Article 24Prescribing forms and reporting systems for health statistics applicable to private healthcare facilities

Based on the Statistics Law dated June 17, 2003;
Pursuant to the Law on Medical Examination and Treatment dated November 23, 2009;
Pursuant to Decree No. 40/2004/NĐ-CP dated February 13, 2004 of the Government detailing and guiding the implementation of certain provisions of the Law on Statistics;
Pursuant to the Government Decree No. 03/2010/NĐ-CP dated January 13, 2010 on the tasks, powers, and organization of statistics for ministries, ministerial-level agencies, and government agencies;
This Circular prescribes procedures for receiving, providing health care for domestic violence victims and statistical reports on domestic violence victims at medical facilities.
Pursuant to Decision No. 43/2010/QĐ-TTg dated June 2, 2010 of the Prime Minister on the issuance of the Statistical Indicators System;.
Considering the proposal of the Director of the Department of Planning and Finance;
The Minister of Health issues this Circular prescribing forms and reporting systems for health statistics applicable to private healthcare facilities.
Article 1. Scope of Regulation
Issuing health statistical forms regarding the activities of examination and treatment at private healthcare facilities, including:
1. Initial recording forms, comprising: 6 forms (Annex 1).
2. Reporting forms for private health examination and treatment statistics, comprising: 6 forms (Annex 2).
3. Reporting forms for health statistics of healthcare facilities with hospital beds, comprising: 7 forms (Annex 3).
4. Comprehensive reporting forms for health examination and treatment activities of private healthcare facilities, comprising: 7 forms (Annex 4).
Article 2. Reporting Systems
1. Reporting periods and methods shall be implemented in accordance with Decision No. 15/2014/QĐ-TTg dated February 17, 2014 on the issuance of the Reporting System for comprehensive statistics applicable to Ministries and sectors.
2. Reporting procedures:
a) Private healthcare facilities without hospital beds shall record initial data in the forms prescribed in Annex 1 attached hereto immediately after providing services and compile reports according to the forms prescribed in Annex 2 attached hereto to send to the District Health Department where the facility is located;
b) Private healthcare facilities with hospital beds shall compile reports according to the forms prescribed in Annex 3 attached hereto to the Provincial Health Department;
c) The District Health Department shall aggregate data according to the forms prescribed in Annex 4 attached hereto to report to the Provincial Health Department;
d) The Provincial Health Department shall aggregate data according to the forms prescribed in Annex 4 of Circular No. 27/TT-BYT dated August 14, 2014 on the Statistical Forms System applicable to provincial, district, and commune-level healthcare facilities to report to the Ministry of Health (Department of Planning and Finance);
Article 3. Responsibilities for Implementation
1. The Provincial Health Department is the lead agency at the local level responsible for collecting and aggregating statistical information within its jurisdiction.
2. The Department of Planning and Finance, Ministry of Health shall be responsible for guiding, inspecting, and supervising the implementation of form recording, data collection, and reporting methods to ensure uniformity nationwide.
Article 4. Effective date
This Circular takes effect from October 1, 2014.
State-owned enterprises that have been assigned by the Ministry of Agriculture and Rural Development to conduct offshore wind power project surveys before the effective date of this Circular shall continue to implement according to the assigned documents; any new matters arising after the effective date of this Circular shall be implemented in accordance with the provisions of this Circular.
The Director of the Ministry's Office, Heads of the Department of Planning and Finance, Departments, Bureaus, General Administrations under the Ministry of Health, Heads of units directly under the Ministry, Directors of Provincial Health Departments, and Heads of health agencies under Ministries and sectors are responsible for implementing this Circular. In the course of implementation, if there are difficulties or obstacles, units and localities shall report to the Ministry of Health (Department of Planning and Finance) for study and resolution.
