation Team member is considered to have a conflict of interest with the evaluated storage facility if they fall into any of the following situations:
a) Have worked for the evaluated storage facility within the last five years;
b) Have participated in advisory activities for the evaluated storage facility within the last five years;
c) Currently have financial interests with the evaluated storage facility;
d) Have a spouse, child, parent, sibling, parent-in-law, or parent of spouse working for the evaluated storage facility.
Article 18. Responsibilities and Authorities of the Evaluation Team
1. Responsibilities of the Inspection Team:
a) Evaluate all activities of the storage facility according to the corresponding GSP principles and standards as stipulated in Article 3 of this Circular, updated GSP principles and standards, and related regulatory and professional documents; record specific contents of the evaluation, identified issues, draft an evaluation report, and issue a GSP evaluation report;
b) Report the evaluation results or provide explanations regarding the GSP evaluation report in cases where the storage facility disagrees with the content of the GSP evaluation report;
c) Maintain confidentiality over all information related to the evaluation period and all information related to the storage facility's medicine and active pharmaceutical ingredient storage activities; except with the consent of the storage facility or upon request of authorized state agencies for inspection, supervision, and investigation purposes.
2. Authorities of the Inspection Team:
a) Evaluate all areas and warehouses within the storage facility and have the right to request inspections of other related areas concerning the storage facility's medicine and active pharmaceutical ingredient storage activities;
b) Request provision of documents and materials related to business operations, quality management, and storage of medicines and active pharmaceutical ingredients of the storage facility;
c) Collect documents and evidence (photocopies of documents, photographs, videos) proving issues discovered during the evaluation process;
d) Take samples of medicines and active pharmaceutical ingredients for quality testing as prescribed by law;
đ) Draft minutes and require the storage facility to temporarily suspend storage activities in one, several, or all warehouses. If during the evaluation process, the Evaluation Team discovers serious violations affecting the quality of one or more medicine products or active pharmaceutical ingredients, they must report to the competent authority for handling in accordance with the law.
Chapter VII.IMPLEMENTING PROVISIONS
Article 19.[16] Effective Date
1. This Circular shall take effect from January 10, 2019.
2. The following documents and provisions cease to be effective from the date this Circular takes effect:
a) Decision No. 2701/2001/QĐ-BYT dated June 29, 2001 of the Minister of Health on the implementation of Good Storage Practice standards for drugs;
b) The provisions on Good Storage Practice for drugs in Decision No. 27/2007/QĐ-BYT dated April 19, 2007 of the Minister of Health on the roadmap for implementing the principles and standards of Good Manufacturing Practice for drugs and Good Storage Practice for drugs;
c) The provisions on Good Storage Practice for drugs in Circular No. 45/2011/TT-BYT dated December 21, 2011 of the Minister of Health amending and supplementing certain articles of Decision No. 1570/2000/QĐ-BYT dated May 22, 2000 of the Minister of Health on the implementation of the "Good Laboratory Practice" principle; Decision No. 2701/2001/QĐ-BYT dated June 29, 2001 of the Minister of Health on the implementation of the "Good Storage Practice" principle; Circular No. 06/2004/TT-BYT dated May 28, 2004 of the Minister of Health guiding the production and processing of drugs; Decision No. 3886/2004/QĐ-BYT dated November 3, 2004 of the Minister of Health on the implementation of the principles and standards of "Good Manufacturing Practice for Drugs" according to the recommendations of the World Health Organization; Circular No. 13/2009/TT-BYT dated September 1, 2009 of the Minister of Health guiding drug advertising activities; Circular No. 22/2009/TT-BYT dated November 24, 2009 of the Minister of Health on drug registration; Circular No. 47/2010/TT-BYT dated December 29, 2010 of the Minister of Health guiding drug export and import activities and direct contact drug packaging;
d) The provisions on Good Storage Practice for drugs concerning medicinal materials in Circular No. 03/2016/TT-BYT dated January 21, 2016 of the Minister of Health on the business activities of medicinal materials;
đ) Provision: "If a retail pharmacy does not submit the periodic evaluation application as prescribed, the health administration agency of the province shall revoke the Drug Business Condition Certificate of the retail pharmacy in accordance with Clause 2, Article 40 of the Medicine Law" at Clause 5, Article 9 of Circular No. 02/2018/TT-BYT dated January 22, 2018 of the Minister of Health on Good Retail Pharmacy Practice;[17] e) Provision: "If a distributor does not submit the periodic evaluation application as prescribed, the health administration agency of the province shall revoke the Drug Business Condition Certificate of the distributor in accordance with Clause 2, Article 40 of the Medicine Law" at Clause 5, Article 9 of Circular No. 03/2018/TT-BYT dated February 9, 2018 of the Minister of Health on Good Distribution Practice for Drugs and Raw Materials for Drugs;
Article 21.[19] Transitional Provisions[18] 2. For businesses that have been granted a Drug Business Condition Certificate for drug storage and raw material storage without specifying the validity period, when the validity period of the GSP Certificate expires, the storage facility must proceed with the procedures for the competent authority to assess the continued compliance with GSP and implement related procedures in accordance with the law."
Article 20. Cross-referencing Provisions
In case the regulatory legal documents and provisions cited in this Circular are amended, supplemented, or replaced, they shall be implemented according to the new regulatory legal documents.
4. For businesses that have been granted a Drug Business Condition Certificate with a medicinal material storage warehouse that has achieved GSP and has been inspected and announced on the website of the Traditional Medicine Management Department in accordance with Circular No. 03/2016/TT-BYT dated January 21, 2016 of the Minister of Health on the business activities of medicinal materials, they may continue to operate medicinal material import and export storage for three years from the date of announcement on the website.

2. For businesses that have been issued a Business Registration Certificate for pharmaceutical operations within the scope of drug storage without an expiration date, upon the expiry of the GSP Certification's validity period, the storage facility must undertake procedures for the receiving authority to assess the continued compliance with GSP requirements and carry out related procedures as stipulated by law.”

4. For businesses that have been granted a Business Registration Certificate for pharmaceutical operations with a drug storage warehouse that has achieved GSP standards, which have been inspected and announced on the website of the Traditional Medicine Management Department in accordance with Circular No. 03/2016/TT-BYT dated January 21, 2016, issued by the Minister of Health regarding the operation of herbal medicine trading, such businesses may continue to operate the storage of imported and exported herbal medicines for three years from the announcement date on the website.
Article 22.[22] Responsibilities for Implementation
1. The Drug Administration Department shall be responsible for:
a) Take the lead and coordinate with relevant units to organize the dissemination and popularization of this Circular. Be responsible for compiling a list of Good Storage Practices (GSP) criteria suitable for each type of storage facility according to principles of transparency, clarity, and accuracy for evaluation purposes, to be submitted to the Ministry of Health for issuance, serving as a basis for implementing GSP at storage facilities and for management agencies' evaluations;
b) Implement this Circular for the provincial health sector specialized agencies,[23]health sectors, and drug storage facilities and raw material storage facilities within their assigned functions and tasks;
c) Compile and publish on the Drug Management Department's website a list of national drug storage facilities that have been issued a Business Registration Certificate for Drug Business Conditions and/or a GSP Certificate; update the status of Business Registration Certificates for Drug Business Conditions, GSP Certificates, GSP compliance status, and other information as prescribed, Clause 4, Article 8 of this Circularwithin the scope of their assigned functions and tasks;
d) Publish updated GSP materials on the Ministry of Health's electronic portal and the Drug Management Department's website;
đ) Receive notifications from storage facilities engaged in drug storage and raw material storage activities that do not fall under the issuance of Business Registration Certificates for Drug Business Conditions as stipulated in Clause 3, Article 4 of this Circular, except for provincial and commune-level vaccination management agencies;[24].
e) Be responsible or coordinate with the Ministry Inspectorate to conduct inspections and audits of compliance with this Circular's regulations and handle violations within their authority.
2. The Traditional Medicine Management Department shall be responsible for:
a) Implement this Circular for the provincial health sector specialized agencies,[25]health sectors, and drug storage facilities and raw material storage facilities within their assigned functions and tasks;
b) Compile and publish on the Traditional Medicine and Pharmaceutical Management Department's website a list of national drug storage facilities that have been issued a Business Registration Certificate for Drug Business Conditions and a GSP Certificate; update the status of Business Registration Certificates for Drug Business Conditions, GSP Certificates, GSP compliance status, and other information as prescribed, Clause 4, Article 8 of this Circular within the scope of their assigned functions and tasks;
c) Conduct inspections and audits, and handle violations within their authority.
Article 14. Compliance with Good Storage Practices for Medicines and Raw Materials for Medicines at central-level drug storage facilities of the national health program, people's armed forces units, and vaccine storage facilities under the national expanded immunization program at the central and regional levels.[26] are responsible for:
a) Coordinate with related units to organize the dissemination and popularization of this Circular and provide guidance on implementation to units within their jurisdiction;
b) Participate in inspection, audit, and assessment teams to evaluate GSP compliance; supervise and handle violations within their authority regarding drug storage and raw material storage facilities within their jurisdiction;
c) Receive notifications from storage facilities within their jurisdiction engaged in drug storage activities that do not fall under the issuance of Business Registration Certificates for Drug Business Conditions as stipulated in Clause 4 and Clause 5, Article 4 of this Circular, and vaccine storage facilities in the expanded immunization program at the provincial level and commune-level vaccination management agencies;[27];
d) [28] Publish a list of drug export, import, wholesale drug, and raw material trading enterprises operating through e-commerce on the provincial health sector specialized agency's website within five working days from the date of receipt of the notification from the enterprise.
4. Drug storage facilities and raw material storage facilities shall be responsible for:
a) Organizing the implementation of pharmaceutical laws and standards established in this Circular;
b) Ensuring continuous compliance with GSP principles and standards throughout the operation of the storage facility;
c) Conducting drug storage and raw material storage activities strictly within the scope evaluated and permitted based on compliance with legal regulations and consistent with the operational conditions and capabilities of the storage facility;
d) Facilities with the right to import but not to distribute drugs and raw materials must comply with the provisions of Point c, Clause 10, Article 91 of Decree No. 54/2017/NĐ-CP dated May 8, 2017, and related legal regulations.
đ)[29] Submit a direct notification or send via postal service or email to the provincial health sector specialized agency where the facility conducts business operations before commencing e-commerce activities according to Model No. 07 prescribed in Appendix VII accompanying this Circular, and before ceasing e-commerce activities according to Model No. 08 prescribed in Appendix VII promulgated together with this Circular.
During implementation, if there are difficulties or obstacles, they should be reported to the Ministry of Health for timely consideration and resolution./.

