Circular No. 02/2020/TT-BTP dated April 8, 2020, issued by the Minister of Justice to issue, guide the use, and manage the templates for nationality books and nationality-related documents, which took effect from May 24, 2020, was amended and supplemented by:
Circular No. 04/2024/TT-BTP dated June 6, 2024, issued by the Minister of Justice to amend and supplement some articles of Circular No. 02/2020/TT-BTP dated April 8, 2020, issued by the Minister of Justice to issue, guide the use, and manage the templates for nationality books and nationality-related documents, and Circular No. 04/2020/TT-BTP dated May 28, 2020, issued by the Minister of Justice detailing the implementation of certain provisions of the Household Registration Law and Decree No. 123/2015/NĐ-CP dated November 15, 2015, of the Government detailing certain provisions and measures to implement the Household Registration Law, which took effect from June 6, 2024.

The Minister of Justice issues this Circular on issuing, guiding the use, and managing the templates for nationality books and nationality-related documents[1].
Article 1. Scope of Regulation
This Circular issues, guides the use, and manages the templates for the Record Book of Nationality Case Files, the Register Book for Determination of Vietnamese Nationality, the Issue Book for Certificate of Vietnamese Nationality, and the Issue Book for Certificate of Vietnamese Origin (hereinafter referred to collectively as the Nationality Books) and the templates for nationality-related documents.
Article 2. Subjects using nationality registers and nationality documents
1. Vietnamese citizens and foreigners who have needs to request resolution of matters related to nationality;
2. Diplomatic missions of the Socialist Republic of Vietnam abroad[2], State Committee for Overseas Vietnamese;
3. Departments of Justice of provinces and centrally governed cities;
4. People's Committees of provinces and centrally governed cities;
5. Ministry of Foreign Affairs;
6. Ministry of Justice.

Article 5. Printing and using templates for Nationality Books and nationality-related documents
1. Templates for Nationality Books and nationality-related documents issued according to the List in Article 3 and Article 4 of this Circular are published on the Ministry of Justice’s website (at the address: www.moj.gov.vn). Authorities with jurisdiction over the handling of nationality case files may access and print them for use.
2. Persons requesting resolution of nationality matters (hereinafter referred to collectively as persons making requests) may access the Ministry of Justice’s website to print and use the templates for Applications, Biographical Forms, and Application Forms. In cases where persons making requests cannot print themselves, the authority with jurisdiction over the handling of the case file shall be responsible for printing and providing them free of charge to persons making requests.
Article 6. Principles for Using the Nationality Register
1. Civil servants handling nationality affairs at the Department of Justice, diplomatic officials, consular officials handling nationality affairs at the agencies representing the Socialist Republic of Vietnam abroad,[8]the State Committee on Vietnamese Citizens Abroad (hereinafter referred to collectively as civil servants handling nationality affairs) must personally record in the Nationality Register. The recorded content must be accurate; handwriting must be clear, complete, without abbreviations, erasures; do not use multiple ink colors in one register, do not use red ink.
In cases where information technology is applied to print the Nationality Register on a machine, it must be printed with good quality black ink that does not smudge, lose clarity, or fade.
In cases where nationality matters are handled through a single-window mechanism, the civil servant receiving files at the single-window unit is responsible for recording in the Register of File Receipts for nationality matters.
2. The Nationality Register must be stamped across from the first page to the last page immediately upon opening the register. Entries must be made consecutively page by page, without leaving any blank pages. In cases where information technology is applied, annually, printing and binding into a register and stamping across from the first page to the last page must be done periodically.
3. The Register of File Receipts for nationality matters (Model TP/QT-2024-STLHSQT)[9]is used to record applications for acquisition, reacquisition, and renunciation of Vietnamese nationality.
Article 7. Principles for Using Forms Related to Nationality
1.[10] Depending on each type of matter, individuals requesting resolution of nationality matters must fill out all relevant information accurately according to the columns and sections in the Application Form, Personal History Statement, and Declaration Form as specified in each form issued along with this Circular.
2.[11] The file receiver shall extract information from the National Population Database in accordance with the law if the individual requesting resolution of nationality matters has provided their surname, middle name, given name; date of birth; personal identification number/citizen identity card/citizen card/id card. If the required information is not available in the National Population Database, the individual requesting resolution of nationality matters shall be asked to provide it fully.
3. Individuals making requests are responsible for the information they have recorded in the Application Form, Personal History Statement, and Declaration Form; in cases of intentional misrepresentation of information, they will be dealt with according to the provisions of the law depending on the severity.
Article 8. Statistics of Matters Handled and Resolved in a Year; Archiving the Nationality Register
1. At the end of the year, the person who has recorded in the nationality register must clearly tally the total number of cases recorded in the register within one year; if multiple registers are used in one year, the total number of volumes and the number of cases recorded must be noted; sign, write clearly the full name, and report to the head of the agency for confirmation and stamping.
2. The Nationality Register must be permanently archived and preserved in accordance with the law on archiving.
Article 9. Effective Date[12]
This Circular takes effect from May 24, 2020 and replaces Circular No. 08/2010/TT-BTP dated March 25, 2010 of the Ministry of Justice guiding forms related to nationality and the model of the Register of File Receipts for nationality matters./.

[12] Article 3 and Article 4 of Circular No. 04/2024/TT-BTP amending and supplementing certain provisions of Circular No. 02/2020/TT-BTP dated April 8, 2020 issued by the Minister of Justice guiding the use and management of nationality record books and nationality-related documents, and Circular No. 04/2020/TT-BTP dated May 28, 2020 issued by the Minister of Justice detailing the implementation of certain provisions of the Household Registration Law and Decree No. 123/2015/NĐ-CP dated November 15, 2015 of the Government detailing certain provisions and measures to implement the Household Registration Law, shall take effect from June 6, 2024, and are stipulated as follows:
"Article 3. Transitional Provisions
1. Nationality files and household registration registration files received before the effective date of this Circular that have not yet been resolved shall continue to be resolved according to the provisions of Circular No. 02/2020/TT-BTP and Circular No. 04/2020/TT-BTP.
2. Nationality record book models issued together with Circular No. 02/2020/TT-BTP shall continue to be used until December 31, 2024.
Article 4. Responsibilities for Implementation and Effectiveness
1. This Circular shall take effect from June 6, 2024.
2. The Director of the Household Registration, Nationality, and Notarization Department, Heads of units under the Ministry of Justice, Directors of Justice Departments of provinces and centrally-administered cities, and relevant agencies, organizations, and individuals shall be responsible for implementing this Circular."
