Based on the Statistics Law dated June 17, 2003;
Pursuant to Decree No. 40/2004/NĐ-CP dated February 13, 2004 of the Government detailing and guiding the implementation of certain provisions of the Law on Statistics;
Pursuant to the Government Decree No. 03/2010/NĐ-CP dated January 13, 2010 on the tasks, powers, and organization of statistics for ministries, ministerial-level agencies, and government agencies;
This Circular prescribes procedures for receiving, providing health care for domestic violence victims and statistical reports on domestic violence victims at medical facilities.
Pursuant to Decision No. 43/2010/QĐ-TTg dated June 2, 2010 of the Prime Minister on the issuance of the Statistical Indicators System;
Considering the proposal of the Director of the Department of Planning and Finance;
The Minister of Health issues this Circular to stipulate the Medical Statistics Form System applicable to provincial, district, and commune health facilities.
Article 1. Medical Statistics Form System
Attached to this Circular is the Medical Statistics Form System for provincial, district, and commune levels, including:
1. Record books for health service provision and health management activities at commune health centers, including: 12 record books and 01 form (Annex 1);
2. Medical statistics reporting forms for communes, wards, and towns (hereinafter referred to as communes), including: 10 forms (Annex 2).
3. Medical statistics reporting forms for districts, cities under provinces, and towns (hereinafter referred to as districts), including: 16 forms (Annex 3).
4. Medical statistics reporting forms for provinces and centrally-administered municipalities (hereinafter referred to as provinces), including: 18 forms (Annex 4).
Article 2. Reporting System
1. Reporting periods and methods shall be implemented in accordance with Decision No. 15/2014/QĐ-TTg of the Prime Minister dated February 17, 2014 on the issuance of the Reporting System for Comprehensive Statistical Reports applicable to Ministries and sectors.
2. Reporting procedures:
a) Commune level:
- Reporting unit: Commune Health Station;
- Receiving unit: The central unit at the district level assigned by the Provincial Department of Health;
b) District level:
- Reporting unit: The central unit at the district level assigned by the Provincial Department of Health;
- Receiving unit: Provincial Department of Health;
c) Provincial level:
- Reporting unit: Provincial Department of Health;
- Receiving unit: Ministry of Health (Department of Planning and Finance).
Article 3. Implementation Organization
1. The Provincial Department of Health shall be responsible for assigning a district health unit to serve as the central unit to implement the reporting system for comprehensive statistical reports on all health activities within the district.
2. The Department of Planning and Finance, Ministry of Health shall be responsible for guiding, inspecting, and supervising the recording of forms, data collection methods, and report compilation to ensure uniformity nationwide.
Article 4. Effective date
This Circular takes effect from October 1, 2014.
Decision No. 3440/QĐ-BYT dated September 17, 2009 of the Minister of Health on the issuance of the Medical Statistics Books and Forms System shall cease to be effective from the date this Circular takes effect.
Article 5. Responsibilities for Implementation
The Director of the Ministry's Office, Heads of the Department of Planning and Finance, Heads of Departments, Directors of Bureaus, General Directors of General Departments under the Ministry of Health, Heads of agencies and units directly under the Ministry, Provincial Department of Health Directors of centrally-administered municipalities, and Heads of health agencies of Ministries and sectors are responsible for implementing this Circular. In the course of implementation, if there are difficulties or obstacles, units and localities shall report to the Ministry of Health (Department of Planning and Finance) for study and resolution.