PRINCIPLES AND STANDARDS FOR GOOD STORAGE PRACTICES FOR DRUGS AND RAW MATERIALS
(Issued together with Circular No. 36/2018/TT-BYT dated November 22, 2018, by the Minister of Health)
1. Terminology
Batch of Drugs, Raw Materials
A specific quantity of drugs or raw materials produced in a single process or series of processes and considered uniform.
Batch number
A combination of numbers and/or letters used to uniquely identify a batch of drugs or raw materials.
Shipment (Lot)
The quantity of drugs or raw materials supplied in a single order. A shipment (lot) may include one or more cases or boxes and may contain drugs or raw materials from one or more different batches.
Re-test Date
The date when a raw material must be retested to determine whether it still meets quality standards for continued use in drug production.
Packaging
Packaging Material
Storage
The storage of drugs or raw materials until they are used up.
Separate Storage
The condition where drugs and drug raw materials are stored in isolation either mechanically or through other effective means while awaiting decisions to permit entry into or exit from storage for compounding, production, packaging, distribution, or destruction.
Sampling
Activities designed to obtain a representative portion of a drug or drug raw material according to a suitable statistical procedure for a specific purpose, such as approving batches or releasing batches.
Contamination
The unwanted appearance of chemical or microbiological impurities or foreign substances in or on an initial material, intermediate product, or finished product during production, sampling, packaging, re-packaging, storage, or transportation.
Value of commissions, discounts, and promotional allowances received during procurement
A supplier of drugs or drug raw materials upon request. Suppliers may be agents, brokers, distributors, manufacturers, or traders. Where required, suppliers must be certified by competent authorities.
Quality system
An appropriate infrastructure system including organizational structure, procedures, processes, and resources and systematic actions to ensure confidence that a product (or service) will meet specified quality requirements.
Standard Operating Procedure (SOP)
A documented and approved process providing guidance for performing activities not necessarily related to a specific product but of general nature (such as equipment operation, maintenance and cleaning, validation, facility sanitation and environmental control, sampling, and inspection).
2. Organization and Management
2.1. Each establishment must have a complete organizational structure, clearly depicted in an organizational chart. The organizational chart must clearly describe the responsibilities, authorities, and relationships of all departments (offices, divisions, teams, etc.) within the establishment and key personnel.
2.2. There must be a job description specifying the tasks and responsibilities of each employee, approved by the establishment's head. Assigning responsibilities to employees must ensure that they are not overloaded with work and are consistent with their professional capabilities. Individuals must understand and be familiar with the tasks and responsibilities assigned to them..
2.3. Responsibilities of the technical supervisor:
a) Responsible for all activities related to the technical aspects of the establishment, including compliance with relevant management regulations concerning product quality;
b) Responsible for approving the entry into and exit from storage of products, or may delegate this authority to individuals with equivalent professional qualifications.
2.4. The establishment must designate a person with specific authority and responsibility and provide necessary resources to ensure the development and maintenance of the quality system as well as the identification and adjustment of deviations from the requirements of the applied quality system.
2.5. Administrative and technical management personnel must have sufficient authority and resources to perform their assigned tasks, while implementing, operating, and maintaining the quality system within their scope of work.
3. Personnel
Qualifications and Experience
3.1. The storage facility must have sufficient staff with appropriate qualifications to carry out activities related to receipt, storage, handling, transportation, sanitation, maintenance, and other activities to ensure the quality of drugs and drug raw materials. Among these:
a) For drugs and drug raw materials not subject to special control, the warehouse manager must comply with the following requirements:
- The warehouse manager must be a person with the necessary qualifications and knowledge about pharmacy, warehousing operations (storage methods, inventory management, tracking receipts and issues, drug and drug raw material quality...).
- The warehouse manager must have at least a secondary-level pharmacist qualification for facilities storing chemical pharmaceuticals, vaccines, and medical biological products.
b) For drugs and drug raw materials subject to special control: Personnel must comply with the provisions of Articles 44 and 45 of Decree No. 54/2017/ND-CP, Clause 19 of Article 4 and Clauses 21 and 22 of Article 5 of Decree No. 155/2018/ND-CP, Circular No. 20/2017/TT-BYT dated May 10, 2017, issued by the Minister of Health detailing certain provisions of the Drug Law and Decree No. 54/2017/ND-CP dated May 8, 2017, of the Government regarding drugs and drug raw materials subject to special control and other related regulations.
Training
3.2. All employees must be trained and updated on good warehousing practices for drugs and drug raw materials, legal regulations, implementation procedures, and safety requirements appropriate to their positions. Employees must undergo initial training before starting warehousing activities and must continue to receive annual updates as needed for their assigned tasks. Training effectiveness must be evaluated.
3.3. All employees must be trained and comply with general hygiene and personal hygiene requirements.
3.4. Employees involved in receiving, storing, repackaging, and repackaging of specially controlled drugs and drug raw materials; drugs and drug raw materials with strong activity (sex hormones, cytotoxic chemicals...); drugs and drug raw materials with high sensitivity (beta-lactam antibiotics...), products with fire and explosion risks (such as flammable liquids and solids, compressed gases) must be specifically trained for these activities.
Other Requirements
3.5. Employees and staff working in warehouses must undergo regular health checks as prescribed by law. Those suffering from respiratory diseases or having open wounds are not allowed to directly work in areas handling unpackaged drugs and drug raw materials.
3.6. Employees working in storage areas must be equipped with and wear appropriate protective clothing for the activities in the warehouse. Employees handling drugs and drug raw materials with hazardous properties must be provided with necessary protective clothing to ensure safety and prevent harm to the user.
4. Quality System
4.1. The establishment must have a document defining the establishment's quality policy, which describes overall objectives and quality-related requirements. This document must be approved by the establishment's leadership and communicated to all employees.
4.2. The quality system must be established and operated, and must include an organizational structure, procedures, processes, appropriate resources, and synchronized activities to ensure that products or services and the documentation of the system meet the set quality requirements.
4.3. There must be provisions to ensure that registration authorities, manufacturers, importers, drug management/health administration agencies, and other relevant competent authorities are immediately notified in cases where medicines or medicinal ingredients are determined or suspected to be counterfeit. These products must be stored separately in areas with clear signage, ensuring security and product identification labels to prevent the risk of continued distribution.
4.4. In cases where storage facilities implement electronic means in one, several, or all activities related to receiving, storing, issuing from stock, inspecting, and quality control, the facility must have procedures and necessary means to manage the implementation of electronic means at the facility to ensure traceability of product origin and maintain product quality.
4.5. The facility must have provisions and conduct supplier and distributor evaluations to ensure that medicines and medicinal ingredients are only purchased from compliant suppliers and sold or distributed to compliant recipients.
4.6. The facility must assess risks to the quality and integrity of medicines and medicinal ingredients; based on this, establish and implement measures to mitigate or eliminate these risks. The facility must also establish and periodically review and adjust the quality system to identify and address newly emerging risks.
Traceability of the origin of medicines and medicinal ingredients
4.7. Storage facilities must have provisions to ensure traceability in records for received, stored, quarantined, issued, returned products, facilitating product recall or other necessary actions.
4.8. The facility must maintain records containing information about all entities involved in the supply, distribution, storage, use, and transportation of medicines and medicinal ingredients, including at least the name and address of such entities.
4.9. Storage facilities must establish provisions and measures to ensure that each batch of medicines and medicinal ingredients always has accompanying records to facilitate traceability of origin or determine the next route of the product.
5. Factory, Equipment
5.1. Warehouses must be constructed in dry, safe locations, equipped with drainage systems to ensure medicines and medicinal ingredients are protected from underground water, heavy rain, and floods.
Warehouses must have a specific address, with adequate public or internal traffic systems to ensure convenient transportation, entry and exit, protection, fire prevention, and firefighting.
5.2. Warehouses must be designed, constructed, equipped, repaired, and maintained systematically to protect medicines and medicinal ingredients from potential adverse effects such as temperature and humidity changes, waste and odors, animals, insects, and pests, without affecting the quality of medicines and medicinal ingredients.
Warehouse ceilings, walls, and roofs must be designed and constructed to ensure air circulation, durability against weather impacts like sun, rain, and typhoons.
Warehouse floors must be sufficiently high, flat, smooth, strong, and properly treated to avoid underground water impact, ensure employee operations, and facilitate mechanical equipment movement. There should be no cracks or crevices that accumulate dust or serve as hiding places for pests and insects.
5.3. Storage warehouses must have sufficient area to arrange zones for the following activities:
- Receiving, inspecting medicines and medicinal ingredients, cleaning packaging materials;
- Quarantine (medicines and medicinal ingredients after receipt awaiting inspection and quality control);
- Inspecting and controlling quality of medicines and medicinal ingredients for warehousing;
-[30] If sampling of raw materials is required, sampling and handling of sampling tools must be conducted in the sampling area of the facility or carried out under a contract with another facility having a sampling area meeting the requirements specified in Section 5.6 of this annex;
- Storing medicines and medicinal ingredients;
- Storing medicines and medicinal ingredients requiring special storage conditions;
- Storing medicines and medicinal ingredients requiring special monitoring or separate storage;
- Quarantining goods awaiting processing (returned goods, recalled goods, suspected counterfeit goods, goods suspected of quality issues...);
- Storing medicines and medicinal ingredients before disposal;
- Storing medicines and medicinal ingredients that have been issued from stock awaiting transportation;
- Packaging for transportation and labeling of transportation packaging for medicines and medicinal ingredients;
- Issuing from stock;
- Storing packaging materials;
- Storing equipment and tools for loading, unloading, and moving;
- Labeling imported medicines;
- Changing uniforms, storing personal protective equipment, warehouse office.
5.4. Warehouse zones must have signs clearly indicating the function of each zone, with suitable size and volume, providing enough space for sorting and arranging goods according to different types of medicines and medicinal ingredients; separating by type and batch of medicines and medicinal ingredients, ensuring even air circulation.
When necessary, the positions and sizes between warehouse zones can be adjusted to suit storage activities, but must ensure compliance with storage conditions.
5.5. Storage areas must be designed or adjusted to ensure storage conditions as required and ease of cleaning.
5.6. [31] For facilities with sampling areas, these areas must be designed and equipped with systems complying with the regulations on sampling areas for medicinal ingredients as stipulated in the Circular of the Ministry of Health on Good Manufacturing Practices for Medicines and Medicinal Ingredients.
5.7. The storage warehouse for products with fire and explosion risks (such as flammable liquids and solids, compressed gases...) must be designed and constructed to comply with legal regulations for storing explosive products, located far from other warehouses and residential areas. The warehouse must be well-ventilated and equipped with explosion-proof lights. Electrical switches must be placed outside the warehouse.
5.7. Appropriate equipment and devices must be provided to ensure storage conditions (for example: exhaust fans, air conditioners, thermometers, cargo vehicles, forklifts, hygrometers, cold rooms, refrigerators, vaccine temperature indicators, electronic freeze tags...). These devices must be regularly inspected, maintained, and serviced to ensure stable and accurate operation. Measuring instruments must be calibrated periodically in accordance with legal regulations on inspection and calibration of measuring instruments.
5.8. Automatic detection and warning systems (such as bells, lights, and/or messages) must be installed to promptly report any incidents or deviations from storage conditions, especially for drugs requiring special storage conditions (temperature, humidity).
5.9. The warehouse must be adequately lit to allow all activities within the warehouse area to be carried out accurately and safely.
5.10. There must be sufficient shelving and racks for stacking goods. The distance between shelves and the warehouse floor must be wide enough to facilitate cleaning, inventory checks, and loading/unloading of goods.
Shelves and pallet systems must be kept clean, regularly maintained, and coded to identify their positions for storing goods in the warehouse.
5.11. [32] Information technology equipment connected to the internet must be available to manage drug and medicinal ingredient storage activities through application software. A mechanism for connecting information from drug and medicinal ingredient import, export, and distribution activities to customers must be established to ensure traceability of drug origins and medicinal ingredients; ensure full data extraction when requested by regulatory authorities; and synchronize and update data with the Ministry of Health's pharmaceutical information system as directed.
5.12. Sufficient firefighting equipment and necessary instructions for fire prevention and control measures such as automatic fire suppression systems, fire extinguisher cylinders, sand boxes, water systems, and fire hoses must be provided.
5.13. Storage areas must be dry, clean, and free from garbage and pests; they must avoid exposure to odors, contaminants, and strong light.
5.14. Handwashing facilities and restrooms must have good ventilation and be appropriately located (isolated from receiving, storage, and processing areas).
5.15. Rules governing entry and exit to the warehouse area must be established, and preventive measures must be taken to prevent unauthorized access.
Adequate lighting must be provided in storage areas to enable all activities to be performed accurately and safely. Direct sunlight must not shine onto drugs and medicinal ingredients.
6. Drug and Medicinal Ingredient Storage
6.1. Drugs and medicinal ingredients must be stored under conditions that maintain quality and comply with legal regulations. Drug and medicinal ingredient batches must be distributed and issued according to the "First Expiry, First Out" (FEFO) principle or the "First In, First Out" (FIFO) principle. In certain cases, these principles may not be strictly followed, but it must be ensured that expired products are not distributed.
6.2. Drugs and medicinal ingredients must be arranged on shelves and panels and stored at a height above the floor. Drug and medicinal ingredient packages can be stacked but must be done so without risk of collapse or damage to lower packages.
6.3. Drug packaging must remain intact throughout the storage period. Packaging materials for different types of drugs must not be interchanged.
6.4. Special controlled drug and medicinal ingredient storage areas must display clear signs indicating each type of special controlled drug and medicinal ingredient.
6.5. Security measures must be implemented to prevent loss of special controlled drugs and medicinal ingredients as specified in Decree 54/2017/ND-CP, Decree 155/2018/ND-CP, Circular 20/2017/TT-BYT, and subsequent regulations.
a) Special controlled drug and medicinal ingredient storage areas must display clear signs indicating each type of special controlled drug and medicinal ingredient.
b) Toxic drugs and toxic medicinal ingredients must be packaged to prevent leakage during transportation.
6.6. Drugs and medicinal ingredients with strong activity (such as sex hormones...); highly sensitive drugs (such as beta-lactam antibiotics...), and products with fire and explosion risks (such as flammable liquids and solids and compressed gases) must be stored separately with safety measures in compliance with relevant legal regulations.
6.7. Smelly drugs must be stored in sealed packaging in separate areas to prevent their odors from affecting other drugs and medicinal ingredients.
6.8. Light-sensitive drugs must be stored in sealed packaging, away from light, in enclosed rooms or dark rooms.
6.9. Volatile drugs and moisture-sensitive drugs must be stored in cold storage units with sealed packaging. Strong desiccants must be stored in dry conditions in sealed glass or plastic containers. If possible, stoppers should be coated with paraffin.
6.10. Certain types of vaccines that are easily damaged by freezing (such as VGB, DPT, DT, Td, tetanus, DPT-VGB-Hib, typhoid, cholera...) must be particularly noted during the arrangement and storage process. These vaccines shall not be placed close to the refrigerator wall, bottom of the refrigerator, or near the cooling grid emitting cold air in the cold storage room/chamber; they must be placed at the top of the refrigerator (for refrigerators with an upper-opening door) or on the middle shelf (for refrigerators with a front-opening door). The safety level of the storage area must be monitored using electronic freeze indicators (Freeze Tag) or automatic temperature recording devices equipped with alarm systems.
6.11. Physical isolation measures must be arranged between separate storage areas in the warehouse. Medicines and raw materials for medicines stored in these areas must have clear labels indicating their specific storage conditions, and only authorized personnel may access these areas. Electronic management methods can replace physical isolation, but such methods must be evaluated and controlled to ensure that there is no confusion, loss, or misplacement of stored medicines and raw materials.
6.12. Damaged or defective medicines and raw materials for medicines must be removed from the storage warehouse and kept separately.
Broken or leaking products must be cleaned up as soon as possible to prevent contamination, cross-contamination, and harm to other products or workers in the area. Written procedures must be established to handle such situations.
Storage Conditions
6.13. Storage conditions for medicines and raw materials for medicines must comply strictly with the approved label information or as prescribed.
Unless there are special requirements (such as continuous cold storage), deviations from the above storage conditions are only acceptable for short periods, such as when moving locally within the warehouse.
6.14. Guidelines for storage conditions:
a) Ambient storage conditions:
Store in a dry environment (humidity 75%), at temperatures ranging from 15 - 30°C. In harsh weather conditions, at certain times of the day, the temperature may exceed 30°C but should not exceed 32°C, and humidity should not exceed 80%. Ensure good ventilation, avoid exposure to odors, contaminants, and strong light.
If the label does not specify storage conditions, store under ambient conditions.
b) Special storage conditions: Include cases requiring different storage conditions than ambient conditions.

