CIRCULAR
Issuing the basic health statistics index catalog applicable to provincial, district, and commune levels

THE MINISTER OF HEALTH
Pursuant to the Government Decree No. 03/2010/NĐ-CP dated January 13, 2010 on the tasks, powers, and organization of statistics for ministries, ministerial-level agencies, and government agencies;
The Government Decree No. 63/2012/NĐ-CP dated August 31, 2012 on the functions, tasks, powers, and organizational structure of the Ministry of Health;
Pursuant to Decision No. 43/2010/QĐ-TTg dated June 2, 2010 of the Prime Minister on issuing the system of statistical indices;
Considering the proposal of the Director of the Department of Planning and Finance.
The Minister of Health issues this Circular on issuing the basic health statistics index catalog applicable to provincial, district, and commune levels.
Article 1. Basic health statistics index catalog applicable to provincial, district, and commune levels
Attached to this Circular is the Basic Health Statistics Index Catalog applicable to provincial, district, and commune levels.
Article 2. Implementation
1. The Department of Planning and Finance, Ministry of Health shall be responsible for guiding, inspecting, and supervising the implementation of data collection and calculation of indices at all levels to ensure uniformity nationwide.
2. The Department of Health shall assign a health unit at the district level to be the focal point for collecting and calculating the assigned indices according to this Circular.
Article 3. Effectiveness
This Circular takes effect from November 20, 2014.
Article 4. Responsibilities for Implementation
The Permanent Deputy Minister of Health, the Inspector General of the Ministry of Health, heads of departments, bureaus, general administrations under the Ministry of Health, and directors of provincial health departments are responsible for implementing this Circular.
In the course of implementation, if there are difficulties or obstacles, units and localities should report to the Ministry of Health (Department of Planning and Finance) for study and resolution./.
