CIRCULAR
Amending and Supplementing Certain Articles of Circular No. 31/2023/TT-BCA
dated July 20, 2023 by the Minister of Public Security on the model passport, travel document, and related forms, which has been amended and supplemented by Circular No. 68/2025/TT-BCA dated July 31, 2025
issued by the Minister of Public Security
Based on Law No. 49/2019/QH14 on Exit and Entry of Vietnamese Citizens, amended and supplemented by Laws Nos. 23/2023/QH15 and 118/2025/QH15;
Based on Law No. 26/2023/QH15 on Identity Documents, amended and supplemented by Law No. 118/2025/QH15;
Based on Decree No. 02/2025/NĐ-CP dated February 18, 2025 of the Government on the functions, tasks, powers, and organizational structure of the Ministry of Public Security, amended and supplemented by Decree No. 11/2025/NĐ-CP dated July 1, 2025;
In accordance with the proposal of the Director of the Immigration Administration Bureau;
The Minister of Public Security promulgates this Circular amending and supplementing certain articles of Circular No. 31/2023/TT-BCA dated July 20, 2023 by the Minister of Public Security on the model passport, travel document, and related forms,, which has been amended and supplemented by Circular No. 68/2025/TT-BCA dated July 31, 2025.
Article 1. Replace the forms annexed to Circular No. 31/2023/TT-BCA dated July 20, 2023 by the Minister of Public Security on the model passport, travel document, and related forms
1. Replace Form Application for Domestic Ordinary Passport for Individuals Aged 14 or Older (Model TK01) with the Form Application for Domestic Ordinary Passport for Individuals Aged 14 or Older (Model TK01) annexed to this Circular.
2. Replace Form Application for Domestic Ordinary Passport for Individuals Under 14 Years of Age (Model TK01a) with the Form Application for Domestic Ordinary Passport for Individuals Under 14 Years of Age (Model TK01a) annexed to this Circular.
3. Replace Form Application for Overseas Ordinary Passport for Individuals Aged 14 or Older (Model TK02) with the Form Application for Overseas Ordinary Passport for Individuals Aged 14 or Older (Model TK02) annexed to this Circular.
4. Replace Form Application for Overseas Ordinary Passport for Individuals Under 14 Years of Age (Model TK02a) with the Form Application for Overseas Ordinary Passport for Individuals Under 14 Years of Age (Model TK02a) annexed to this Circular.
5. Replace Form Application for Confirmation of Identity for Vietnamese Citizens Abroad Applying for a Passport (Model TK03) with the Form Application for Confirmation of Identity for Vietnamese Citizens Abroad Applying for a Passport (Model TK03) annexed to this Circular.
6. Replace Form Application for Restoration of Validity of Ordinary Passport (Model TK04) with the Form Application for Restoration of Validity of Ordinary Passport (Model TK04) annexed to this Circular.
7. Replace Form Report Lost Ordinary Passport (Model TK05); with the Form Report Lost Ordinary Passport (Model TK05) annexed to this Circular.
8. Replace Form Statement on Failure to Collect Passport (Model TK06) with the Form Statement on Failure to Collect Passport (Model TK06) annexed to this Circular.
9. Replace Form Notification of Transfer of Lost Ordinary Passport Report (Model VB01) with the Form Notification of Transfer of Lost Ordinary Passport Report (Model VB01) annexed to this Circular.
Article 2. Scope of Application and Transitional Provisions
1. This Circular shall take effect from July 1, 2026.
2. In the case where citizens have submitted an application for issuance, cancellation, restoration, or reporting loss of a national identity card prior to the effective date of this Circular and have not yet received a decision, they are not required to submit the new form.
3. The Bureau of Immigration and Emigration shall be responsible for guiding, monitoring, inspecting, and urging the implementation of this Circular.
4. Heads of units under the Ministry of Public Security, Directors of Provincial and Municipal Public Security Bureaus shall be responsible for implementing this Circular.
In the course of implementation, if any difficulties arise, public security organs at all levels shall report to the Ministry of Public Security (through the Bureau of Immigration and Emigration) for timely guidance.