6.15. Storage conditions must be checked at specified intervals (at least twice per day). These intervals are determined based on continuous monitoring of storage conditions in the warehouse and seasonal changes. Inspection results must be recorded and filed. Records of monitoring data on storage conditions must be available for review.
6.16. Monitoring equipment for storage conditions during storage and transportation must be placed in areas/positions with the greatest potential for variation, based on the assessment of temperature uniformity in the warehouse.
Each warehouse or warehouse section (where sections are physically separated and have independent air conditioning systems) must be equipped with at least one temperature monitoring device capable of automatically recording tracked temperature data at appropriate intervals (at least every 30 minutes). Automatic recording devices must be placed in positions with the highest risk based on the assessment of temperature uniformity.
For medicines and raw materials for medicines with special storage requirements (e.g., vaccines, biological products), continuous monitoring equipment (e.g., temperature) must be used during storage and transportation. The use of monitoring equipment and recorded data must be documented.
6.17. Temperature uniformity assessments in warehouses must be conducted according to the principles outlined in the World Health Organization’s guidance on temperature mapping of storage areas. (Temperature mapping of storage areas).Assessment results must demonstrate uniform temperature throughout the entire storage area.
Labels and Packaging
6.18. Medicines and raw materials for medicines must be stored in suitable packaging that does not adversely affect the quality of the medicines and raw materials, while also protecting them from environmental influences, including bacterial contamination.
6.19. All packaging of medicines and raw materials for medicines must bear clear labels containing all required information as stipulated in Circular No. 01/2018/TT-BYT dated January 18, 2018, of the Ministry of Health regarding labeling of medicines and raw materials for medicines and drug user guides. For packaging not required to be labeled according to this Circular, the following minimum information must be displayed on the packaging: product name, batch number, expiration date or retest date, specific storage conditions.
6.20. Abbreviations, names, or codes not specified for mandatory display on the packaging labels must not be used. International and/or national abbreviations, names, or codes may be used on transport packaging labels.
Control and Inventory Management
6.21. Periodic inventory checks of medicines and raw materials for medicines in the warehouse must be conducted by comparing current stock levels with quantities recorded on tracking sheets. In all cases, inventory checks must be performed when each batch of medicines and raw materials for medicines is fully utilized.
6.22. All discrepancies and losses identified during the inventory check of stored medicines and medicinal ingredients must be investigated through a specific procedure to determine the cause (due to errors, incorrect entry and exit, theft of medicines and medicinal ingredients...). Records of these investigations must be retained.
6.23. Medicines and medicinal ingredients with damaged packaging, not intact, missing labels, torn or unclear labels, or suspected of quality issues shall not be issued. In such cases, the warehouse custodian must immediately notify the quality control department for review and assessment. All actions taken must be recorded.
6.24. Partially used boxes of medicines and medicinal ingredients must be securely resealed to prevent spillage or contamination during subsequent storage.
Control of Expired Goods
6.25. All medicines and medicinal ingredients must be periodically checked for expiration dates. Measures must be taken to prevent the issuance of expired medicines and medicinal ingredients.
7. Receiving Goods
7.1. Upon receiving medicines and medicinal ingredients, each shipment must be inspected against purchase orders, confirming the physical condition of each box, such as label information, batch number, type of medicine or medicinal ingredient, and quantity.
7.2. Each shipment must be inspected for uniformity (appearance) of boxes of medicines and medicinal ingredients according to each batch of medicines and medicinal ingredients.
7.3. Each box of medicines and medicinal ingredients must be carefully inspected for contamination, adulteration, misidentification, tampering, damage, and isolated for further investigation if there are suspicions of these possibilities. The warehouse custodian must immediately notify the quality control department for review and assessment. These actions must be recorded.
All boxes of medicines and medicinal ingredients that are damaged, have lost their seals, or are suspected of being contaminated shall not be admitted into the warehouse, and if they are not immediately destroyed, they must be stored in a separate quarantine area, not sold, or mixed with other medicines and medicinal ingredients.
7.4. If sampling is required, it must be conducted by personnel trained in accordance with Circular No. 11/2018/TT-BYT dated May 4, 2018 of the Ministry of Health on the quality of medicines and medicinal ingredients and strictly follow written sampling guidelines. Boxes of medicines and medicinal ingredients that have been sampled must be labeled to indicate that they have been sampled.
7.5. After sampling, medicines and medicinal ingredients must be quarantined. Batch separation must be maintained throughout the quarantine and subsequent storage periods.
7.6. Medicines and medicinal ingredients must remain in isolation until an official decision is made regarding acceptance or rejection.
7.7. Measures must be taken to ensure that rejected medicines and medicinal ingredients are not put into use. These products must be stored separately from other medicines and medicinal ingredients while awaiting destruction or return to the supplier.
8. Dispatching and Transport
8.1. The dispatching and transport of medicines and medicinal ingredients may only be carried out upon receipt of a dispatch order. Receipt of the dispatch order and dispatching must be recorded in the file.
8.2. Only medicines and medicinal ingredients meeting quality standards and within their expiration date may be dispatched and issued.
8.3. Medicines and medicinal ingredients under quarantine must be approved by the person responsible for quality before they can be dispatched from the warehouse.
8.4. [33] Foreign-invested pharmaceutical business establishments may only dispatch goods and deliver them to wholesale drug establishments at their own drug storage warehouses, transported from customs warehouses to their own warehouses or between their own warehouses, and conduct delivery and transportation activities in accordance with point d, đ, e Clause 3 Article 53a of the Pharmacy Law. For radioactive drugs with an expiration period of 30 days or less, the establishment may proceed in accordance with the provisions at Section 9.11 Clause 9 Appendix I.
8.5. The storage facility must monitor the storage conditions during the transportation of drugs and drug ingredients according to the approved or announced storage conditions on the labels of the drugs and drug ingredients. Records must be kept for review.
8.6. When using dry ice in cold chain systems, special attention must be paid to ensure that drugs and drug ingredients do not come into direct contact with the dry ice, as this may adversely affect the quality of the drugs and drug ingredients, for example causing freezing damage.
8.7. Transportation must ensure the integrity of the drugs and drug ingredients and maintain the storage conditions.
8.8. External containers must protect the drugs and drug ingredients from external influences and be clearly labeled, without being erasable.
8.9. The delivery, receipt, and transportation of drugs and drug ingredients that require special control must comply with the relevant legal regulations.
9. Procedures and Documentation
General Principles
9.1. Documents, particularly guidelines and procedures related to activities affecting the quality of drugs and drug ingredients, must be carefully drafted, finalized, reviewed, and distributed.
9.2. The title, nature, and purpose of each document must be clearly stated. The content of the documents must be clear and unambiguous. These documents must be organized systematically and easily accessible for review.
9.3. All documents must be completed, approved, signed, dated, and not altered without permission.
9.4. Documentation must be regularly reviewed and updated. When a document is revised, there must be a process to ensure that the old version is not mistakenly used.
9.5. All records and ledgers must always be available for reference and review and must be stored in a manner that ensures they cannot be altered, destroyed, damaged, and/or lost.
9.6. Records and ledgers must contain sufficient information to facilitate tracing the origin of drugs and drug ingredients. These records and ledgers must enable the recall of any batch of products if necessary, as well as the investigation of counterfeit drugs and drug ingredients or those at risk of counterfeiting.
9.7. In cases where electronic recordkeeping is implemented, backup copies must be maintained to prevent data loss.
9.8. Approved procedures and work instructions must be readily available and posted in visible locations.
Procedures
9.9. Standard operating procedures must be established for all activities within the drug and drug ingredient storage area, including the handling of expired items. These documents must describe in detail the storage procedures, the flow of drugs and drug ingredients, maintenance of storage equipment, recording of storage conditions and safety in storage and transport, issuance and dispatch procedures, including customer orders, returned goods, recall procedures, and information flow within the organizational system when a recall is required.
9.10. Procedures for drafting, reviewing, approving, using, and controlling changes to all documentation related to storage processes must be established and maintained. Such procedures must be available whether developed internally or obtained from external sources (such as service providers for maintenance, calibration, etc.).
9.11.[34] There must be procedures for receiving and dispatching goods, taking into account the nature of the drugs and drug ingredients and necessary preventive measures. For radioactive drugs with a shelf life of 30 days or less, the facility may transfer the drugs from the import port to the point of use; the receiving and dispatching procedures must specify the steps involved, the responsibilities of the personnel concerned, and the records for this situation to ensure the management of the receiving and dispatching process and the control of radioactive drugs.
9.12. There must be procedures for assessing the uniformity of storage conditions (temperature, humidity) for storage warehouses, cold rooms, refrigerators, transport vehicles, and other related equipment and means.
9.13. There must be security procedures to prevent theft or counterfeiting of products at storage facilities; procedures for disposing of unsellable or unusable products and for storing records.
9.14. There must be procedures for dealing with damaged or leaking containers to completely eliminate potential contamination risks. Special attention must be given to containers holding hazardous products.
9.15. There must be procedures for investigating and addressing non-compliance with storage requirements, such as temperature deviations.
9.16. There must be procedures for identifying, collecting, coding, retrieving, storing, maintaining, removing, and accessing all current records and documents.
9.17. There must be procedures to ensure workplace safety, asset protection, environmental protection, and the integrity of drugs and drug ingredients.
9.18. [35] There must be procedures specifying the frequency and methods of warehouse cleaning and storage area cleaning; procedures for cleaning sampling areas (for facilities with sampling areas).
9.19. There must be procedures for controlling pest species (insects, rodents, etc.). Pesticides used must not pose a risk of contaminating drugs and drug ingredients.
Documentation
9.20. Detailed records must be kept for all activities within the drug and drug ingredient storage area, including the handling of expired items.
9.21. Detailed records of all receipts and dispatches of drugs and drug ingredients must be maintained; these records must be organized in a defined system, facilitating easy retrieval, for example, by internal batch number.
9.22. Each incoming shipment record must minimally include a description of the goods, quality, quantity, manufacturer, original batch number of the manufacturer, supplier, date of receipt, internal batch number, and expiration date.
9.23. The export file must include at least the following information:
- Date, month, year of warehouse dispatch;
- Name and full address of the entity responsible for transportation, telephone number and name of the contact person;
- Name and full address of the receiving entity;
- Description of the medicine or medicinal ingredient (for example: name, formulation, concentration, batch number and quantity);
- Conditions for transportation and storage to be applied.
9.24. There must be a written or electronic file for each medicine or medicinal ingredient stored, indicating the recommended storage conditions, warnings to be noted, and re-inspection period (if applicable). Such files must comply with relevant laws regarding labeling and packaging.
9.25. Separate record-keeping files must be maintained for cases where medicines or medicinal ingredients requiring special control are stored, as stipulated in Decree No. 54/2017/NĐ-CP dated May 8, 2017, Decree No. 155/2018/NĐ-CP dated November 12, 2018, Circular No. 20/2017/TT-BYT, and other related regulatory documents.
Record Keeping and Storage of Files
9.26. Compliance with legal provisions on the retention period for files for each medicine or medicinal ingredient must be observed. In the absence of such provisions, files must be retained for at least one additional year from the expiration date of the medicine or medicinal ingredient batch.
10. Returned Goods
10.1. Returned medicines or medicinal ingredients, including those recalled, must be processed according to approved procedures and documented.
10.2. All returned medicines or medicinal ingredients must be stored in a segregated area and may only be returned to regular storage areas upon approval by authorized personnel based on satisfactory quality assessments to ensure user safety.
10.3. Any batch of medicine or medicinal ingredient that is reissued must be identified and documented.
10.4. Files related to all returned, rejected, and/or destroyed medicines or medicinal ingredients must be retained for a specified period.
10.5. All returned medicines or medicinal ingredients, after being assessed by the quality assurance department as not meeting quality standards or posing risks to users, must be handled in accordance with legal regulations.
11. Recalled Products
11.1. A timely and effective recall process must be established for medicines or medicinal ingredients when errors or suspected errors or counterfeits are discovered, and a responsible person must be assigned to carry out the recall. This system must comply with legal requirements and be regularly reviewed and updated as necessary.
11.2. All recalled medicines or medicinal ingredients must be stored in secure areas that meet storage conditions and isolated pending further actions. If isolation conditions are unavailable, the products must be securely packaged, clearly labeled, and accompanied by appropriate records.
11.3. Special storage conditions for recalled medicines or medicinal ingredients must be maintained throughout the storage and transport period until final disposal decisions are made.
11.4. All files and records must be readily available for the person responsible for the recall. These files and records must contain complete information about distributed medicines or medicinal ingredients (including exported items).
12. Internal Audit
12.1. The quality system must include internal audit activities. Internal audits must be conducted to monitor the implementation and compliance with GSP principles and track corrective and preventive actions, if any.
12.2. Internal audits must be carried out independently and thoroughly by qualified and authorized personnel.
12.3. Records of all internal audits must be kept. The audit report must include all findings from the audit and recommendations for corrective measures, if any. An effective follow-up program must be implemented. Management must review the audit reports and records of any corrective actions taken.

ANNEX II
GOOD PRACTICES FOR THE STORAGE OF MEDICINAL SUBSTANCES AND TRADITIONAL MEDICINES
(Issued together with Circular No. 36/2018/TT-BYT dated November 22, 2018, by the Minister of Health)
1. Personnel
Qualifications and Experience
1.1. The storage facility must have sufficient staff with appropriate qualifications to perform activities related to receipt, storage, handling, transportation, cleaning, maintenance, and other activities to ensure the quality of medicinal substances and traditional medicines. Specifically:
a) The professional head of the importing entity or service provider for the storage of medicinal substances and traditional medicines must have the required qualifications and experience as stipulated in Article 17 and Article 22 of the Medicine Law.
b) The warehouse manager must meet the following requirements:
- Must possess the necessary knowledge and understanding of medicines, medicinal substances, traditional medicines, and warehousing practices (storage methods, record-keeping, tracking of receipts and issues, quality of medicinal substances and traditional medicines...).
- Must have a minimum qualification of a pharmacy technician.
1.2. The facility must establish an organizational chart clearly describing the responsibilities, authorities, and relationships of all departments (offices, divisions, teams...) within the facility and key personnel.
Job descriptions defining tasks and responsibilities for each individual must be established and approved by the facility's head. Assignment of responsibilities must avoid overloading and match the professional capabilities. Individuals must understand and be familiar with their assigned tasks and responsibilities..
Training
1.3. All staff must be trained and updated on good storage practices for medicines and medicinal ingredients, legal regulations, operational procedures, and safety requirements relevant to their positions.
1.4. All staff must be trained and adhere to general hygiene and personal hygiene regulations.
Other Requirements
1.5. Staff working in the warehouse must undergo regular health checks as prescribed by law.
1.6. Staff working in the storage area must be equipped with and wear appropriate protective clothing suitable for warehouse activities.
2. Location of the Storage Facility
2.1. Must be constructed in a dry, safe location with a drainage system to ensure that traditional herbal medicines and remedies are protected from underground water, heavy rain, and floods.
2.2. Must have a definite address located in a convenient transportation area for the export and import of traditional herbal medicines and remedies.
3. Design and construction of storage facilities
3.1. The storage area for traditional herbal medicines and remedies must be spacious enough to accommodate different types of products according to the scale of business operations. The storage facility for traditional herbal medicines and remedies must include areas for the following activities:
- Receiving, inspecting, cleaning, and sanitizing packaging materials;
- Quality control checks on traditional herbal medicines and remedies before storage;
- Sampling traditional herbal medicines and remedies and handling sampling equipment;
- Storing traditional herbal medicines; storing traditional remedies;
- Storing toxic traditional herbal medicines and remedies;
- Isolating goods awaiting processing (returned goods, recalled goods, suspected counterfeit goods, goods suspected of quality issues, etc.);
- Isolating goods prior to disposal;
- Packaging for transport and labeling transport packaging for traditional herbal medicines and remedies;
- Issuing from stock;
- Storing packaging materials;
- Storing equipment and tools for loading, unloading, and moving;
- Changing uniforms, storing personal protective equipment, warehouse office.
3.2. For entities importing or providing storage services for traditional herbal medicines and remedies, the minimum total area must be 500 square meters, and the minimum volume must be 1,500 cubic meters.2, the minimum capacity must be 1,500 m3.
3.3. The receiving area, inspection waiting area, storage area for traditional herbal medicines, and storage area for traditional remedies must be separate and isolated from other areas to prevent cross-contamination and dust accumulation.
The pathways within each area must be designed to follow a unidirectional flow process consistent with the warehouse layout and design.
3.4. The sampling area for traditional herbal medicines and remedies must be designed and equipped with appropriate systems to ensure effective sampling operations.
3.5. Each area of the warehouse must have clear signage indicating its function, and must have suitable dimensions and volumes to allow for proper classification and arrangement of traditional herbal medicines and remedies.
3.6. The storage area must be designed to prevent the intrusion of insects, rodents, and other animals, and to prevent mold growth, pests, and cross-contamination.
3.7. The ceiling, walls, and roof of the warehouse must be sturdy, well-ventilated, and resistant to weather conditions such as sun, rain, typhoons, and floods.
3.8. The warehouse floor must be high, flat, smooth, strong, and properly treated to prevent underground water damage and facilitate the movement of mechanical transportation vehicles; it must not have cracks or crevices where dust can accumulate or pests can hide.
4. Equipment
4.1. Appropriate equipment and devices to ensure storage conditions: exhaust fans, air conditioning systems, cargo trucks, forklifts, thermometers, hygrometers... Dehumidifiers must be available to maintain storage conditions during rainy or humid seasons.
Equipment used to monitor storage conditions: thermometers, hygrometers... must be regularly inspected and calibrated, and the results recorded and stored.
4.2. Adequate lighting must be provided to enable all activities in the warehouse to be carried out accurately and safely.
4.3. There must be sufficient shelving and racks for stacking goods.
The distance between the shelves and the warehouse floor must be at least 15 cm to allow for proper cleaning and prevent debris and dust accumulation. Traditional herbal medicines must not be placed directly on the floor.
The distance between shelves must be wide enough to allow for the movement and operation of handling, transportation, cleaning equipment, and to ensure inspection and verification.
4.4. There must be adequate fire prevention equipment and necessary guidelines as required by fire safety regulations.
4.5. There must be rules governing entry and exit from the warehouse area, and measures must be taken to prevent unauthorized access.
10.0 mg/kg
5.1. The storage area must be clean, free of dust and pests. A written sanitation program must be established specifying the frequency and methods used to clean the factory and warehouse.
5.2. All warehouse staff working in the storage area must undergo regular health checks. Handwashing stations and restrooms must be well-ventilated and appropriately located (isolated from receiving, storage, and processing areas).
5.3. Warehouse workers must wear appropriate protective clothing, and there must be changing rooms and personal lockers.
6. Storage procedures
6.1. General requirements:
a) Traditional herbal medicines and remedies must be rotated so that those received first or with earlier expiration dates are used first. The principle of "First In, First Out" (FIFO) or "First Expired, First Out" (FEFO) must be followed.FIFO - First In First Out) or those with the nearest expiration date should be dispatched first (FEFO - First Expires First OutThese procedures must be carried out.
b) Removed traditional herbal medicines and remedies must be clearly marked and segregated to prevent their use in production, circulation, or consumption.
c) Regular or spot-check programs for inspecting and re-evaluating the condition of products based on their nature and storage conditions must be established to determine compliance with standards and suitability for use, for example, after long-term storage or exposure to temperature (heat) or humidity.
d) A record-keeping system and operational procedures must be maintained to ensure proper storage and monitoring of the receipt, dispatch, and quality of traditional herbal medicines and remedies.
đ) Traditional herbal medicines and remedies must be stored under dry, ventilated conditions with temperatures maintained between 15 - 25°C or up to 30°C depending on climatic conditions, with relative humidity not exceeding 70%.
6.2. Labels and packaging:
a) Traditional herbal medicines and remedies must be stored in appropriate packaging to protect them from environmental influences.
b) All packaging of traditional herbal medicines and remedies must have clear, readable labels containing all required information as per legal labeling requirements for traditional herbal medicines and remedies. Abbreviations, names, or codes not permitted must not be used.
c) Must maintain separate records for each type of packaging, label, or product being stored, indicating storage conditions, necessary preventive measures, and expiration dates (if applicable).
d) Must have a dedicated area for storing labels and printed packaging materials. Specific regulations must be established for the receipt and distribution of these labels and packaging materials.
đ) Must comply with the requirements of the pharmacopoeia and relevant laws concerning labels and packaging.
6.3. The acceptance of traditional herbal medicines must be carried out in a receiving area separate from the storage area. This area must have storage conditions to protect traditional herbal medicines from adverse weather effects during the waiting period for unloading.
6.4. Inspection upon receipt of traditional herbal medicines:
a) Traditional herbal medicines must be inspected and compared with related documentation regarding species, quantity, and other information on the label such as product name, manufacturer, supplier, batch number, expiration date...
b) Each batch must be checked for uniformity, and if necessary, divided into smaller batches according to the supplier's batch number;
c) All traditional herbal medicines with damaged packaging, missing seals, or suspected contamination must not be admitted into storage, and if they are not immediately destroyed, they must be stored in a separate quarantine area and not sold or mixed with other traditional herbal medicines;
d) Traditional herbal medicines requiring cold storage must be quickly inspected, classified, and stored according to the instructions on the label and legal regulations.
6.5. Storage of traditional herbal medicines:
a) Traditional herbal medicines must be stored in dry, well-ventilated warehouses. Pallets must be arranged reasonably to ensure air circulation. Suitable packaging materials for traditional herbal medicines may include glass, plastic, paper, etc. Traditional herbal medicines containing essential oils also need to be stored in sealed packaging.
b) Toxic traditional herbal medicines must be stored strictly in accordance with the relevant regulations.
c) Packaging of traditional herbal medicines must remain intact throughout the storage process. Packaging materials for different types should not be interchanged.
d) Periodic inventory checks of traditional herbal medicines in the warehouse must be conducted by comparing the current stock with the recorded quantities on the tracking documents. In all cases, inventory checks must be performed when each batch is used up.
đ) All discrepancies and losses must be investigated to determine the cause due to mixing, carelessness, or other errors.
e) Regularly check batch numbers and expiration dates to ensure compliance with the FIFO or FEFO principle and to identify near-expired or expired products.
g) Periodically inspect the quality of stored goods to detect damage caused by storage conditions such as temperature, humidity, or other factors that may affect the quality of traditional herbal medicines.
h) Traditional herbal medicines that do not meet quality standards or have expired must be stored in a separate area, labeled, and marked as awaiting disposal. Measures must be taken to prevent the distribution and use of expired traditional herbal medicines or those that do not meet quality standards.
6.6. Distribution - Inventory Rotation (Transportation regulations do not apply to entities authorized for import/export but not yet authorized for drug distribution)
a) Only traditional herbal medicines with intact packaging and within their expiration dates can be distributed. Damaged packaging or those suspected of poor quality cannot be distributed or sold.
b) Loading and dispatching can only be carried out after receiving a written dispatch order. Dispatch procedures must be established based on the nature of the product and after considering all preventive measures.
c) Distribution must follow the first-in-first-out (FIFO) inventory rotation principle.
d) For products requiring special storage conditions during transportation, such conditions must be ensured.
e) Partially used packaging containing traditional herbal medicines must be securely resealed to prevent spillage or contamination during subsequent storage.
g) Packaging containing damaged traditional herbal medicines, unsealed, missing labels, or torn, unclear labels must not be sold or distributed and must be reported immediately to the quality control department.
h) Transport documents must clearly state: transport time; customer name and address; product name and quantity shipped.
7. Returned Goods
7.1. All returned traditional herbal medicines must be stored in a quarantine area and can only be returned to the storage and circulation area after approval by an authorized person confirming their quality.
7.2. All returned traditional herbal medicines, after being evaluated by the quality assurance department as not meeting quality standards or safety requirements for users, must be handled in accordance with legal regulations.
8. Documentation
8.1. Operating procedures must be posted in easily readable locations and approved by authorized personnel. These procedures must accurately describe each step of the storage process for traditional herbal medicines.
8.2. An appropriate record-keeping system must be established for tracking the receipt and dispatch of traditional herbal medicines. If electronic records are used, they must comply with legal regulations. Measures must be established to prevent unauthorized access, use, or modification of stored data.
8.3. The records must be stored for each incoming shipment and must comply with legal regulations on recordkeeping. The minimum retention period for records is one year after the expiration date of the medicinal materials.
ANNEX III
PRINCIPLES AND GOOD PRACTICES FOR THE STORAGE OF MEDICINES AND MEDICINE RAW MATERIALS FOR NON-COMMERCIAL STORAGE FACILITIES
(Issued together with Circular No. 36/2018/TT-BYT dated November 22, 2018, by the Minister of Health)
1. Personnel
Qualifications and Experience
1.1. The storage facility must have sufficient staff with appropriate qualifications to carry out activities related to receipt, storage, handling, transportation, cleaning, maintenance, and other activities necessary to ensure the quality of medicines and medicine raw materials. Among these:
a) The person responsible for technical matters must meet the requirements set forth in Articles 17 and 22 of the Medicine Law.
b) For medicines and medicine raw materials not subject to special control, the warehouse manager must meet the following requirements:
- Must have the necessary qualifications and knowledge regarding pharmacy and warehousing practices (storage methods, bookkeeping, tracking receipts and shipments, quality of medicines and medicine raw materials...).
- Must have a minimum qualification of a secondary-level pharmacist for facilities storing chemical pharmaceuticals, vaccines, and medical products.
c) For medicines and medicine raw materials subject to special control, personnel must meet the requirements stipulated in Articles 44 and 45 of Decree No. 54/2017/ND-CP, Clause 19 of Article 4 and Clauses 21 and 22 of Article 5 of Decree No. 155/2018/ND-CP, Circular No. 20/2017/TT-BYT dated May 10, 2017 issued by the Minister of Health detailing certain provisions of the Medicine Law and Decree No. 54/2017/ND-CP dated May 8, 2017 of the Government on medicines and medicine raw materials subject to special control, and other relevant regulations.
1.2. The facility must establish an organizational chart clearly describing the responsibilities, authorities, and relationships of all departments (offices, divisions, teams...) within the facility and key personnel.
Job descriptions defining tasks and responsibilities for each individual must be established and approved by the facility's head. Assignment of responsibilities must avoid overloading and match the professional capabilities. Individuals must understand and be familiar with their assigned tasks and responsibilities..
Training
1.3. All staff must be trained and updated on good storage practices for medicines and medicinal ingredients, legal regulations, operational procedures, and safety requirements relevant to their positions.
1.4. All staff must be trained and adhere to general hygiene and personal hygiene regulations.
1.5. Staff involved in receiving, storing, repackaging, and re-packaging specially controlled medicines and medicine raw materials; medicines and medicine raw materials with strong activity (sex hormones, cytotoxic chemicals...); highly sensitive medicines and medicine raw materials (beta-lactam antibiotics...), products with fire and explosion risks (such as flammable liquids and solids, compressed gases) must be specifically trained for these activities.
Other Requirements
1.6. Staff and officers working in the warehouse must undergo regular health checks as required by law. Those suffering from respiratory diseases or having open wounds are not allowed to work in areas directly handling unpackaged medicines and medicine raw materials.
1.7. Staff working in the storage area must be equipped with and wear appropriate protective clothing suitable for their duties in the warehouse. Staff handling hazardous medicines and medicine raw materials must be provided with necessary protective clothing to ensure safety and prevent harm to themselves.
2. Warehouses and Equipment
2.1. The warehouse must be constructed in a dry, safe location with a drainage system to ensure that medicines and medicine raw materials are protected from underground water, heavy rain, and floods.
Warehouses must have a specific address, with adequate public or internal traffic systems to ensure convenient transportation, entry and exit, protection, fire prevention, and firefighting.
2.2. The warehouse must be designed, built, equipped, repaired, and maintained systematically to protect medicines and medicine raw materials from adverse effects such as temperature and humidity changes, waste and odors, animals, insects, and pests, without affecting the quality of medicines and medicine raw materials.
Warehouse ceilings, walls, and roofs must be designed and constructed to ensure air circulation, durability against weather impacts like sun, rain, and typhoons.
The warehouse floor must be high enough, flat, smooth, sturdy, and properly treated to prevent underground water damage, facilitate employee movement, and mechanical equipment traffic. There should be no cracks or crevices where dust can accumulate or pests can hide.
2.3. The storage warehouse must have sufficient space to arrange areas for the following activities:
- Receiving, inspecting medicines and medicinal ingredients, cleaning packaging materials;
- Quarantine (medicines and medicine raw materials received awaiting inspection and quality control);
- Inspecting and controlling quality of medicines and medicinal ingredients for warehousing;
-[36] In cases requiring sampling of raw materials, sampling and handling of sampling tools must be carried out in the sampling area of the facility or performed under contract with another facility's sampling area that meets the requirements specified in Section 2.6 of this Appendix;
- Storing medicines and medicinal ingredients;
- Storage of medicines and medicine raw materials requiring special storage conditions;
- Storing medicines and medicinal ingredients requiring special monitoring or separate storage;
- Quarantining goods awaiting processing (returned goods, recalled goods, suspected counterfeit goods, goods suspected of quality issues...);
- Storing medicines and medicinal ingredients before disposal;
- Storing medicines and medicinal ingredients that have been issued from stock awaiting transportation;
- Packaging for transportation and labeling of transportation packaging for medicines and medicinal ingredients;
- Issuing from stock;
- Storing packaging materials;
- Storing equipment and tools for loading, unloading, and moving;
- Changing clothes, storing personal protective equipment, and office space;
2.4. Each area of the warehouse must have clear signage indicating its function, must have appropriate size and volume, and sufficient space to allow classification and arrangement of goods according to different types of medicines and raw materials; separation by type and batch of medicines and medicine raw materials, ensuring even air circulation.
When necessary, the positions and sizes between warehouse zones can be adjusted to suit storage activities, but must ensure compliance with storage conditions.
2.5. Storage areas must be designed or adjusted to ensure storage conditions as required and ease of cleaning.
2.6. [37] For facilities with a sampling area, this area must be designed and equipped with systems meeting the requirements for raw material sampling areas as stipulated in the Circular of the Ministry of Health on Good Manufacturing Practices for Medicines and Medicine Raw Materials.
2.7. Warehouses storing products with fire and explosion risks (such as flammable liquids and solids, compressed gases...) must be designed and constructed according to legal regulations for storing explosive products, located far from other warehouses and residential areas. The warehouse must be well-ventilated and equipped with explosion-proof lighting. Electrical switches must be placed outside the warehouse.
2.8. Appropriate equipment and devices must be provided to ensure storage conditions (for example: exhaust fans, air conditioners, thermometers, cargo vehicles, forklifts, hygrometers, cold rooms, refrigerators, vaccine temperature indicators, electronic freeze tags...). These devices must be regularly inspected, maintained, and serviced to ensure stable and accurate operation. Measuring instruments must be periodically calibrated in accordance with legal regulations on measuring instrument verification and calibration.
2.9. Automatic detection and warning systems (such as bells, lights, and/or messages) must be installed to promptly alert to storage condition failures, especially for medicines with special storage requirements (temperature, humidity).
2.10. The warehouse must be adequately lit to allow all activities within the warehouse area to be conducted accurately and safely.
2.11. There must be sufficient racks and shelves to arrange goods. The distance between shelves and the warehouse floor must be wide enough to ensure warehouse cleaning, inspection, and loading/unloading of goods.
Shelves and pallet systems must be kept clean, regularly maintained, and coded to identify their positions for storing goods in the warehouse.
2.12. [38] Information technology equipment connected to the internet must be available to manage drug and medicinal ingredient storage activities through application software. A mechanism for connecting information from drug and medicinal ingredient import, export, and distribution activities to customers must be established to ensure traceability of drug origins and medicinal ingredients; ensure full data extraction when requested by regulatory authorities; and synchronize and update data with the Ministry of Health's pharmaceutical information system as directed.
2.13. There must be sufficient fire-fighting equipment, including necessary guidelines for fire prevention and explosion control such as automatic fire suppression systems, fire extinguisher cylinders, sand boxes, water systems, and fire hoses.
2.14. Storage areas must be dry, clean, and free from garbage and pests; they must avoid influences from odors, contaminants, and strong light.
2.15. Hand washing facilities and toilets must have good ventilation and be properly arranged (isolated from receiving, storage, and drug processing areas).
2.16. There must be internal regulations governing entry and exit from the warehouse area, and preventive measures must be taken to prevent unauthorized persons from entering.
2.17. Adequate lighting must be provided in storage areas to ensure all activities can be carried out accurately and safely. Direct sunlight must not shine on drugs or raw materials.
3. Drug and raw material storage
3.1. Drugs and raw materials must be stored under conditions that maintain their quality and comply with legal regulations. Drug and raw material batches must be distributed and issued according to the "First Expiry, First Out" (FEFO) or "First In, First Out" (FIFO) principles. In certain cases, these principles may not be strictly followed but must ensure that expired products are not distributed.
3.2. Drugs and raw materials must be arranged on shelves and panels and stored at a height above the floor. Pallets and boxes of drugs and raw materials can be stacked but must ensure there is no risk of collapse or damage to the packaging below.
3.3. Drug and raw material packaging must remain intact throughout the storage period. Packaging materials should not be reused for different types of products.
3.4 Receiving and dispatch areas must protect drugs and raw materials from direct weather effects. Receiving areas must be designed and equipped to allow cleaning of incoming packages if necessary before storage.
3.5. Security measures must be implemented to prevent loss of drugs and raw materials requiring special control as stipulated in Decree 54/2017/ND-CP, Decree No. 155/2018/ND-CP, Circular 20/2017/TT-BYT, and subsequent regulations.
a) Special controlled drug and medicinal ingredient storage areas must display clear signs indicating each type of special controlled drug and medicinal ingredient.
b) Toxic drugs and toxic medicinal ingredients must be packaged to prevent leakage during transportation.
3.6. Drugs and raw materials with strong activity (sex hormones, cytotoxic chemicals...); highly sensitive drugs (such as beta-lactam antibiotics...) and products with fire and explosion risks (flammable liquids, solids, and compressed gases) must be stored separately with safety and security measures in accordance with relevant legal regulations.
3.7. Drugs and raw materials with odor must be stored in sealed packaging in separate areas to prevent odors from affecting other drugs and raw materials.
3.8. Drugs and raw materials sensitive to light must be stored in sealed packaging, away from light, in enclosed rooms or dark rooms.
3.9. Drugs and raw materials that are volatile and moisture-sensitive must be stored in cold storage with sealed packaging. Strong desiccants must be stored in dry conditions with sealed glass or plastic containers. If possible, stoppers should be coated with paraffin.
3.10. Certain vaccines prone to freezing damage (such as VGB, DPT, DT, Td, tetanus, DPT-VGB-Hib, typhoid, cholera...) must be particularly noted during arrangement and storage. These vaccines must not be placed close to refrigerator walls, bottoms, or near cooling units in cold storage rooms; they should be positioned at the top of the refrigerator (for top-opening refrigerators) or on middle shelves (for front-opening refrigerators). Safety monitoring of storage areas must be conducted using electronic freeze indicators or automatic temperature recorders with alarm devices.
3.11. Physical isolation measures must be set up between special storage areas in the warehouse. Drugs and raw materials stored in these areas must have clear signage indicating their status, and only authorized personnel may access them. Electronic management methods can replace physical isolation, but these methods must be reviewed and controlled to prevent confusion and loss of stored drugs and raw materials.
3.12. Damaged or defective drugs and raw materials must be removed from storage and kept separately.

Storage Conditions
3.13. Storage conditions for drugs and raw materials must comply with approved or published label information.
Unless there are special requirements (such as continuous cold storage), deviations from the above storage conditions are only acceptable for short periods, such as when moving locally within the warehouse.
3.14. Guidance for storage conditions:
a) Ambient storage conditions:
Store in a dry environment (humidity 75%), at temperatures from 15-30°C. During extreme weather, temperatures may exceed 30°C but not surpass 32°C, and humidity must not exceed 80%. Ensure good ventilation and avoid influences from odors, contaminants, and strong light.
If the label does not specify storage conditions, store under ambient conditions.
b) Special storage conditions: Include cases requiring different storage conditions than ambient conditions.

3.15. Storage conditions must be checked at predetermined appropriate times (at least twice daily). Inspection results must be recorded and filed. Records of storage condition monitoring data must be available for review.
3.16. Monitoring equipment for storage conditions during storage and transportation must be placed in areas/positions with the greatest temperature fluctuations based on warehouse temperature uniformity assessment results.
Each warehouse or warehouse section (where sections are physically separated and have independent air conditioning systems) must be equipped with at least one temperature monitoring device capable of automatically recording tracked temperature data at appropriate intervals (at least every 30 minutes). Automatic recording devices must be placed in positions with the highest risk based on the assessment of temperature uniformity.
For medicines and raw materials for medicines with special storage requirements (e.g., vaccines, biological products), continuous monitoring equipment (e.g., temperature) must be used during storage and transportation. The use of monitoring equipment and recorded data must be documented.
3.17. The temperature uniformity assessment in the warehouse must be conducted in accordance with the principles set forth in the World Health Organization's guidance on assessing the temperature uniformity of storage warehouses. (Temperature mapping of storage areas).Assessment results must demonstrate uniform temperature throughout the entire storage area.
Labels and Packaging
3.18. Medicines and medicinal ingredients must be stored in appropriate packaging that does not adversely affect the quality of the medicines and medicinal ingredients, while also protecting them from environmental influences, including preventing contamination.
3.19. All packaging of medicines and medicinal ingredients must bear clear labels containing all required information as stipulated in Circular No. 01/2018/TT-BYT dated January 18, 2018, issued by the Ministry of Health, regarding the labeling of medicines and medicinal ingredients and the drug usage guide. For packaging not required to be labeled according to this Circular, the following minimum information must be displayed on the packaging: product name, batch number, expiration date or retest date, specific storage conditions.
3.20. Abbreviations, names, or codes not specified for mandatory display on the packaging labels shall not be used. International and/or national abbreviations, names, or codes may be used on transport packaging labels.
Control and Inventory Management
3.21. Periodic reconciliation of medicines and medicinal ingredients in the warehouse must be carried out by comparing the current stock of medicines and medicinal ingredients with the recorded quantities on the medicine and medicinal ingredient tracking forms. In all cases, reconciliation must be performed when each batch of medicines and medicinal ingredients is fully utilized.
3.22. Tất cả các sai lệch, thất thoát khi đối chiếu số lượng thuốc, nguyên liệu làm thuốc lưu kho phải được điều tra theo quy trình cụ thể để xác định nguyên nhân (do nhầm lẫn, do xuất nhập chưa đúng, do trộm cắp thuốc, nguyên liệu làm thuốc…). Sổ sách ghi chép về các cuộc điều tra này phải được lưu giữ.
3.23. Không được cấp phát các thuốc, nguyên liệu làm thuốc có bao bì bị hư hại, không còn nguyên vẹn, mất nhãn hoặc nhãn bị rách, không rõ ràng, hoặc có nghi ngờ về chất lượng. Trường hợp này, thủ kho phải thông báo ngay với bộ phận kiểm tra chất lượng để xem xét, đánh giá. Mọi hành động tiến hành phải được ghi chép lại.
3.24. Các thùng thuốc, nguyên liệu làm thuốc đã được sử dụng một phần cần phải được đóng kín lại một cách an toàn để tránh việc rơi vãi hoặc nhiễm bẩn trong thời gian bảo quản sau này.
Kiểm soát hàng hết hạn dùng
3.25. Tất cả thuốc, nguyên liệu làm thuốc phải được kiểm tra định kỳ về hạn dùng. Phải tiến hành các biện pháp đề phòng việc cấp phát thuốc, nguyên liệu làm thuốc đã hết hạn dùng.
4. Nhập hàng
4.1. Cơ sở phải có các quy định và thực hiện việc đánh giá nhà cung cấp để bảo đảm thuốc, nguyên liệu làm thuốc chỉ được mua, cung ứng từ các cơ sở cung cấp đáp ứng quy định của pháp luật và thuốc, nguyên liệu làm thuốc phải được cấp phép lưu hành, sử dụng.
4.2. Khi nhận thuốc, nguyên liệu làm thuốc, mỗi chuyến hàng cần được kiểm tra so với lệnh/đơn mua hàng, phải xác nhận tình trạng vật lý của từng thùng hàng, ví dụ: thông tin trên nhãn, số lô, loại thuốc, nguyên liệu làm thuốc và số lượng.
Mỗi chuyến hàng phải được kiểm tra về tính đồng nhất (về ngoại quan) của các thùng thuốc, nguyên liệu làm thuốc theo từng lô thuốc, nguyên liệu làm thuốc.
4.3. Mỗi thùng thuốc, nguyên liệu làm thuốc phải được kiểm tra cẩn thận về khả năng bị nhiễm bẩn, tạp nhiễm, nhầm lẫn, bị can thiệp, bị hư hỏng và phải thực hiện việc cách ly để tiếp tục điều tra nếu có nghi ngờ về các khả năng này. Thủ kho phải thông báo ngay với bộ phận kiểm tra chất lượng để xem xét, đánh giá. Các hành động này phải được ghi chép lại.
Tất cả các thùng thuốc, nguyên liệu làm thuốc bị hư hại, mất dấu niêm phong hoặc bị nghi ngờ có tạp nhiễm thì không được nhập kho, và nếu không được huỷ bỏ ngay thì phải được bảo quản ở khu vực biệt trữ riêng, không được bán, hoặc để lẫn với các thuốc, nguyên liệu làm thuốc khác.
4.4. Trường hợp cần tiến hành lấy mẫu, việc lấy mẫu phải được thực hiện bởi người đã được đào tạo theo quy định tại Thông tư số 11/2018/TT-BYT ngày 04/5/2018 của Bộ Y tế quy định về chất lượng thuốc, nguyên liệu làm thuốc và tuân thủ nghiêm ngặt hướng dẫn lấy mẫu bằng văn bản. Các thùng thuốc, nguyên liệu làm thuốc đã được lấy mẫu phải được dán nhãn thể hiện tình trạng đã lấy mẫu.
4.5. Sau khi lấy mẫu, thuốc, nguyên liệu làm thuốc phải được biệt trữ. Phải duy trì việc phân tách theo lô trong suốt quá trình biệt trữ và bảo quản sau đó.
4.6. Thuốc, nguyên liệu làm thuốc phải được duy trì tình trạng cách ly cho đến khi có quyết định chính thức về việc chấp nhận hay loại bỏ.
4.7. Phải có biện pháp đảm bảo thuốc và nguyên liệu bị loại bỏ không được đưa ra sử dụng. Các sản phẩm này phải được bảo quản tách biệt khỏi các thuốc và nguyên liệu khác trong khi chờ hủy hoặc gửi trả nhà cung cấp.
5. Xuất hàng và vận chuyển
5.1. Cơ sở phải có các quy định và biện pháp bảo đảm thuốc, nguyên liệu làm thuốc chỉ được phân phối, cấp phát cho các cơ sở đáp ứng quy định của pháp luật.
5.2. Việc xuất và vận chuyển thuốc, nguyên liệu làm thuốc chỉ được tiến hành sau khi nhận được lệnh xuất hàng. Việc tiếp nhận lệnh xuất hàng và việc xuất hàng phải được ghi chép vào hồ sơ.
5.3. Chỉ được xuất, cấp phát các thuốc, nguyên liệu làm thuốc đạt tiêu chuẩn chất lượng, còn trong hạn sử dụng. Các thuốc, nguyên liệu làm thuốc đang trong quá trình biệt trữ phải được người chịu trách nhiệm về chất lượng cho phép mới được xuất kho.

5.5. Cơ sở phải tiến hành kiểm soát điều kiện bảo quản trong quá trình vận chuyển thuốc, nguyên liệu làm thuốc theo đúng điều kiện bảo quản ghi trên trên nhãn thuốc, nguyên liệu làm thuốc đã được phê duyệt hoặc công bố theo quy định. Phải lưu hồ sơ theo dõi để rà soát.
5.6. Khi sử dụng đá khô trong dây chuyền lạnh, phải đặc biệt chú ý đảm bảo thuốc, nguyên liệu làm thuốc không tiếp xúc trực tiếp với đá khô, vì có thể gây ảnh hưởng xấu đến chất lượng thuốc, nguyên liệu làm thuốc, ví dụ bị đông băng.
5.7. Việc vận chuyển phải đảm bảo tính toàn vẹn của thuốc, nguyên liệu làm thuốc và điều kiện bảo quản được duy trì.
5.8. Thùng chứa bên ngoài phải cho phép bảo vệ thuốc và nguyên liệu làm thuốc khỏi các tác động từ bên ngoài và được dán nhãn rõ ràng, không tẩy xóa được.
5.9. Việc giao, nhận, vận chuyển thuốc, nguyên liệu làm thuốc phải kiểm soát đặc biệt phải theo đúng quy định của pháp luật có liên quan
6. Quy trình và hồ sơ tài liệu
Nguyên tắc chung
6.1. Các tài liệu, đặc biệt là các hướng dẫn và quy trình liên quan tới các hoạt động ảnh hưởng đến chất lượng của thuốc, nguyên liệu làm thuốc, phải được soạn thảo, hoàn thiện, rà soát và phân phối một cách cẩn trọng.
6.2. Tiêu đề, bản chất và mục đích của mỗi tài liệu phải được nêu rõ ràng. Nội dung của các tài liệu phải rõ ràng, rành mạch. Các tài liệu này phải được sắp xếp quy củ và dễ kiểm tra.
6.3. Tất cả các tài liệu đều phải được người có trách nhiệm hoàn thiện, phê duyệt, ký và ghi ngày, tháng, năm và không được thay đổi khi chưa được phép.
6.4. Hồ sơ, tài liệu phải thường xuyên được rà soát và cập nhật. Khi có một tài liệu nào đó được sửa chữa thì phải có quy trình để đảm bảo phiên bản cũ đã bị thay thế không bị sử dụng do nhầm lẫn.
6.5. Tất cả các hồ sơ, sổ sách phải luôn sẵn sàng để phục vụ việc tra cứu, rà soát và phải được bảo quản và lưu trữ bằng các phương tiện bảo đảm chúng không bị sửa chữa, tiêu hủy, gây hư hỏng và/hoặc mất mát.
6.6. Hồ sơ, sổ sách phải có đủ thông tin để tạo điều kiện cho việc truy nguyên nguồn gốc thuốc, nguyên liệu làm thuốc. Các hồ sơ, sổ sách này phải tạo điều kiện cho việc thu hồi một lô sản phẩm bất kỳ, nếu cần, cũng như điều tra các thuốc, nguyên liệu làm thuốc giả hoặc các thuốc, nguyên liệu làm thuốc có khả năng bị làm giả.
6.7. Trường hợp cơ sở xây dựng và lưu trữ hồ sơ dưới dạng điện tử thì phải có bản sao dự phòng tránh trường hợp sự cố mất dữ liệu.
6.8. Các quy trình, hướng dẫn công việc đã được phê duyệt cần phải có sẵn, treo tại các nơi dễ đọc
Các quy trình
6.9. Phải có các quy trình thao tác chuẩn đối với tất cả các hoạt động trong khu vực bảo quản thuốc, nguyên liệu làm thuốc, kể cả việc xử lý hàng hết hạn. Các văn bản này phải mô tả đầy đủ quy trình bảo quản, đường đi của thuốc, nguyên liệu làm thuốc, bảo trì bảo dưỡng kho tàng thiết bị dùng trong bảo quản, quy định về việc ghi chép điều kiện bảo quản, an toàn thuốc tại kho và trong quá trình vận chuyển, việc cấp phát, xuất hàng, việc lập hồ sơ, bao gồm cả các bản ghi về đơn đặt hàng của khách hàng, thuốc trả về, quy trình thu hồi và luồng thông tin trong hệ thống tổ chức khi có yêu cầu thu hồi thuốc, nguyên liệu làm thuốc.
6.10. Phải xây dựng và duy trì các quy trình về việc soạn thảo, rà soát, phê duyệt, sử dụng và kiểm soát những thay đổi của tất cả tài liệu liên quan tới quá trình bảo quản. Phải có sẵn các quy trình liên quan dù chúng là do nội bộ cơ sở xây dựng hay lấy từ nguồn bên ngoài (từ các cơ sở thực hiện dịch vụ một số hoạt động như bảo trì, hiệu chuẩn…).
6.11. Phải có quy trình nhập, xuất hàng, trong đó có cân nhắc đến bản chất của thuốc, nguyên liệu làm thuốc và các biện pháp phòng ngừa cần thiết.
6.12. Phải có quy trình đánh giá độ đồng đều điều kiện bảo quản (nhiệt độ, độ ẩm) đối với kho bảo quản, buồng lạnh, tủ lạnh, xe vận chuyển và các thiết bị, phương tiện khác có liên quan.
6.13. Phải có các quy trình đảm bảo an ninh để đề phòng tình trạng trộm cắp hoặc làm giả sản phẩm tại các cơ sở bảo quản; quy trình hủy các sản phẩm không bán được hoặc không sử dụng được và quy trình về việc lưu trữ hồ sơ.
6.14. Phải có các quy trình để xử lý các thùng chứa hàng bị hư hỏng và/hoặc đổ vỡ, rò rỉ nhằm bảo đảm loại bỏ hoàn toàn các nguy cơ có thể gây tạp nhiễm. Cần đặc biệt lưu ý các thùng hàng chứa các sản phẩm có nguy cơ độc hại.
6.15. Phải có các quy trình để điều tra và xử lý các trường hợp không tuân thủ các yêu cầu bảo quản như sai lệch nhiệt độ.
6.16. Phải có các quy trình về nhận biết, thu thập, đánh mã số, hồi cứu, bảo quản, bảo trì, loại bỏ và tiếp cận đối với tất cả các hồ sơ, tài liệu đang áp dụng.
6.17. Phải có quy trình để bảo đảm an toàn lao động, tài sản, bảo vệ môi trường và tính toàn vẹn của thuốc, nguyên liệu làm thuốc.
6.18. [40] Phải có quy trình quy định tần suất và phương pháp vệ sinh nhà kho và các khu vực bảo quản; quy trình vệ sinh khu vực lấy mẫu thuốc, nguyên liệu làm thuốc (đối với cơ sở có khu vực lấy mẫu).
6.19. Phải có quy trình quy định về kiểm soát các loài vật gây hại (côn trùng, chuột bọ…). Các chất dùng để diệt các loài vật gây hại phải không có nguy cơ gây ô nhiễm cho thuốc, nguyên liệu làm thuốc.
Hồ sơ tài liệu
6.20. Phải có hồ sơ ghi chép đối với tất cả các hoạt động trong khu vực bảo quản thuốc, nguyên liệu làm thuốc, kể cả việc xử lý hàng hết hạn.
6.21. Phải có hồ sơ chi tiết thể hiện tất cả các lần nhận, xuất của thuốc, nguyên liệu làm thuốc; các hồ sơ này phải được sắp xếp theo một hệ thống xác định, thuận lợi cho việc tra cứu, ví dụ: theo số lô nội bộ.
6.22. Hồ sơ cho mỗi chuyến hàng nhập phải tối thiểu bao gồm mô tả về hàng hóa, chất lượng, số lượng, nhà sản xuất, số lô gốc của nhà sản xuất, nhà cung cấp, ngày nhận, số lô nội bộ và hạn dùng.
6.23. Hồ sơ xuất hàng phải bao gồm tối thiểu các thông tin sau đây:
- Ngày, tháng, năm xuất kho;
- Tên và địa chỉ đầy đủ của đơn vị chịu trách nhiệm vận chuyển, số điện thoại và tên của người liên hệ;
- Tên và địa chỉ đầy đủ của đơn vị tiếp nhận;
- Mô tả về thuốc, nguyên liệu làm thuốc (ví dụ: tên, dạng bào chế, hàm lượng, số lô và số lượng);
- Các điều kiện vận chuyển và bảo quản được áp dụng.
6.24. Phải có hồ sơ bằng văn bản viết hoặc điện tử đối với mỗi thuốc hoặc nguyên liệu làm thuốc được bảo quản, trong đó chỉ ra điều kiện bảo quản được khuyến cáo, cảnh báo cần lưu ý và hạn tái kiểm (nếu có). Phải đáp ứng các quy định của pháp luật liên quan đến nhãn và bao bì.
6.25. Có hồ sơ sổ sách riêng đối với trường hợp có bảo quản thuốc, nguyên liệu làm thuốc phải kiểm soát đặc biệt theo quy định tại Nghị định 54/2017/NĐ- CP ngày 08/5/2017, Nghị định số 155/2018/NĐ-CP ngày 12/11/2018 và Thông tư số 20/2017/TT-BYT và văn bản quy phạm pháp luật khác có liên quan.
Lưu trữ, bảo quản hồ sơ
6.26. Phải tuân thủ quy định pháp luật về thời gian bảo quản hồ sơ. Trường hợp không có quy định, hồ sơ phải được lưu ít nhất cho đến sau thời điểm hết hạn của lô thuốc, nguyên liệu làm thuốc cộng thêm 01 (một) năm.
7. Hàng trả về
7.1. Thuốc, nguyên liệu làm thuốc trả về, bao gồm cả thuốc, nguyên liệu làm thuốc bị thu hồi, phải được xử lý theo quy trình đã được phê duyệt và lưu hồ sơ.
7.2. Tất cả thuốc, nguyên liệu làm thuốc trả về phải được bảo quản trong khu vực biệt trữ và chỉ được đưa trở lại khu bảo quản thường sau khi được sự phê duyệt bởi người có thẩm quyền căn cứ trên các đánh giá thoả đáng về chất lượng, đảm bảo an toàn cho người sử dụng.
7.3. Bất cứ lô thuốc, nguyên liệu làm thuốc nào khi tái cấp phát phải được nhận dạng và ghi hồ sơ.
7.4. Hồ sơ liên quan đến tất cả các thuốc, nguyên liệu làm thuốc bị trả về, bị từ chối tiếp nhận và/hoặc bị tiêu hủy phải được lưu giữ trong một khoảng thời gian xác định.
7.5. Tất cả các thuốc, nguyên liệu làm thuốc trả về, sau khi được bộ phận bảo đảm chất lượng đánh giá là không đảm bảo chất lượng, không đảm bảo an toàn cho người sử dụng phải được xử lý theo quy định của pháp luật.
8. Sản phẩm bị thu hồi
8.1. Phải có quy trình thu hồi từ thị trường một cách kịp thời và hiệu quả đối với thuốc, nguyên liệu làm thuốc khi phát hiện có lỗi hoặc nghi ngờ có lỗi hoặc bị giả mạo, và có người được giao trách nhiệm thu hồi. Hệ thống này phải tuân thủ quy định pháp luật. Quy trình này phải được kiểm tra và cập nhật thường xuyên khi cần.
8.2. Tất cả các thuốc, nguyên liệu bị thu hồi phải được bảo quản ở khu vực bảo đảm an ninh, đáp ứng điều kiện bảo quản của hàng hóa và cách ly để chờ các hoạt động xử lý tiếp theo. Đối với những nơi không có điều kiện cách ly sản phẩm bị thu hồi thì sản phẩm đó phải được bao gói an toàn, ghi nhãn rõ ràng và có sổ sách ghi chép phù hợp kèm theo.
8.3. Các điều kiện bảo quản đặc biệt đối với thuốc, nguyên liệu làm thuốc bị thu hồi phải được duy trì trong suốt quá trình bảo quản và chờ vận chuyển cho đến khi có quyết định cuối cùng về việc xử lý sản phẩm bị nghi ngờ.
8.4. Tất cả các hồ sơ, sổ sách phải sẵn sàng để cung cấp cho người chịu trách nhiệm đối với việc thu hồi. Các hồ sơ, sổ sách này phải có đầy đủ các thông tin về các thuốc, nguyên liệu làm thuốc đã cấp phát (kể cả thuốc, nguyên liệu làm thuốc được xuất khẩu).
9. Tự thanh tra
9.1. Hệ thống chất lượng phải bao gồm hoạt động tự thanh tra. Hoạt động tự thanh tra phải được thực hiện để theo dõi việc triển khai và tuân thủ các nguyên tắc GSP và theo dõi các hành động khắc phục và phòng ngừa, nếu có.
9.2. Việc tự thanh tra phải do người có chuyên môn và có thẩm quyền tiến hành một cách độc lập và chi tiết.
9.3. Kết quả của tất cả các đợt tự thanh tra phải được ghi chép. Biên bản tự thanh tra phải bao gồm tất cả các điểm phát hiện được trong đợt tự thanh tra và các đề xuất về biện pháp khắc phục, nếu có. Cần có chương trình theo dõi một cách hiệu quả. Người quản lý phải đánh giá biên bản tự thanh tra và các hồ sơ về bất kỳ hành động khắc phục nào đã được thực hiện./.

1.1. Cơ sở bảo quản phải có đủ nhân viên với trình độ phù hợp để thực hiện các hoạt động liên quan đến xuất nhập, bảo quản, cấp phát thuốc và các hoạt động khác nhằm đảm bảo chất lượng thuốc. Trong đó:
a) Đối với thuốc không phải kiểm soát đặc biệt, thủ kho phải đáp ứng các quy định sau:
- Phải có trình độ, hiểu biết cần thiết về dược, về nghiệp vụ bảo quản (phương pháp bảo quản, quản lý sổ sách, theo dõi xuất nhập, chất lượng thuốc…).
- Phải có trình độ tối thiểu là dược sĩ trung học.
c) Đối với thuốc phải kiểm soát đặc biệt: Nhân sự phải đáp ứng quy định tại Thông tư 20/2017/TT-BYT ngày 10/05/2017 của Bộ trưởng Bộ Y tế quy định chi tiết một số điều của Luật dược và Nghị định số 54/2017/NĐ-CP ngày 08/05/2017 của Chính phủ về thuốc và nguyên liệu làm thuốc phải kiểm soát đặc biệt và các quy định khác có liên quan.
1.2. Phải có bản mô tả công việc xác định rõ nhiệm vụ và trách nhiệm có liên quan cho từng cá nhân, được người đứng đầu cơ sở phê duyệt. Cá nhân phải hiểu, nắm rõ nhiệm vụ và trách nhiệm được giao.
Đào tạo
1.3. Tất cả nhân viên phải được đào tạo, cập nhật về thực hành tốt bảo quản thuốc, các quy định luật pháp, các quy trình thao tác, các quy định về vệ sinh, an toàn phù hợp với vị trí công việc.
1.4. Nhân viên tham gia vào các hoạt động tiếp nhận, bảo quản, đóng gói, đóng gói lại thuốc kiểm soát đặc biệt; thuốc, nguyên liệu có hoạt lực mạnh (hormon sinh dục, hóa chất độc tế bào…); thuốc có tính nhạy cảm cao (kháng sinh nhóm betalactam…), các sản phẩm có nguy cơ gây cháy nổ (như các chất lỏng và chất rắn dễ bắt lửa, dễ cháy và các loại khí nén) phải được đào tạo cụ thể cho hoạt động này.
Yêu cầu khác
1.6. Nhân viên và cán bộ làm việc trong kho phải được kiểm tra sức khỏe định kỳ theo quy định của pháp luật. Người mắc các bệnh về đường hô hấp, hoặc có vết thương hở không được làm việc trong khu vực bảo quản có trực tiếp xử lý thuốc có bao bì hở.
1.7. Nhân viên làm việc trong khu vực bảo quản phải được trang bị và mặc trang phục bảo hộ phù hợp với hoạt động tại kho.
2. Nhà xưởng, trang thiết bị
2.1. Kho phải được xây dựng ở nơi cao ráo, an toàn, phải có hệ thống cống rãnh thoát nước, để đảm bảo thuốc tránh được ảnh hưởng của nước ngầm, mưa lớn và lũ lụt.
2.2. Nhà kho phải được thiết kế, xây dựng, trang bị, sửa chữa và bảo trì một cách hệ thống sao cho có thể bảo vệ thuốc tránh được các ảnh hưởng bất lợi có thể có, như: sự thay đổi nhiệt độ và độ ẩm, chất thải và mùi, các động vật, sâu bọ, côn trùng và không ảnh hưởng tới chất lượng thuốc.
Trần, tường, mái nhà kho phải được thiết kế, xây dựng sao cho đảm bảo sự thông thoáng, luân chuyển của không khí, vững bền chống lại các ảnh hưởng của thời tiết như nắng, mưa, bão lụt.
Nền kho phải đủ cao, phẳng, nhẵn, đủ chắc, cứng và không được có các khe, vết nứt gãy... là nơi tích luỹ bụi, trú ẩn của sâu bọ, côn trùng.
2.3 Kho bảo quản phải có diện tích đủ rộng để bố trí các khu vực cho các hoạt động sau:
- Tiếp nhận, kiểm nhập thuốc;
- Bảo quản thuốc;
- Bảo quản thuốc yêu cầu các điều kiện bảo quản đặc biệt;
- Bảo quản thuốc phải kiểm soát đặc biệt hoặc phải bảo quản riêng biệt;
- Biệt trữ hàng chờ xử lý (hàng trả về, hàng thu hồi, hàng bị nghi ngờ là hàng giả, hàng nghi ngờ về chất lượng…);
- Chuẩn bị, đóng gói và cấp phát thuốc;
2.4. Các khu vực của kho phải có biển hiệu chỉ rõ công năng của từng khu vực, phải có diện tích và thể tích phù hợp, đủ không gian để cho phép việc phân loại, sắp xếp hàng hóa theo các chủng loại thuốc khác nhau; phân cách theo từng loại và từng lô thuốc, đảm bảo không khí được lưu thông đều.
2.5. Phải trang bị các phương tiện, thiết bị phù hợp để đảm bảo các điều kiện bảo quản (ví dụ: quạt thông gió, điều hòa không khí, nhiệt kế, xe nâng, ẩm kế, phòng lạnh, tủ lạnh, chỉ thị nhiệt độ vaccin, chỉ thị đông băng điện tử (Freeze tag)…). Các thiết bị phải được kiểm tra, bảo trì, bảo dưỡng định kỳ để đảm bảo hoạt động ổn định, chính xác. Các thiết bị đo phải được hiệu chuẩn định kỳ theo quy định của pháp luật về kiểm định, hiệu chuẩn thiết bị đo.
2.6. Phải có các phương tiện phát hiện và cảnh báo tự động (như chuông, đèn…) kịp thời về các sự cố, sai lệch về điều kiện bảo quản đối với các thuốc có yêu cầu đặc biệt về điều kiện bảo quản (nhiệt độ).
2.7. Kho phải được chiếu đủ sáng, cho phép tiến hành một cách chính xác và an toàn tất cả các hoạt động trong khu vực kho. Không được để ánh sáng mặt trời chiếu trực tiếp vào thuốc.
2.8. Có đủ các trang bị, giá, kệ để xếp hàng. Khoảng cách giữa các giá kệ, giá kệ với nền kho phải đủ rộng đảm bảo cho việc vệ sinh kho, kiểm tra đối chiếu và xếp, dỡ hàng hóa.
2.9. Phải có đủ các trang thiết bị phòng chữa cháy, bản hướng dẫn cần thiết cho công tác phòng chống cháy nổ như: hệ thống phòng chữa cháy tự động, hoặc các bình khí chữa cháy, thùng cát, hệ thống nước và vòi nước chữa cháy.
2.10. Nơi rửa tay, phòng vệ sinh phải được thông gió tốt và bố trí phù hợp (cách ly với khu vực tiếp nhận, bảo quản, xử lý thuốc).
2.11. Có nội quy quy định việc ra vào khu vực kho, và phải có các biện pháp phòng ngừa, ngăn chặn việc ra vào của người không được phép.
3. Bảo quản thuốc
3.1. Thuốc phải được bảo quản trong điều kiện đảm bảo duy trì chất lượng và theo đúng quy định của pháp luật. Các lô thuốc phải được cấp phát theo nguyên tắc “Hết hạn trước xuất trước” (FEFO- First Expires First Out) hoặc nguyên tắc “Nhập trước xuất trước (FIFO- First In First Out).
3.2. Thuốc phải sắp xếp trên giá, kệ, tấm kê panel và được bảo quản ở vị trí cao hơn sàn nhà. Các bao, thùng thuốc có thể xếp chồng lên nhau nhưng phải đảm bảo không có nguy cơ đổ vỡ, hoặc gây hại tới bao bì, thùng thuốc bên dưới.
3.3. Bao bì thuốc phải được giữ nguyên vẹn trong suốt quá trình bảo quản. Không sử dụng bao bì đóng gói của loại này cho loại khác.
3.4 Các khu vực giao, nhận hàng phải đảm bảo bảo vệ thuốc tránh khỏi tác động trực tiếp của thời tiết.
3.5. Phải có biện pháp về an ninh, bảo đảm không thất thoát thuốc phải kiểm soát đặc biệt quy định tại Thông tư 20/2017/TT-BYT và quy định sau:
a) Khu vực bảo quản thuốc kiểm soát đặc biệt phải có biển thể hiện rõ từng loại thuốc kiểm soát đặc biệt tương ứng.
b) Thuốc độc làm thuốc phải được bao gói đảm bảo không bị thấm và rò rỉ trong quá trình vận chuyển.
3.6. Việc bảo quản thuốc có hoạt lực mạnh (hormon sinh dục, hóa chất độc tế bào…); thuốc có tính nhạy cảm cao (kháng sinh nhóm betalactam…), các sản phẩm có nguy cơ gây cháy nổ (như các chất lỏng và chất rắn dễ bắt lửa, dễ cháy và các loại khí nén) ở các khu vực riêng biệt, có các biện pháp bảo đảm an toàn và an ninh theo đúng quy định của các văn bản quy phạm pháp luật liên quan.
3.7. Các thuốc có mùi cần được bảo quản trong bao bì kín, tại khu vực riêng, tránh để mùi hấp thụ vào các thuốc khác.
3.8. Các thuốc nhạy cảm với ánh sáng phải được bảo quản trong bao bì kín, tránh ánh sáng, trong buồng kín hoặc trong phòng tối.
3.10. Phải bố trí biện pháp cách ly vật lý giữa các khu vực biệt trữ trong kho. Các thuốc được biệt trữ ở các khu vực này phải có biển hiệu rõ ràng đối với từng tình trạng biệt trữ và chỉ những người được giao nhiệm vụ mới được phép tiếp cận khu vực này.
3.11. Phải chuyển các thuốc bị vỡ, hỏng ra khỏi kho bảo quản và để tách riêng.
Phải thu dọn các sản phẩm bị đổ vỡ, rò rỉ càng sớm càng tốt để tránh khả năng gây ô nhiễm, nhiễm chéo và gây nguy hại tới sản phẩm khác hoặc nhân viên làm việc tại khu vực đó. Phải có các quy trình bằng văn bản để xử lý các tình huống này.
Điều kiện bảo quản
3.12. Các điều kiện bảo quản thuốc phải tuân thủ theo đúng thông tin trên nhãn đã được phê duyệt hoặc công bố theo quy định.
3.14. Hướng dẫn đối với các điều kiện bảo quản:
a) Bảo quản điều kiện thường:
Bảo quản trong môi trường khô (độ ẩm 75%), ở nhiệt độ từ 15 - 30°C. Trong điều kiện thời tiết khắc nghiệt, tại một số thời điểm trong ngày, nhiệt độ có thể trên 30°C nhưng không vượt quá 32°C và độ ẩm không vượt quá 80%. Phải thoáng khí, tránh ảnh hưởng từ các mùi, các yếu tố gây tạp nhiễm và ánh sáng mạnh.
Nếu trên nhãn không ghi rõ điều kiện bảo quản thì bảo quản ở điều kiện thường.
b) Điều kiện bảo quản đặc biệt: Bao gồm các trường hợp có yêu cầu bảo quản khác với bảo quản ở điều kiện thường.
c) Hướng dẫn về điều kiện bảo quản cụ thể:

3.15. Các điều kiện bảo quản được kiểm tra vào những thời điểm phù hợp đã định trước (tối thiểu 2 lần/trong ngày). Kết quả kiểm tra phải được ghi chép và lưu hồ sơ. Hồ sơ ghi chép số liệu theo dõi về điều kiện bảo quản phải có sẵn để tra cứu.
3.16. Thiết bị theo dõi điều kiện bảo quản khi bảo quản và trong quá trình vận chuyển phải được đặt ở những khu vực/vị trí có khả năng dao động nhiều nhất được xác định trên cơ sở kết quả đánh giá độ đồng đều nhiệt độ trong kho.
3.17. Thuốc gây nghiện, thuốc hướng tâm thần, tiền chất dùng làm thuốc, nguyên liệu làm thuốc là dược chất gây nghiện, thuốc hướng tâm thần, tiền chất dùng làm thuốc phải bảo quản tại kho/tủ riêng đáp ứng quy định tại Điều 4 của Thông tư số 20/2017/TT-BYT ngày 10/5/2017 của Bộ trưởng Bộ Y tế.
- Thuốc dạng phối hợp có chứa dược chất gây nghiện, thuốc dạng phối hợp có chứa dược chất hướng tâm thần, thuốc dạng phối hợp có chứa tiền chất dùng làm thuốc phải có kho/tủ riêng hoặc khu vực riêng đáp ứng quy định tại Điều 4 của Thông tư số 20/2017/TT-BYT ngày 10/5/2017 của Bộ trưởng Bộ Y tế.
- Thuốc độc, thuốc trong Danh mục thuốc bị cấm sử dụng trong một số ngành, lĩnh vực phải được bảo quản ở khu vực riêng biệt, không được để cùng các thuốc khác, phải được bao gói đảm bảo không bị thấm và rò rỉ thuốc độc trong quá trình cấp phát.
3.18. Thuốc yêu cầu điều kiện bảo quản có kiểm soát về nhiệt độ, độ ẩm, ánh sáng (vắc xin, thuốc có yêu cầu bảo quản lạnh, âm sâu…) phải được bảo quản ở kho lạnh hoặc tủ lạnh có thể tích phù hợp. Kho lạnh hoặc tủ lạnh phải đảm bảo có nhiệt độ đồng nhất trong giới hạn bảo quản cho phép. Thiết bị theo dõi nhiệt độ phải được đặt ở những khu vực/vị trí có khả năng dao động nhiều nhất được xác định trên cơ sở kết quả đánh giá độ đồng nhất nhiệt độ trong kho/tủ lạnh; trong đó phải có ít nhất 01 thiết bị theo dõi nhiệt độ có khả năng tự động ghi lại dữ liệu nhiệt độ đã theo dõi với tần suất ghi phù hợp (thường 01 hoặc 02 lần trong khoảng thời gian 01 giờ, tùy theo mùa).
Phải có các điều kiện, phương tiện phát hiện và cảnh báo (như chuông, đèn…) kịp thời về các sự cố, sai lệch về điều kiện bảo quản (nhiệt độ, độ ẩm).
3.19. Việc đánh giá độ đồng đều nhiệt độ trong kho phải được tiến hành theo nguyên tắc được ghi tại Hướng dẫn của Tổ chức Y tế thế giới về đánh giá độ đồng đều nhiệt độ của kho bảo quản (Temperature mapping of storage areas). Kết quả đánh giá độ đồng đều nhiệt độ phải cho thấy sự đồng nhất về nhiệt độ trong toàn bộ kho bảo quản.
3.20. Việc sắp xếp vắc xin được thực hiện theo quy định tại tài liệu Hướng dẫn bảo quản vắc xin ban hành kèm theo Quyết định số 1730/QĐ-BYT ngày 16/5/2014 của Bộ trưởng Bộ Y tế quy định việc bảo quản vắc xin.
Kiểm soát và luân chuyển hàng
3.21. Phải định kỳ tiến hành việc đối chiếu thuốc trong kho để kiểm soát hạn dùng và đối chiếu so sánh thuốc hiện còn và lượng thuốc còn tồn theo phiếu theo dõi xuất nhập thuốc. Trong mọi trường hợp, việc đối chiếu phải được tiến hành khi mỗi lô thuốc được sử dụng hết.
3.22. Tất cả các sai lệch, thất thoát khi đối chiếu số lượng thuốc lưu kho phải được điều tra theo quy trình cụ thể để xác định nguyên nhân (do nhầm lẫn, do xuất nhập chưa đúng, do trộm cắp thuốc…). Sổ sách ghi chép về các cuộc điều tra này phải được lưu giữ.
3.23. Không được cấp phát các thuốc có bao bì bị hư hại, không còn nguyên vẹn, mất nhãn hoặc nhãn bị rách, không rõ ràng, hoặc có nghi ngờ về chất lượng. Trường hợp này, thủ kho phải thông báo ngay với bộ phận kiểm tra chất lượng để xem xét, đánh giá. Mọi hành động tiến hành phải được ghi chép lại.
4. Nhập hàng
4.1. Cơ sở phải có quy định và biện pháp để đảm bảo thuốc chỉ được mua từ các cơ sở cung cấp đáp ứng quy định của pháp luật và thuốc phải được cấp phép lưu hành, sử dụng.
4.2. Khi nhận thuốc, mỗi chuyến hàng cần được kiểm tra so với lệnh/đơn mua hàng, phải xác nhận tình trạng vật lý của từng thùng hàng, ví dụ: thông tin trên nhãn, số lô, loại thuốc và số lượng. Mỗi chuyến hàng phải được kiểm tra về tính đồng nhất (về ngoại quan) của các thùng thuốc theo từng lô thuốc.
4.3. Mỗi thùng thuốc phải được kiểm tra cẩn thận về khả năng bị nhiễm bẩn, tạp nhiễm, nhầm lẫn, bị can thiệp, bị hư hỏng và phải thực hiện việc cách ly để tiếp tục điều tra nếu có nghi ngờ về các khả năng này. Thủ kho phải thông báo ngay với bộ phận kiểm tra chất lượng để xem xét, đánh giá. Các hành động này phải được ghi chép lại.
Tất cả các thùng thuốc bị hư hại, mất dấu niêm phong hoặc bị nghi ngờ có tạp nhiễm thì không được nhập kho, và nếu không được huỷ bỏ ngay thì phải được bảo quản ở khu vực biệt trữ riêng, không được bán, hoặc để lẫn với các thuốc khác.
4.4. Phải có các quy định và biện pháp đảm bảo thuốc bị loại bỏ không được đưa ra sử dụng. Các sản phẩm này phải được bảo quản tách biệt khỏi các thuốc khác trong khi chờ hủy hoặc gửi trả nhà cung cấp.
5. Cấp phát
5.2. Chỉ được xuất kho, cấp phát các thuốc đạt tiêu chuẩn chất lượng, còn trong hạn sử dụng.
5.3. Các thuốc đang trong quá trình biệt trữ phải được người chịu trách nhiệm về chất lượng cho phép mới được xuất kho.
6. Hồ sơ tài liệu
6.1. Phải có các quy trình thao tác chuẩn bằng văn bản và hồ sơ ghi chép đối với tất cả các hoạt động kho, tối thiểu phải có các quy trình sau đây:
- Quy trình nhập thuốc và kiểm tra thuốc nhập kho;
- Quy trình bảo quản thuốc trong kho;
- Quy trình kiểm tra, theo dõi chất lượng thuốc trong kho;
- Quy trình vệ sinh kho;
- Quy trình kiểm tra, bảo trì, bảo dưỡng thiết bị bảo quản;
- Quy trình kiểm soát mối, mọt, côn trùng, các loài gặm nhấm trong kho;
- Quy trình xử lý thuốc bị hư hỏng, đổ vỡ;
- Quy trình theo dõi, ghi chép điều kiện bảo quản;
- Quy trình cấp phát;
- Quy trình tiếp nhận và xử lý thuốc trả về;
- Quy trình biệt trữ;
- Quy trình định kỳ đối chiếu thuốc trong kho.
6.2. Tất cả tài liệu, quy trình phải có nội dung rõ ràng, rành mạch, dễ hiểu, dễ thực hiện và phải được phê duyệt bởi người có thẩm quyền. Hồ sơ, tài liệu phải thường xuyên được rà soát và cập nhật.
6.3. Tất cả các hồ sơ, sổ sách phải lưu giữ để phục vụ việc tra cứu thuận lợi, có biện pháp đảm bảo số liệu không bị sử dụng, tiêu hủy, gây hư hỏng và/hoặc mất mát, sửa chữa một cách bất hợp pháp.
6.4. Phải tuân thủ quy định pháp luật về thời gian bảo quản hồ sơ. Trường hợp không có quy định, hồ sơ phải được lưu trữ tối thiểu thêm 01 (một) năm kể từ thời điểm hết hạn của lô thuốc.
6.5. Hồ sơ tài liệu thuốc gây nghiện, thuốc hướng tâm thần, tiền chất; thuốc dạng phối hợp có chứa dược chất gây nghiện, thuốc dạng phối hợp có chứa dược chất hướng tâm thần, thuốc dạng phối hợp có chứa tiền chất; thuốc độc, thuốc trong danh mục bị cấm sử dụng trong một số ngành, lĩnh vực theo đúng quy định tại các văn bản quy phạm có liên quan.
6.6. Khuyến khích sử dụng máy tính kết nối internet và thực hiện quản lý hoạt động bảo quản thuốc bằng phần mềm. Kết nối thông tin hoạt động nhập, cấp phát thuốc; thông tin về chất lượng thuốc từ nhà sản xuất đến bệnh nhân, cũng như việc cung cấp thông tin cho cơ quan quản lý khi được yêu cầu.
7. Thuốc trả về, thuốc bị thu hồi
7.1. Thuốc trả về phải được bảo quản tại khu riêng và dán nhãn để phân biệt. Chỉ được cấp phát quay trở lại sau khi được xem xét, đánh giá về chất lượng, đảm bảo an toàn cho người sử dụng.
7.2. Thuốc trả về sau khi được đánh giá là không đảm bảo chất lượng, không đảm bảo an toàn cho người sử dụng phải được xử lý theo quy định.
7.3. Đối với thuốc bị thu hồi theo thông báo của cơ quan quản lý, nhà sản xuất, nhà cung cấp phải ngừng cấp phát, cách ly và bảo quản ở khu vực biệt trữ, có dán nhãn phân biệt. Phải duy trì các điều kiện bảo quản cho đến khi trả lại nhà cung cấp hoặc có quyết định cuối cùng về việc xử lý.
8. Tự thanh tra
8.1. Việc tự thanh tra phải do người có chuyên môn và có thẩm quyền tiến hành một cách độc lập và chi tiết.
8.2. Kết quả của tất cả các đợt tự thanh tra phải được ghi chép. Biên bản tự thanh tra phải bao gồm tất cả các điểm phát hiện được trong đợt tự thanh tra và các đề xuất về biện pháp khắc phục, nếu có. Cần có chương trình theo dõi một cách hiệu quả. Người quản lý phải đánh giá biên bản tự thanh tra và các hồ sơ về bất kỳ hành động khắc phục nào đã được thực hiện.

I. Phân loại mức độ tồn tại
1) Tồn tại nghiêm trọng: là những sai lệch so với tiêu chuẩn GSP dẫn đến thuốc, nguyên liệu làm thuốc không đảm bảo chất lượng, an toàn, hiệu quả và gây nguy cơ ảnh hưởng nghiêm trọng đến sức khỏe, tính mạng của người sử dụng hoặc của cộng đồng; hoặc là sự kết hợp của một số tồn tại nặng cho thấy một thiếu sót nghiêm trọng của hệ thống. Nó bao gồm cả những phát hiện về gian lận, giả mạo, sửa chữa số liệu/dữ liệu.
2) Tồn tại nặng: là tồn tại không nghiêm trọng nhưng có thể dẫn đến việc bảo quản sản phẩm, nguyên liệu không tuân thủ theo hướng dẫn bảo quản của nhà sản xuất, hoặc liên quan tới một sai lệch lớn so với các quy định của GSP hoặc điều kiện bảo quản; hoặc liên quan tới việc không tuân thủ các quy trình bảo quản hoặc việc người có thẩm quyền không đáp ứng đủ yêu cầu về trách nhiệm trong công việc; hoặc tổ hợp của các tồn tại khác, không tồn tại nào trong tổ hợp đó được xem là tồn tại nặng, nhưng khi xuất hiện cùng nhau các tồn tại này sẽ tạo thành một tồn tại nặng và cần được phân tích và báo cáo như một tồn tại nặng.
3) Tồn tại nhẹ: là những tồn tại mà không xếp loại thành tồn tại nghiêm trọng hoặc tồn tại nặng, nhưng là một sai lệch so với tiêu chuẩn GSP.
II. Đánh giá mức độ tuân thủ GSP
1) Mức độ 1: Cơ sở không có tồn tại nghiêm trọng và tồn tại nặng.
2) Mức độ 2: Cơ sở không có tồn tại nghiêm trọng và có tồn tại nặng.
3) Mức độ 3: Cơ sở có tồn tại nghiêm trọng.

I - TỔNG QUAN VỀ HỒ SƠ TỔNG THỂ CỦA CƠ SỞ
Hồ sơ tổng thể (Site master file - SMF) của cơ sở bảo quản thuốc, nguyên liệu làm thuốc là tài liệu do cơ sở bảo quản soạn thảo, bao gồm thông tin cụ thể về chính sách quản lý chất lượng và các hoạt động bảo quản, kiểm soát chất lượng thuốc, nguyên liệu làm thuốc và các hoạt động khác có liên quan được thực hiện tại cơ sở.
Khi nộp lên cơ quan quản lý, SMF phải cung cấp thông tin rõ ràng về các hoạt động bảo quản của cơ sở để phục vụ cho nhiệm vụ quản lý nói chung cũng như quá trình lên kế hoạch và kiểm tra GSP một cách hiệu quả.
Một tài liệu SMF phải bao gồm đầy đủ thông tin, nhưng tốt nhất, không nên vượt quá 25-30 trang kể cả phần phụ lục kèm theo. Nên chú trọng vào các thông tin tổng quan, bản vẽ tổng thể và sơ đồ bố cục của cơ sở hơn là các nội dung mô tả bằng lời. SMF bao gồm cả các phụ lục phải được thiết kế để có thể đọc được rõ ràng khi in trên khổ giấy A4.
Hồ sơ tổng thể của cơ sở bảo quản là một phần của hệ thống hồ sơ tài liệu thuộc hệ thống quản lý chất lượng của cơ sở và cần phải được cập nhật thường xuyên. Tài liệu này phải được ghi chú rõ ràng số phiên bản, ngày hiệu lực và ngày được xem xét. SMF phải là tài liệu được xem xét định kỳ để đảm bảo thông tin cập nhật và mang tính đại diện cho các hoạt động hiện hành của cơ sở. Mỗi phụ lục có thể có ngày hiệu lực riêng để cho phép quá trình cập nhật một cách độc lập.
Lịch sử cập nhật, sửa đổi của SMF được coi là một phần của SMF, trong đó ghi tóm tắt các thay đổi của nội dung SMF và các phụ lục, đi kèm với thời gian thay đổi, lý do thay đổi.
II. NỘI DUNG HỒ SƠ TỔNG THỂ
1. Thông tin chung về cơ sở
1.1. Thông tin liên hệ của cơ sở
- Tên và địa chỉ chính thức của cơ sở;
- Tên và địa chỉ chi tiết của nhà kho nơi bảo quản thuốc và nguyên liệu làm thuốc;
- Thông tin liên lạc của cơ sở, bao gồm cả điện thoại trực 24/24 của người có trách nhiệm trong trường hợp thuốc có vi phạm hoặc phải thu hồi;
- Các thông tin định vị khác (nếu có): Tọa độ GPS, mã vùng bưu chính…
1.2. Hoạt động được cấp phép của cơ sở
- Bản sao giấy chứng nhận đủ điều kiện kinh doanh do cơ quan có thẩm quyền cấp (Phụ lục I).
- Mô tả tóm tắt các hoạt động nhập khẩu, xuất khẩu, bảo quản và các hoạt động khác đã được cho phép bởi cơ quan quản lý có thẩm quyền, bao gồm cả các hoạt động đã được cơ quan quản lý nước ngoài đánh giá, với thông tin về phạm vi chưa được ghi rõ trong giấy chứng nhận đủ điều kiện kinh doanh dược[41].
- Các loại sản phẩm, nguyên liệu được bảo quản tại địa điểm (liệt kê tại Phụ lục II) mà chưa được ghi rõ trong Phụ lục I.
- Danh mục các đợt kiểm tra GSP được tiến hành tại địa điểm trong thời gian 5 năm vừa qua, bao gồm thông tin về ngày tháng, tên của cơ quan có thẩm quyền thực hiện việc kiểm tra. Bản sao của Giấy chứng nhận GSP hiện hành (Phụ lục III), nếu có.
1.3. Các hoạt động khác được thực hiện tại cơ sở
- Mô tả các hoạt động bảo quản các sản phẩm không phải là thuốc tại địa điểm, nếu có.
2. Hệ thống quản lý chất lượng
2.1. Hệ thống quản lý chất lượng của cơ sở
- Mô tả tóm tắt hệ thống quản lý chất lượng của cơ sở, tiêu chuẩn áp dụng;
- Trách nhiệm liên quan đến việc duy trì hệ thống chất lượng, bao gồm cả việc quản lý cao cấp;
- Thông tin về các hoạt động đã được đánh giá chứng nhận, bao gồm ngày tháng và nội dung chứng nhận, tên của cơ sở cấp chứng nhận.
2.2. Quản lý các nhà cung cấp và các cơ sở hợp đồng
- Tóm tắt về cơ sở cung cấp/ hiểu biết về chuỗi cung cấp và chương trình đánh giá bên ngoài;
- Tóm tắt về hệ thống đánh giá cơ sở hợp đồng và các nhà cung cấp quan trọng khác;
- Tóm tắt về việc chia sẻ trách nhiệm giữa người hợp đồng và người nhận hợp đồng trong việc tuân thủ các quy định về đảm bảo chất lượng.
2.3. Quản lý nguy cơ về chất lượng
- Mô tả tóm tắt về phương pháp quản lý nguy cơ về chất lượng (QRM) được sử dụng tại cơ sở;
- Mục đích của QRM, bao gồm các mô tả ngắn gọn về bất kỳ hoạt động nào được thực hiện ở mức độ toàn bộ công ty/tập đoàn và hoạt động được thực hiện tại địa điểm /cơ sở. Bất cứ việc áp dụng hệ thống QRM để đánh giá sự liên tục của việc cung ứng cũng cần được chỉ rõ.
3. Nhân sự
- Sơ đồ nhân sự cần thể hiện sự sắp xếp nhân sự trong hệ thống quản lý chất lượng, các vị trí chịu trách nhiệm chính, bao gồm sự quản lý cấp cao và các nhân sự được đào tạo/ được ủy quyền;
- Số lượng nhân sự tham gia vào quá trình quản lý, bảo quản.
4. Nhà xưởng và thiết bị
4.1. Nhà xưởng
- Mô tả ngắn gọn về cơ sở: Diện tích cơ sở/khu vực bảo quản và danh sách các tòa nhà trong phạm vi cơ sở. Nếu các nhóm thuốc, nguyên liệu được phân chia bảo quản tại các tòa nhà riêng biệt trong cùng địa chỉ cơ sở thì thông tin về các tòa nhà này phải được thể hiện cùng với thông tin nhận dạng nhóm thuốc, nguyên liệu tương ứng (nếu chưa được nhận dạng như tại mục 1.1);
- Thông tin mô tả đơn giản về các khu vực bảo quản (không yêu cầu cần phải có các bản vẽ thiết kế hoặc bản vẽ kĩ thuật);
- Bản vẽ bố cục tại kho và các khu vực bảo quản, với các khu vực biệt trữ và xử lý các chất có độc tính cao, hoạt chất nguy hiểm và các nguyên liệu nhạy cảm (nếu có);
- Mô tả ngắn gọn về các điều kiện bảo quản cụ thể (nếu áp dụng) nhưng không được thể hiện trên các bản vẽ.
4.1.1. Mô tả tóm tắt về hệ thống điều hòa không khí
- Mô tả các nguyên tắc và bố trí hệ thống xử lý không khí, nhiệt độ, độ ẩm, tỷ lệ khí hồi (%).
4.1.2. Mô tả tóm tắt các hệ thống phụ trợ khác
4.2. Thiết bị
4.2.1. Thiết bị chính
Liệt kê danh mục các thiết bị chính được xác định như tại Phụ lục VIII.
4.2.2. Vệ sinh thiết bị
Mô tả ngắn gọn về các biện pháp vệ sinh, điều kiện vệ sinh (ví dụ tài liệu về hướng dẫn vệ sinh, làm sạch tại chỗ...).
4.2.3. Hệ thống máy tính
5. Hồ sơ tài liệu
- Mô tả về hệ thống hồ sơ tài liệu tại cơ sở (ví dụ hệ thống tài liệu điện tử, tài liệu bản cứng);
- Khi các tài liệu và hồ sơ được bảo quản hoặc lưu trữ bên ngoài cơ sở: Danh mục các loại tài liệu/hồ sơ; tên và địa chỉ của cơ sở lưu trữ thông tin, và tính toán khoảng thời gian cần thiết để truy xuất thông tin từ những hồ sơ tài liệu bên ngoài đó.
6. Bảo quản
6.1. Các sản phẩm, nguyên liệu được bảo quản
Có thể tham chiếu đến các Phụ lục I và Phụ lục II.
- Các dạng sản phẩm, nguyên liệu được bảo quản tại cơ sở.
- Các hoạt chất có độc tính cao hoặc hoạt chất nguy hiểm được bảo quản (chẳng hạn các hoạt chất có dược tính mạnh và/hoặc hoạt chất nhạy cảm);
6.2. Thẩm định, hiệu chuẩn
- Mô tả ngắn gọn về chính sách thực hiện thẩm định độ đồng đều nhiệt độ, độ ẩm; hiệu chuẩn các thiết bị đo, theo dõi, giám sát nhiệt độ, độ ẩm.
7. Xử lý khiếu nại, sản phẩm không đạt yêu cầu và thu hồi sản phẩm
7.1. Xử lý khiếu nại
Mô tả ngắn gọn về hệ thống xử lý các khiếu nại.
7.2. Xử lý các sản phẩm không đạt yêu cầu

Mô tả ngắn gọn về hệ thống tự thanh tra của cơ sở, trong đó tập trung vào các lĩnh vực được giám sát trong quá trình thanh tra theo kế hoạch, các quy định và hoạt động theo dõi sau thanh tra.

Các biện pháp kiểm soát thay đổi đã áp dụng đối với những thay đổi đó (tái thẩm định, thay đổi quy trình, đào tạo lại…).
6. Phụ lục đính kèm
Để cung cấp thông tin một cách đầy đủ, chi tiết, cập nhật về điều kiện hiện tại của cơ sở, chúng tôi xin gửi kèm theo báo cáo này Bản cập nhật Hồ sơ tổng thể của cơ sở.
III. KẾT LUẬN
Chúng tôi xin cam đoan toàn bộ nội dung báo cáo và các tài liệu kèm theo là đúng sự thật và hoàn toàn chịu trách nhiệm trước pháp luật về tính chính xác và trung thực của chúng.
Chúng tôi đồng ý và sẵn sàng để Bộ Y tế (Cục Quản lý Dược/Cục Quản lý Y, Dược cổ truyền) tiến hành đánh giá tại cơ sở về việc duy trì đáp ứng tiêu chuẩn GSP trong phạm vi chứng nhận mà chúng tôi đã được cấp.

Has been inspected in connection with the issuance of Pharmaceutical business license and in accordance with the national regulations at Article 33 of Pharmaceutical Law nº 105/2016/QH13 dated 06/04/2016, Article 21 of Decree nº 163/2025/NĐ-CP dated 29/6 /2025 of the Government, detailing some Articles and measures to implement the Pharmaceutical Law, Circular nº 36/2018/TT-BYT dated 22/11/2018 of the Ministry of Health on Good Storage Practice for medicinal products and materials (GSP), Circular No. 11/2025/TT-BYT dated 16/5/2025 amending and supplementing a number of articles of Circular No. 02/2018/TT-BYT dated January 22, 2018 of the Minister of Health on Good Pharmacy Practices (GPP), Circular No. 03/2018/TT-BYT dated February 9, 2018 of the Minister of Health on Good Distribution Practices for medicinal products and materials (GDP) and Circular number 36/2018/TT-BYT dated 22/11/2018 of the Ministry of Health on Good Storage Practice for medicinal products and materials (GSP).
Căn cứ kết quả đánh giá cơ sở bảo quản được thực hiện ngày , cơ sở bảo quản nêu trên được công nhận đáp ứng tiêu chuẩn Thực hành tốt bảo quản thuốc, nguyên liệu làm thuốc (GSP) theo quy định tại Thông tư số 36/2018/TT-BYT ngày 22/11/2018 của Bộ trưởng Bộ Y tế Việt Nam, phù hợp với hướng dẫn GSP của Tổ chức Y tế thế giới.
From the knowledge gained during inspection of this establisment, which was conducted on ………, it is considered that it complies with the requirements of Good Storage Practice for medicinal products and medicinal materials laid down in Circular nº 36/2018/TT-BYT dated 22/11/2018 by Vietnam Ministry of Health, which is harmonized with the requirements of GSP guideline of World Health Organization.
Giấy chứng nhận này thể hiện tình trạng tuân thủ GSP của cơ sở bảo quản tại thời điểm đánh giá nêu trên và có hiệu lực không quá 3 năm kể từ ngày đánh giá. Tuy nhiên, căn cứ theo nguyên tắc quản lý rủi ro, thời gian hiệu lực của Giấy chứng nhận có thể được rút ngắn hoặc kéo dài và sẽ được ghi tại mục Những nội dung hạn chế hoặc làm rõ.
This certificate reflects the status of the warehousing site at the time of the inspection noted above and should not be relied upon to reflect the compliance status if more than three years have elapsed since the date of that inspection. However, this period of validity maybe reduced or extended using regulatory risk management principles by an entry in the Restrictions or Clarifying remarks field.
Giấy chứng nhận chỉ có hiệu lực khi thể hiện đầy đủ các trang và bao gồm cả Phần 1 và Phần 2.
This certificate is valid only when presented with all pages and both Part1 and Part 2.
Tính xác thực của Giấy chứng nhận này có thể được xác nhận thông qua nội dung đăng tải trên Trang thông tin điện tử của Cục Quản lý Dược. Nếu không có, hãy liên hệ với Cục Quản lý Dược để được làm rõ.

(2) Áp dụng cho trường hợp có thay thế Giấy chứng nhận GSP.

[1] Thông tư số 29/2020/TT-BYT ngày 31 tháng 12 năm 2020 có hiệu lực kể từ ngày 15 tháng 02 năm 2021 có căn cứ ban hành như sau:

Căn cứ Nghị định số 54/2017/NĐ-CP ngày 08 tháng 5 năm 2017 của Chính phủ quy định chi tiết một số điều và biện pháp thi hành Luật dược;
Căn cứ Nghị định số 69/2018/NĐ-CP ngày 15 tháng 5 năm 2018 của Chính phủ quy định chi tiết một số điều của Luật quản lý ngoại thương;
Căn cứ Nghị định số 96/2012/NĐ-CP ngày 15 tháng 11 năm 2012 của Chính phủ quy định về điều trị nghiện các chất dạng thuốc phiện bằng thuốc thay thế;
Căn cứ Nghị định số 15/2018/NĐ-CP ngày 02 tháng 02 năm 2018 của Chính phủ quy định chi tiết thi hành một số điều của Luật an toàn thực phẩm;
Căn cứ Nghị định số 34/2016/NĐ-CP ngày 14 tháng 5 năm 2016 của Chính phủ quy định chi tiết một số điều và biện pháp thi hành Luật ban hành văn bản quy phạm pháp luật;
Căn cứ Nghị định số 75/2017/NĐ-CP ngày 20 tháng 6 năm 2017 của Chính phủ quy định chức năng, nhiệm vụ, quyền hạn và cơ cấu tổ chức của Bộ Y tế;
Theo đề nghị của Vụ trưởng Vụ Pháp chế,
Bộ trưởng Bộ Y tế ban hành Thông tư sửa đổi, bổ sung và bãi bỏ một số văn bản quy phạm pháp luật do Bộ trưởng Bộ Y tế ban hành, liên tịch ban hành.”.
Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025 có hiệu lực kể từ ngày 01 tháng 7 năm 2025 có căn cứ ban hành như sau:
“Căn cứ Luật Dược ngày 06 tháng 4 năm 2016;
Căn cứ Luật sửa đổi, bổ sung một số điều của Luật dược ngày 21 tháng 11 năm 2024;
Căn cứ Nghị định số 42/2025/NĐ-CP ngày 27 tháng 02 năm 2025 của Chính phủ quy định chức năng, nhiệm vụ, quyền hạn và cơ cấu tổ chức của Bộ Y tế;
Theo đề nghị của Cục trưởng Cục Quản lý Dược, Cục trưởng Cục Quản lý Y, Dược cổ truyền;
Bộ trưởng Bộ Y tế ban hành Thông tư sửa đổi, bổ sung một số điều của Thông tư số 02/2018/TT-BYT ngày 22 tháng 01 năm 2018 của Bộ trưởng Bộ Y tế quy định về thực hành tốt cơ sở bán lẻ thuốc, Thông tư số 03/2018/TT-BYT ngày 09 tháng 02 năm 2018 của Bộ trưởng Bộ Y tế quy định về thực hành tốt phân phối thuốc, nguyên liệu làm thuốc, Thông tư số 36/2018/TT-BYT ngày 22 tháng 11 năm 2018 của Bộ trưởng Bộ Y tế quy định về thực hành tốt bảo quản thuốc, nguyên liệu làm thuốc.”
[2] Cụm từ “Cơ sở bảo quản vắc xin trong chương trình tiêm chủng mở rộng quốc gia tuyến huyện” được thay thế bằng cụm từ “cơ quan được giao quản lý về tiêm chủng cấp xã” theo quy định tại điểm b khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[3] Khoản này được sửa đổi theo quy định tại điểm a khoản 1 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[4] Khoản này được bổ sung theo quy định tại điểm b khoản 1 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[5] Khoản này được sửa đổi, bổ sung theo quy định tại khoản 2 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[6] Khoản này được sửa đổi, bổ sung theo quy định tại khoản 7 Điều 1 Thông tư số 29/2020/TT-BYT ngày 31 tháng 12 năm 2020, có hiệu lực kể từ ngày 15 tháng 02 năm 2021.
[7] Khoản này được sửa đổi theo quy định tại điểm a khoản 3 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[8] Điểm này được sửa đổi, bổ sung theo quy định tại điểm b khoản 3 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[9] Cụm từ “Cơ sở bảo quản vắc xin trong chương trình tiêm chủng mở rộng quốc gia tuyến huyện” được thay thế bằng cụm từ “cơ quan được giao quản lý về tiêm chủng cấp xã” theo quy định tại điểm b khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[10] Cụm từ “Sở Y tế tỉnh, thành phố trực thuộc Trung ương” được thay thế bằng cụm từ “cơ quan chuyên môn về y tế của tỉnh” theo quy định tại điểm c khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025; cụm từ “(sau đây được gọi tắt là Sở Y tế)” được bãi bỏ theo quy định tại điểm d khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[11] Cụm từ “Sở Y tế” được thay thế bằng cụm từ “cơ quan chuyên môn về y tế của tỉnh” theo quy định tại điểm c khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[12] Cụm từ “Sở Y tế” được thay thế bằng cụm từ “cơ quan chuyên môn về y tế của tỉnh” theo quy định tại điểm c khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[13] Cụm từ “Sở Y tế” được thay thế bằng cụm từ “cơ quan chuyên môn về y tế của tỉnh” theo quy định tại điểm c khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[14] Cụm từ “Cơ sở bảo quản vắc xin trong chương trình tiêm chủng mở rộng quốc gia tuyến huyện” được thay thế bằng cụm từ “cơ quan được giao quản lý về tiêm chủng cấp xã” theo quy định tại điểm b khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[15] Cụm từ “Sở Y tế” được thay thế bằng cụm từ “cơ quan chuyên môn về y tế của tỉnh” theo quy định tại điểm c khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[16] Điều 3 của Thông tư số 29/2020/TT-BYT ngày 31 tháng 12 năm 2020 quy định như sau:
“Điều 3. Hiệu lực thi hành
1. Thông tư này có hiệu lực thi hành từ ngày 15 tháng 02 năm 2021.
2. Riêng các quy định tại các khoản 5, 6, 7, 8 và 11 Điều 1 Thông tư này có hiệu lực từ ngày 01 tháng 01 năm 2021.
3. Các quy định liên quan đến nộp hồ sơ, tài liệu và tra cứu trực tuyến được áp dụng trong giai đoạn dịch Covid - 19 cho đến thời điểm Bộ Y tế xem xét, điều chỉnh phù hợp với yêu cầu thực tiễn.”
Điều 5 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025 quy định như sau:
“Điều 5. Hiệu lực thi hành
1. Thông tư này có hiệu lực kể từ ngày 01 tháng 07 năm 2025.
2. Các quy định về việc cơ sở kinh doanh dược liên thông và cập nhật đầy đủ dữ liệu với hệ thống thông tin về dược theo hướng dẫn của Bộ Y tế thực hiện từ ngày 01 tháng 01 năm 2026”.
[17] Cụm từ “Sở Y tế” được thay thế bằng cụm từ “cơ quan chuyên môn về y tế của tỉnh” theo quy định tại điểm c khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[18] Cụm từ “Sở Y tế” được thay thế bằng cụm từ “cơ quan chuyên môn về y tế của tỉnh” theo quy định tại điểm c khoản 9 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.
[19] Điều 4 của Thông tư số 29/2020/TT-BYT ngày 31 tháng 12 năm 2020 quy định như sau:
“Điều 4. Quy định chuyển tiếp
1. Các hồ sơ đã nộp cho cơ quan tiếp nhận hồ sơ trước ngày Thông tư này có hiệu lực nhưng đang trong quá trình giải quyết được áp dụng theo quy định có liên quan tại Thông tư này hoặc các quy định trước ngày Thông tư này có hiệu lực theo hướng thuận tiện cho doanh nghiệp, tổ chức, cá nhân.
2. Các quy định về công bố thông tin, cập nhật, khai báo và báo cáo theo hình thức trực tuyến tại Thông tư này được áp dụng theo triển khai của cơ quan nhà nước có thẩm quyền”.
Điều 6 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025 quy định như sau:
“Điều 6. Điều khoản chuyển tiếp
Hồ sơ đã nộp trước ngày Thông tư này có hiệu lực thi hành được tiếp tục thực hiện theo quy định tại thời điểm nộp hồ sơ hoặc áp dụng quy định tại Thông tư này kể từ ngày 01 tháng 7 năm 2025 theo hướng thuận tiện, đơn giản hoá thủ tục hành chính cho doanh nghiệp, tổ chức, cá nhân”.
[20] Khoản này được bãi bỏ theo quy định tại khoản 4 Điều 3 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025, có hiệu lực kể từ ngày 01 tháng 7 năm 2025.

[22] Điều 5 của Thông tư số 29/2020/TT-BYT ngày 31 tháng 12 năm 2020 quy định như sau:
“Điều 5. Trách nhiệm thi hành
Vụ trưởng Vụ Pháp chế, Chánh Văn phòng Bộ, Chánh Thanh tra Bộ, Vụ trưởng, Cục trưởng, Tổng Cục trưởng các Vụ, Cục, Tổng cục thuộc Bộ Y tế và các cơ quan, tổ chức, cá nhân có liên quan chịu trách nhiệm thi hành Thông tư này”.
Điều 7 Thông tư số 11/2025/TT-BYT ngày 16 tháng 5 năm 2025 quy định như sau:
“Điều 7. Trách nhiệm thi hành
1. Cục Quản lý Dược có trách nhiệm:
a) Chủ trì, phối hợp với các đơn vị liên quan tổ chức phổ biến Thông tư này;
b) Làm đầu mối, phối hợp với các đơn vị có liên quan hướng dẫn triển khai Thông tư cho cơ quan chuyên môn về y tế của tỉnh, Y tế ngành và cơ sở kinh doanh dược thuộc phạm vi chức năng, nhiệm vụ được giao;
c) Thực hiện kiểm tra việc tuân thủ các quy định của Thông tư này và xử lý hoặc kiến nghị xử lý vi phạm theo thẩm quyền.
2. Cục Quản lý Y, Dược cổ truyền có trách nhiệm:
a) Làm đầu mối, phối hợp với các đơn vị có liên quan hướng dẫn triển khai Thông tư cho cơ quan chuyên môn về y tế của tỉnh, Y tế ngành và cơ sở phân phối thuốc cổ truyền, dược liệu thuộc phạm vi chức năng, nhiệm vụ được giao;
b) Thực hiện kiểm tra việc tuân thủ các quy định của Thông tư này và xử lý hoặc kiến nghị xử lý vi phạm theo thẩm quyền.
3. Ủy ban nhân dân cấp tỉnh có trách nhiệm chỉ đạo các cơ quan thuộc và trực thuộc kiểm tra việc tuân thủ các nguyên tắc, tiêu chuẩn thực hành tốt của các cơ sở kinh doanh dược trên địa bàn và chỉ đạo cơ quan chuyên môn về y tế của tỉnh thực hiện các công việc sau đây:
a) Phối hợp với các đơn vị liên quan tổ chức phổ biến Thông tư này và hướng dẫn triển khai cho các đơn vị trên địa bàn;
b) Thực hiện kiểm tra việc tuân thủ các quy định của Thông tư này và xử lý vi phạm theo thẩm quyền.
4. Cơ sở kinh doanh dược có trách nhiệm:
a) Tổ chức nghiên cứu, triển khai việc thực hiện quy định tại Thông tư này;
b) Bảo đảm tuân thủ tiêu chuẩn thực hành tốt trong suốt quá trình hoạt động của cơ sở;
c) Thực hiện hoạt động kinh doanh thuốc, nguyên liệu làm thuốc theo đúng phạm vi được cấp phép trên cơ sở tuân thủ các quy định của pháp luật.
Trong quá trình thực hiện, nếu có khó khăn, vướng mắc đề nghị các cơ quan, tổ chức, cá nhân phản ánh về Bộ Y tế (Cục Quản lý Dược) để xem xét, giải quyết”.
